Scroll down for original press release in Portuguese Dry eye can be caused by chronic diseases, eye disorders and environmental conditions It is estimated that about 7 million Brazilian adults have symptoms of dry eye. If untreated, the problem can cause damage to the ocular surface. Irritation and sensations of sand in their eyes, made worse by the end of the day, are the main complaints of patients with dry eye syndrome, a disease characterized by decreased tear production by evaporation of the tear deficiency or some of its components affecting the quality of the tear secretion. It is a multifactorial disease that can be attributed to chronic diseases such as Sjögren's syndrome (which affects the lacrimal and salivary glands), other eye disorders or even by environmental conditions such as low humidity, use of contact lenses, activities that require concentration and decrease the frequency of blinking, and environments with air conditioning. It is estimated that approximately 7 million Brazilian adults have symptoms of dry eye, and of these, 65% have already been diagnosed*. "However, few know that – if not properly treated – the problem can cause changes in the ocular surface," warns ophthalmologist Caesar Lipener, president of the Brazilian Society of Contact Lenses, Cornea and Refractometry (SOBLEC) and head of contact lenses, Department of Ophthalmology, School of Medicine, Federal University of São Paulo (Unifesp). This is because dry eye syndrome causes symptoms of discomfort and visual disturbance and brings the tear film instability, jeopardizing the proper lubrication of the cornea. Importance of Teardrops "The tear film that covers and helps maintain the proper humidity in tissues of the ocular surface contains bactericidal substances, prevents the adhesion of microorganisms and plays an important role in oxygenation of the cornea,” the expert explains. It also adds nutrients to the cells of the cornea and conjunctiva and has a refractive function because it regulates the corneal surface from an optical standpoint, allowing a clearer view. Moreover, tear film lubricates eyes to avoid friction while blinking and is responsible for cleaning the ocular surface, removing debris and carbon dioxide. "Because the tear film has so many fundamental functions, maintaining eye health and lubrication properly is so important,” adds Lipener. Below is information about the tear and the tear film: Inside tear Produced by specific glands (lacrimal glands), the tear is a liquid composed of water, mineral, salts, vitamins, electrolytes, proteins (albumin, immunoglobulin, lysozyme) and fat. Its main function is to lubricate the eyes and protect them against infections and foreign substances. The tear There are three types of tear secretion: ● ● ● basal secretion - 1 ml per day, is responsible for lubricating the eyes and decreases with increasing age; reflex secretion - to remove irritating substances produced; "emotional" secretion (crying) - unlocked by the hypothalamus and has a different composition. The tear film consists of three layers: ● ● Tear film ● lipid or oily - the outermost layer spreads over the aqueous layer during blinking, decreasing the evaporation of the tear; aqueous – the intermediate layer contains water, minerals and other substances as the enzyme lysozyme (bactericidal) and immunoglobulin (antibody protein that forms which prevent bacterial adhesion). It is responsible for hydration, oxidation and protection of the ocular surface; mucosa - adheres to the ocular surface, allowing the tear to be distributed homogeneously until the next blink. It also dissolves in the aqueous layer by making the tear film more stable and maintains the moist ocular surface. On dry eye syndrome, the foreign body sensation occurs because when the aqueous layer of the tear film evaporates, dried mucus accumulates on the surface, causing irritation of the eyes. Blinking Under normal conditions, we blink on average 20 times per minute. When we are focused, we blink just four times per minute. This usually happens with tasks that require close attention, such as continued use of computers and directing. The functions of the flash are: ● ● ● ● spreading tears; standardizing the tear film on the ocular surface; cleaning the ocular surface, removing debris; protecting the ocular surface. Diagnosis The diagnosis of dry eye syndrome is made by clinical examination and simple tests performed by an ophthalmologist at the clinic. One test, for example, applies a collyrium that colors the ocular surface and shows the points of cell desquamation of the conjunctiva and cornea. "Only the specialist can evaluate the framework and indicate the most appropriate treatment with topical lubricants in most cases," Lipener says. Learn more about dry eye syndrome: Inside dry eye syndrome The main signs and symptoms of dry eye syndrome are burning, feeling of irritation, gritty feeling, itching, ocular redness (hyperemia, increased amount of blood in the tissue), signs of conjunctival irritation, blurred vision, sensitivity or aversion to light (photophobia), watery eyes, and eyelid tremor. Signs and symptoms Dry eye syndrome by Sjögren's ● Causes Deficiencyaqueous Sjögren's syndrome is an autoimmune disease that is characterized by impairment of the lacrimal and salivary glands. The immune cells attack these glands, reducing the production of tears and saliva. It can also lead to a drying of the skin and mucous membranes. The syndrome may be primary or secondary when it is associated with other autoimmune diseases such as rheumatoid arthritis, systemic lupus erythematosus and systemic sclerosis. Dry eye syndrome withoutSjorgren ● ● disabilities tear - caused by aging or hormonal factors (primary) and due to chronic diseases (secondary) such as AIDS, tuberculosis and lymphoma; lacrimal duct obstruction - can occur ● after any form of cicatricial conjunctivitis; blockade of sensory or motor reflexes - the use of contact lenses, diabetes, surgery to correct refractive errors neurotrophic keratitis and reduce the sensitivity of the cornea and lead to a condition of dry eye by blocking the sensory reflex. Various systemic medication (including antihistamines, beta-blockers, antispasmodics, diuretics, antidepressants and psychotropic drugs) can block the motor reflex lacrimal gland. Intrinsic ● Evaporative ● ● ● meibomitis (blepharitis) - inflammation of meibomian gland, responsible for secretion of lipids that form the layer outer film. When less lipids are secreted, the quantity and quality of the tear are altered; disorders of eyelid opening; decreased frequency of blinking; local medicines. Extrinsic ● ● ● ● Environmental Factors vitamin A deficiency; drops with preservatives, use of contact lenses; ocular surface diseases. Dry weather, air-conditioning, heating, air pollution, excessive heat and strong sunlight and humidity affect lacrimation because they favor its evaporation. Wind also contributes to the framework, as well as performing tasks that require a high degree of continuous visual attention (reading, computer use, working under a microscope and the act of driving, for example). * Kantar Health estimates were made from the National Health and Wellness Survey, conducted by Kantar Health. The survey was conducted with 12,000 Brazilians 18 or older via the Internet in 2011. Among the survey respondents, 5% (485 people) have had symptoms of dry eye. Se não tratado, problema pode causar lesões à superfície ocular As sensações de irritação e de areia nos olhos, que se agravam no final do dia, são as principais queixas dos pacientes que apresentam a síndrome do olho seco: doença caracterizada por diminuição da produção lacrimal, por evaporação da lágrima ou pela deficiência em alguns dos seus componentes – afetando a qualidade da secreção lacrimal. Trata-se de uma doença multifatorial, que pode ser motivada por doenças crônicas como a Síndrome de Sjögren (que compromete as glândulas lacrimais e salivares), por outros distúrbios oftalmológicos ou mesmo por condições ambientais como baixa umidade do ar, uso de lentes de contato, atividades que exigem concentração e diminuem a frequência do piscar, e ambientes com ar-condicionado. Estima-se que aproximadamente 7 milhões dos adultos brasileiros tenham sintomas do olho seco e, destes, 65% já tenham recebido o diagnóstico*. “No entanto, poucos sabem que – se não tratado adequadamente – o problema pode causar alterações na superfície ocular”, alerta o oftalmologista César Lipener, presidente da Sociedade Brasileira de Lentes de Contato, Córnea e Refratometria (SOBLEC) e chefe do setor de lentes de contato Departamento de Oftalmologia da Escola Paulista de Medicina da Universidade Federal de São Paulo (Unifesp). Isto porque a síndrome do olho seco, além de provocar sintomas de desconforto e distúrbios visuais, traz instabilidade ao filme lacrimal, comprometendo a lubrificação adequada da córnea. A importância da lágrima “O filme lacrimal, que recobre e auxilia a manter a umidade adequada nos tecidos da superfície ocular, contém substâncias bactericidas, impede a aderência de microorganismos e ainda desempenha papel importante na oxigenação da córnea”, explica o especialista. Ele agrega ainda nutrientes às células das córneas e da conjuntiva, e possui função refrativa, pois regulariza a superfície da córnea do ponto de vista óptico, possibilitando uma visão mais nítida. Além disso, lubrifica os olhos evitando o atrito das pálpebras durante o piscar e ainda é responsável pela limpeza da superfície ocular, removendo detritos e gás carbônico. “É pelo fato do filme lacrimal acumular tantas funções fundamentais à saúde ocular que a manutenção de uma lubrificação adequada é tão importante”, complementa Lipener. Confira as principais informações sobre a lágrima e o filme lacrimal: Por dentro da lágrima Produzida por glândulas específicas (glândulas lacrimais), a lágrima é um líquido composto por água, sais minerais, vitaminas, eletrólitos, proteínas (albumina, imunoglobulina, lisozima) e gordura. Ela tem como principais funções lubrificar os olhos e protegê-los contra substâncias estranhas e infecções. A lágrima Há três tipos de secreção lacrimal: • • • secreção basal – 1 ml por dia, é responsável pela lubrificação dos olhos e diminui com o passar da idade; secreção reflexa – produzida para eliminar substâncias irritantes; secreção “emocional” – desengatilhada pelo hipotálamo e tem uma composição diferente. É a lágrima do choro. O filme lacrimal é composto por três camadas: • • O filme lacrimal • oleosa ou lipídica – mais externa, espalha-se sobre a camada aquosa durante o piscar diminuindo a evaporação da lágrima; aquosa – intermediária, contém água, sais minerais e outras substâncias como a enzima lisozima (bactericida) e imunoglobulina (proteína que constitui anticorpos que impedem a aderência de bactérias). É responsável pela hidratação, oxigenação e proteção da superfície ocular; mucosa – adere à superfície ocular, permitindo que a lágrima se distribua homogeneamente até o próximo piscar; dissolve-se também na camada aquosa tornando o filme lacrimal mais estável e mantém a superfície ocular úmida. Na síndrome do olho seco, a sensação de corpo estranho se dá porque, quando a camada aquosa do filme lacrimal evapora, o muco seca e se acumula causando irritação na superfície dos olhos. O piscar Em condições normais, piscamos, em média, 20 vezes por minuto. Quando estamos concentrados, piscamos apenas quatro vezes por minuto. Isso normalmente acontece em tarefas que exigem muita atenção, como uso contínuo de computadores e dirigir. As funções do piscar são: • • • • espalhar a lágrima; uniformizar o filme lacrimal na superfície ocular; limpar a superfície ocular, removendo detritos; proteger a superfície ocular. Diagnóstico O diagnóstico da síndrome do olho seco é feito por meio do exame clínico e de testes simples realizados pelo oftalmologista no consultório. Em um dos testes, por exemplo, aplica-se um colírio que colore a superfície ocular e mostra os pontos de descamação celular da conjuntiva e da córnea. “Somente o médico especialista poderá avaliar o quadro e indicar o tratamento mais adequado, com uso de colírios lubrificantes na maioria dos casos”, orienta Lipener. Conheça mais sobre a síndrome do olho seco: Por dentro da síndrome do olho seco Os principais sinais e sintomas da síndrome do olho seco são: ardência; sensação de irritação; sensação de areia; prurido; vermelhidão ocular (hiperemia, aumento da quantidade de sangue no tecido), sinal de irritação na conjuntiva; embaçamento visual; sensibilidade ou aversão à luz (fotofobia); lacrimejamento; e tremor palpebral. Sinais e sintomas Olho seco por síndrome de Sjörgren • A síndrome de Sjögren é uma doença autoimune que se caracteriza pelo comprometimento das glândulas lacrimais e salivares. As células imunes atacam estas glândulas, diminuindo a produção de lágrimas e saliva. Pode levar ainda a um ressecamento da pele e das mucosas. A síndrome de Sjörgren pode ser primária ou ser considerada secundária, quando está associada a outras doenças autoimunes como artrite reumatoide, lúpus eritematoso sistêmico e esclerose sistêmica. Olho seco sem síndrome de Sjörgren Deficiência aquosa • • Causas • deficiência lacrimal – ocasionada pelo processo de envelhecimento ou fatores hormonais (primária) e decorrente de doenças crônicas (secundária) como aids, linfoma e tuberculose; obstrução do ducto lacrimal – pode ocorrer após qualquer forma de conjuntivite cicatricial; bloqueio dos reflexos sensorial ou motor – o uso de lentes de contato, diabetes, cirurgia para correção de erros refracionais e ceratite neurotrófica reduzem a sensibilidade da córnea e levam a uma condição de olho seco por bloqueio do reflexo sensorial. Enquanto vários medicamentos de uso sistêmico (incluindo antihistamínicos, betabloqueadores, antiespasmódicos, diuréticos, antidepressivos e psicotrópicos) podem bloquear o reflexo motor da glândula lacrimal. Intrínseco • Evaporativo • • • meibomite – inflamação da glândula de Meibomio, responsável pela secreção dos lipídios que formam a camada externa do filme. Quando liberam menos secreção, alteram a quantidade e a qualidade da lágrima; desordens de abertura palpebral; diminuição de frequência do piscar; medicamentos locais. Extrínseco • • • • Fatores ambientais deficiência de vitamina A; colírios com conservantes; uso de lentes de contato; doenças da superfície ocular. Tempo seco, ar-condicionado, calefação, poluição do ar, sol forte e calor excessivo diminuem a umidade do ar e afetam a secreção lacrimal, pois favorecem sua evaporação. Vento também contribui para o quadro, assim como a execução de tarefas que exigem alto grau de atenção visual contínua (leitura, uso de computador, trabalho ao microscópio e o ato de dirigir, por exemplo). * Estimativas do Instituto Kantar Health feitas a partir da pesquisa quantitativa National Health and Wellness, conduzida pelo instituto.
