their respective obligations of banking secrecy under Brazilian law for purposes of providing any and all

representante do Investidor baseia-se nas declarações feitas pelo Agente e pelo Investidor nos termos
deste Formulário. O Investidor indenizará e manterá o Representante indene de todas e quaisquer
despesas (incluindo honorários advocatícios), custos, penalidades, perdas, danos, decisões judiciais,
processos e quaisquer outras responsabilidades, de qualquer natureza, incorridas pelo Representante
em razão de qualquer declaração ou garantia incorreta, enganosa, falsa ou imprecisa ora feita pelo
Investidor, ou do descumprimento de promessas feitas neste Formulário.
The Investor acknowledges and agrees that the Representative's agreement to act as representative of the
Investor as set forth herein is based upon the representations and warranties made by the Agent and the Investor
pursuant to this Investor Form. The Investor shall indemnify and hold completely harmless the Representative
from any and all expenses, (including attorney´s fees), costs, penalties, losses, damages, judgements, suits, or
any other liabilities whatsoever incurred by the Representative due to any incorrect, misleading, untrue or
inaccurate representation and warranty so made by the Investor or its breach of covenants hereunder.
O Investidor obriga-se a manter o Representante indene de quaisquer responsabilidades por danos
decorrentes da origem ou natureza dos bens, direitos ou valores ingressados no Brasil.
The Investor agrees to hold the Representative harmless from any liability for damages arising out of the origin or
nature of the assets, rights or amounts entering into Brazil.
O Investidor compromete-se a indenizar o Representante por quaisquer danos ou prejuízos, bem como
ressarcir quaisquer despesas (incluindo custas e honorários advocatícios), que o Representante venha
a ter em razão das operações realizadas pelo Investidor no Brasil.
The Investor agrees to indemnify the Representative for any damages or losses, as well as to reimburse any
expenses (including court costs and fees of counsel) which the Representative may incur as a result of the
transactions carried out by the Investor in Brazil.

documentos e informações do Investidor e do seu Portfólio para outras instituições do mesmo grupo

and information of the Investor and the Investor’s Portfolio to other institutions of the same economic group of Itaú.

behalf of the Investor are personally known to it and that such person(s) have the legal capacity and full power
required for the execution of this Investor Form on behalf of the Investor, and (ii) the signatures of the persons

VII – ADESÃO AO CONTRATO DE REPRESENTAÇÃO PARA INVESTIDOR NÃO RESIDENTE E AO

Unibanco S.A., como representante e Itaú DTVM S.A., como custodiante (“Custodiante”).
The Investor (participating in an omnibus account) hereby adheres to the Representative Services Agreement and
to the Portfolio Custody Agreement entered into by and between the omnibus account holder, Clearstream
Banking S.A. (the “Agent”), and, respectively, Itaú Unibanco S.A., as representative and Itaú DTVM S.A., as
custodian (“Custodian”).
O Investidor declara ter recebido do Agente e do Custodiante, conforme aplicável, cópias dos aludidos
contratos, não tendo dúvida sobre suas cláusulas, e obriga-se a cumprir integralmente os termos e
condições ali previstos.

mentioned agreements, as well as to fully understand them, and agrees to fully comply with the terms and
