(in line with §1 Abs. 1 Nr. 4 Jugendschutzgesetz)
IMPORTANT:
1. The form has to be at hand, filled in and signed in its original form (no copy);
2. the designated guardian has to be of legal age (18+);
3. the designated guardian has to be mature/adult enough to support your child
appropriately when required in every situation;
4. a safe way home for your child has to be guaranteed;
5. the designated guardian cannot be under the influence of alcohol or other drugs while
accompanying your child.

documents (including this one) will be reported to the police. We reserve the right to check for the documents mentioned
(including this one) any time, and to deny any minor’s admission/entrance in case of these documents not being at hand.
