When calling the Operation Center, please request the attending operator to give you the reference
number.
Remember that it is mandatory, if any assistance is needed, that you call the Operation Center
first; if this is not complied with, you will not be entitled to the benefits included in the present
Contract.
To avoid problems, place the call yourself and you will receive the assistance of our cordial team.
We remind you that the telephone call is easy to place and free of charge, see information below:
DIRECT OVERSEAS COLLECT CALL TO OUR OPERATION CENTER IN BRAZIL
You can place this call from any telephone booth, from the hotel where you are staying, or from
any wire-line telephone. Below you will find the telephone number of the main carriers in several
continents, just inform the operator the name or Long Distance code of the Brazilian city and the
telephone number to which you would like to place the collect call.
EXAMPLE: Collect call from GERMANY to BRAZIL Dial 08000800055 and inform the operator
the area code of the city of São Paulo 11 + the telephone number of the Operation Center PRINTED IN YOUR VOUCHER. Below you will find some countries:
Obs.: The telephone numbers of “Brasil direto” were provided by Embratel S/A. Therefore, GTA
cannot be held responsible for the information and all the alterations performed by Embratel, with
no further notice.
Leave tickets, documents, cash and valuable objects in the hotel safe. The identification card the
hotel will provide upon your check-in, will serve as a document.
In case your luggage is not delivered at the destination indicated, DO NOT EXIT THE AIRPORT
before filling a claim at the airline company, obtaining the PIR (Property Irregularity Report) and
communicating the occurrence to the GTA-Global Travel Assistance Operation Center.

GENERAL CONDITIONS
WARNING: The present GENERAL CONDITIONS rule the travel assistance services denominated GTA-GLOBAL TRAVEL ASSISTANCE, which the holder of the “Service Order” (voucher)
accepts integrally.
The object of the services specified below consists in providing the Holder of the “Service
Order” (voucher) with an assistance during his/her trip, in order to propitiate assistance in
events herein foreseen. Such assistance is in force during the period of time of the overseas
trip, and its validity is indicated in the “Service Order” (voucher).
The medical assistance services herein foreseen are not permanent, but provided in a manner so as to propitiate to the Holder of the “Service Order” (voucher) the emergency assistance in events foreseen herein, this is not a health insurance, nor a medical assistance
service plan.
IN ORDER TO LEARN ABOUT THE TRAVEL ASSISTANCE SYSTEM AND THE SERVICES OFFERED HERE AND ALSO TO PROPERLY USE THESE SERVICES, LEARNING ABOUT ITS COVERAGE AND LIMITATIONS, WE RECOMMEND READING THE FOLLOWING RULES OF THIS CONTRACT THROUGHLY.
Since it regards a services contract holding the exclusive objective of providing emergency services in
situations that may compromise the normal continuation of the trip, once the Holder’s medical condition is
stable, and it is possible to carry out a medical repatriation for curing purposes, repatriation or transportation will be proposed to the original location of the trip, at the risk of losing the rights foreseen in these
General Conditions.
Costs and expenses with treatment in the Holder’s permanent domicile will be charged to the Holder, whether to his/her medical assistance plan, personal funds, or to any other service hired by the Holder himself.
I - GENERAL PROVISIONS
1.1 - Beneficiary: It is the Holder of GTA-Global Travel Assistance’s “Service Order” (voucher).
1.2 - Age limits: In the Euro Assist, Euro Business, Full, Euro Sports, All Sports and Union Plans, 71 TO
80 YEARS OLD Holder will be subject to a 50% increase to the fee, and above 80 years old to an increase
of 50% to the fee and reduction of 50% in medical assistance coverage.
In the Global and Planet Plans, 70 YEARS OLD Holder will be subject to an increase of 50% to the fee, and
above 75 YEARS OLD TO 85 YEARS OLD an increase of 50% to the fee and a reduction of 50% on the
medical assistance coverage.
In Plans Executivo Full, Executivo Global, Executivo Planet, Star e Titanium the holder aged more than 70
years will have a 50% percent addition to the fee; the age limit is 75 years old.
In the other plans, ABOVE 75 YEARS OLD Holders will be subject to a reduction of 50% on the medical
coverage.

1.3 - VALIDITY PERIOD:
a) GTA-Global Travel Assistance will provide the assistance services herein foreseen during the contracted
validity period, which will be stated in the “Service Order” (voucher) issued at time of contracting.
b) The period of validity will correspond to the one informed by the Holder at the time the “Service Order”
(voucher) is issued.
c) If, due to any circumstance, under responsibility or not of the Holder, a price lower than the value of the
fee corresponding to the contracted period of validity was stipulated, GTA-Global Travel Assistance will
provide the assistance services and repatriation services covering the period corresponding to the amount
of the fee effectively paid by the Holder.
d) If due to any circumstance, under responsibility or not of the Holder, a concurrent issue of the “Service
Order” (voucher) occurs, only the first “Service Order (voucher) issued will be valid, and any other additional orders will not be in effect.
e) The assistance services and responsibility of GTA-Global Travel Assistance are limited to the validity
period of the “Service Order” (voucher), or at maximum 365 consecutive days per Holder’s traveling period,
and the issue of a “Service Order” (voucher) for a period above 365 days is prohibited.
f) Student category Plans (as of July/11) – Maximum Period of 18 Months, starting at the date of departure
from the Country where the “Service Order” (Voucher) was issued, ending at the date of return to that same
Country.
g) The period of 365 days starts as from the date of departure from the country where the “Service Order”
(voucher) was issued and ends on the date of return to that same country. This delimitation applies also to
GTA-Global Travel Assistance annual plan.
h) GTA-Global Travel Assistance prohibits the issue of a Voucher to a person already travelling overseas.
1.4 - ANNUAL VOUCHER: The Holder of the annual “Service Order” (Voucher) is entitled to an unlimited
quantity of travelling periods.
1.4.1 MULTI TRAVELING PERIODS - FOR THE BENEFICIARY OF EXECUTIVO OURO / PLANET / GLOBAL
/ FULL 90 / FULL 120 / GLOBAL 90 / GLOBAL120 / PLANET 90 / PLANET 120 / EUROBUSINESS PLANS
- The Beneficiary will stay no more than 120 or 90 or 45 consecutive days overseas, according to what is
indicated in the General Conditions of the acquired product. After this period, the beneficiary will have no
more right to the acquired assistance services, even if still travelling. The period of days is based on the
dates of exit from the country where the Voucher was originally issued.
WARNING: THE TOTAL OF CONSECUTIVE DAYS OF TRAVELLING PERIOD FOR THESE PLANS CANNOT
BE SUPERIOR TO 365 (three hundred and sixty five) DAYS.
At the time of requesting assistance, the GTA-Global Travel Assistance Operation Center will request a
copy of the Holder’s passport or any other document, presenting the date of departure from the country of
origin, or the date of entrance in the country in which the holder is requesting the service.
Students holding the Student Plan will be required to provide, by fax to the GTA-Global Travel Assistance
headquarters, a credential of the University or School in which they are studying.
Specific Exclusion: no assistance of any kind will be provided to holders under illegal migration or labor
situations (including non-declared labor in the country in which the assistance is requested).
1.5 - TERRITORIAL VALIDITY: According to the product, the validity will be global, European (countries of
the European Community and Switzerland), regional (except the country of domicile).

a) PLANS EURO ASSIST / EUROBUSINESS / STUDENT EURO ASSIST / RECEPTIVO EURO ASSIST:
European coverage product, if used outside of the European continent will have a 50% reduction in the
medical coverage. For common knowledge, the countries that are part of the European Union, to which Switzerland was added exceptionally for the purpose of validity to the same coverage, are: Germany,
Austria, Belgium, Bulgaria, Cyprus, Denmark, Slovakia, Slovenia, Spain, Estonia, Finland, France, Greece,
Hungary, Ireland, Italy, Latvia, Lithuania, Luxembourg, Malta, Holland, Poland, Portugal, United Kingdom,
Chez Republic, Romania and Sweden.
EURO MAX / RECEPTIVO EURO MAX Plans - the amounts for the coverage will be held for EUROPE, ASIA,
AFRICA and OCEANIA, for the AMERICAS there will be a 50% reduction on the medical coverage and on
sanitary repatriation.
b) GTA-Global Travel Assistance card covering assistance will not provide services in Brazil, EXCEPT FOR
THE RECEPTION PRODUCT (foreigners visiting Brazil):
• BRONZE / RUBI / PRATA / OURO / DIAMANTE PLANS – Age Limit - above 75 years old, 50% reduction
only to the medical benefit.
• UNION AND FULL PLANS – Age Limit – from 71 to 80 years old. 50% addition to the fee, above 80 years
old, 50% addition to the fee and 50% reduction only to the medical benefit. Benefit - Medical and Funeral
Repatriation(1) - included in medical costs.
• EURO ASSIST and EURO MAX PLANS – Age limits - from 71 to 80 years old, 50% addition to the fee,
above 80 years old, 50% addition to the fee and 50% reduction only to the medical benefit. Geographical
Coverage - EURO ASSIST coverage values will be maintained only for countries of the Schengen Treaty
and the European Union, for the other continents, a reduction of 50% on the medical coverage will be
applied Benefit - Medical and Funeral Repatriation(1) - included in medical costs.
Euro MAX Plan - GEOGRAPHICAL COVERAGE: the amounts for the coverage will be held for EUROPE, ASIA,
AFRICA and OCEANIA, for the AMERICAS there will be a 50% reduction on the medical coverage and on
sanitary repatriation. Repatriation due to death is included in medical costs.
REIMBURSMENT: Reimbursement payments will be performed in Brazil, in national currency, by a
deposit to the holder’s banking account in a Brazilian bank.
c) GTA-Global Travel Assistance card covering assistance will not provide services in Brazil, even within
the validity period of the “Service Order” (Voucher).
d) Countries in civil war or among themselves are excluded from the territorial validity. These countries
are examples, but they are not limited to them: Afghanistan, Iraq, Sudan, Somalia, North Korea, etc.
1.6 - PROCEDURE TO REQUEST ASSISTANCE: To request assistance services, the Holder of the “Service
Order” (voucher) should MANDATORILY contact GTA-Global Travel Assistance Operation Center through
a COLLECT CALL placed to the telephone number printed in the Voucher, informing his/her name, number and period of validity of the “Service Order” (voucher), his/her location and the reason for assistance
request.
1.7 - RESPONSIBILITY OF THE “SERVICE ORDER” (VOUCHER) HOLDER: In all the assistance cases
foreseen in the present General Conditions, the Holder is committed to:

a) Request first, and by phone, an authorization from GTA-Global Travel Assistance Operation Center
before taking any initiative or commit himself/herself with any expense.
b) If the Holder is unable to communicate with GTA-Global Travel Assistance Operation Center, he/she
should seek medical assistance from any medical center closer to his/her location, HOWEVER within the
first 24 hours of the event he/she should notify such event to the Operation Center, INCLUDING the reason
that prevented him/her from requesting the assistance, under the risk of automatic cancellation of the right
to obtain direct payment and/or reimbursement of the services contracted from the service providers. This
communication is mandatory, as GTA-Global Travel Assistance will not be responsible for the cost of any
assistance without prior knowledge and approval of its Operation Center.
c) Accept the solutions indicated by GTA-Global Travel Assistance Operation Center.
d) Agree to repatriation to his/her country of origin as long as his/her health conditions allow such repatriation and according to GTA-Global Travel Assistance doctors’ recommendations.
e) In case of reimbursement request, provide the necessary documentation in order to confirm the authenticity of the event, as well as all the original supporting documents covering the expenses to be reimbursed by
GTA-Global Travel Assistance, and all the medical information that allows GTA-Global Travel Assistance
Operation Center to analyze the authorization for service rendering and payment of the assistance provided.
f) Deliver to GTA-Global Travel Assistance the air ticket(s) in his/her possession, duly endorsed, when
GTA-Global Travel Assistance will effect repatriation of Holder whether in case of accident, disease or
death.
g) Grant authorization that allows disclosure of his/her clinical history to facilitate analysis of the case by the
Medical Department of GTA-Global Travel Assistance Operation Center.
1.8 - RESPONSIBILITIES OF GTA-GLOBAL TRAVEL ASSISTANCE: The obligations assumed by GTA-Global
Travel Assistance refer to coverage of expenses resulting from accidents occurred or sudden and acute
illnesses suffered after the start date of the “Service Order” (voucher), as long as his/her departure from
the country of origin is confirmed.
Under the present General Conditions, an accident is understood as being the event that generates a body
damage suffered by the Holder of the “Service Order” (voucher) caused by violent and visible external
agents, out of his/her control and in movement, as well as injury or disease directly caused by such agents
and in an independent manner of any other cause.
1.9 - CANCELLATION OF “SERVICE ORDER” (VOUCHER) OR CHANGE IN VALIDITY PERIOD:
a) The request should be submitted by the Holder through fax 11 3255-5704, mail or personally no later than
the third business day prior to the date scheduled in the “Service Order” (voucher) for start of assistance.
b) After the period mentioned above, the right to request cancellation or change in the validity period will
expire automatically and the Holder shall have no right to any compensation.
c) If the cancellation or change in validity period request is sent by the Holder within the third business day
prior to the date indicated in the “Service Order” (voucher), the amount paid by the Holder will be returned
or a new “Service Order” (voucher) could be issued with a new expiration date.
1.10 - SUBROGATION:
a) Up to the total amount spent in fulfillment of the obligations established in the present General Conditions,
GTA-Global Travel Assistance will continue automatically subrogated in the rights and actions that may
correspond to the Holder or to his/her legal heirs, against third parties, natural individuals or legal entities,
due to the event that motivated the assistance provided.
b) In the subrogation, the rights and actions to be exercised are expressly understood, in light of the transportation companies, as related to full or partial reimbursement of the unused tickets, when GTA-Global
Travel Assistance will carry out at its own expenses, the transportation of the Holder or of his/her remains.
Consequently, the Holder irrevocably transfers in favor of GTA-Global Travel Assistance, the rights and

actions included in this clause, committing himself/herself to execute all the legal acts that are necessary,
including providing cooperation if requested as related to the subrogation herein agreed upon.
1.11 EXEMPTION OF OBLIGATION: GTA GLOBAL Travel Assistance will be exempted from providing the
services herein contracted if the practice of an illicit act or malicious intent is characterized on behalf of
the Holder of the “Service Order” (voucher), for example: simulation of luggage loss, acquisition of other
assistance services to accumulate indemnity for loss of luggage, acquisition of the present assistance with
the intention of using it for medical treatment of already known disease (pre-existence ailment).
In case of the exemplified hypotheses and in other cases that characterize fraud, the Holder will have no
right to obtain rendering of services herein agreed upon and will respond in the civil and criminal sphere for
such conduct.
GTA-Global Travel Assistance reserves the right to demand from the beneficiary the reimbursement of
any expenses effected in an incorrect manner, if illicit or malicious acts are evidenced on behalf of the beneficiary. The collection mentioned above will use as evidences the General Conditions and the receipts of
payments effected by GTA-Global Travel Assistance.
1.12 - EXEMPTION OF RESPONSIBILITY: GTA-Global Travel Assistance shall not be held responsible
and will not indemnify the beneficiary of the “Service Order” (voucher), for any damage, loss, injury or
disease caused by professionals that provided the requested assistance services, whether medical or legal
assistance.
GTA-Global Travel Assistance only provides the services when requested without being responsible for
the results.
In this case, the person, or persons, assigned by GTA-Global Travel Assistance, will be considered as
agent of the “Service Order” (voucher) beneficiary, without recourse of any nature or circumstance against
GTA-Global Travel Assistance due to such assignment.
1.13 - EXCEPTIONAL CIRCUNSTANCES: GTA-Global Travel Assistance will be exempted from any responsibility in cases of acts of God or force majeure such as: catastrophe, earthquake, rebellions, riots, acts
of guerilla or against guerilla, hostilities, retaliation, conflicts, embargos, acts of violence, mobilization of
political nature, strikes, popular acts, lock-out events, or any act of sabotage or terrorism, that unable the
normal rendering of its services due to such reasons.
1.14 - STATUTORY LIMITATION PERIOD: All and any claims related to the obligations assumed by GTAGlobal Travel Assistance through the present General Conditions should be submitted in writing, within the
maximum period of 90 (ninety) days as from the expiration date of the “Service Order” (voucher) of the
beneficiary.
ELAPSED THE PERIOD OF TIME MENTIONED ABOVE no claim of any nature will be accepted and the
contract signed between the parties will be considered as fulfilled.
1.15 - Forum: In case of controversy related to the present contract covering assistance during travel, both
parties elect as from this moment, the forum of the city where the “Service Order” (Voucher) was issued,
expressly waiving to any other even if more privileged.
1.16 - EXEMPTION OF SALES AGENTS RESPONSIBILITY: The representatives of the GTA-Global Travel
Assistance cards (travel agencies, tourism operators, transportation companies and every and any issuer

agent) will not be considered as part of the contract indicated in the present General Conditions, and for this
reason they will be exempted from any responsibility relevant to the object of the contract.
1.17 - GENERAL CONDITIONS (US$, R$ and €,): The General Conditions of the products expressed in values corresponding the United State Dollars (US$), Reais (R$) or to Euros (€), should be provided together
with the card issued and an integrant part of the present contract.
II - DESCRIPTION OF ASSISTANCE SERVICES
2.1 - MEDICAL ASSISTANCE DUE TO ACCIDENT OR ILLNESS
A - GENERAL CONDITIONS
A1 - In case of occurrence of an illness or injury that prevents continuance of the trip, the holder of the
“Service Order” (voucher) may use, free of any charge (up to the limits established in the acquired plan),
the services of professionals and/or hospitals that will be indicated and/or provided by the intervenient GTA-Global Travel Assistance Operation Center, according to the case.
Minor illnesses or injuries that do not hinder the normal continuance of the trip will not be covered by the
present assistance, whereas, upon return from the trip, the Holder may request the reimbursement of the
expenses resulting from such assistance if the minor illnesses or injuries fit the present General Conditions.
A2 - GTA-Global Travel Assistance will be responsible for the costs of services provided to the Holder by
other professionals and/or medical, health centers, only under the following situations:
a) If the Holder is unable to request the assistance to a GTA-Global Travel Assistance Operation Center.
b) If, upon request of assistance to the Operation Center, a medical team belonging to GTA-Global Travel
Assistance is not immediately available on the site where the event occurred. Costs for assistance in such
cases and exclusively in case of emergencies, will be paid directly to the service provider according to
the conditions established in item II - Conditions for reimbursement of expenses covering benefits included
in the General Conditions.
B - SERVICES INCLUDED IN MEDICAL ASSISTANCE BENEFITS
B1- Assistance provided by medical teams in case of emergency.
B2- Assistance provided by experts when indicated by the medical professional responsible for the required
assistance.
B3- Complementary medical examinations: laboratory analyses, radiologic examinations and others services when deemed necessary and duly authorized by the Operation Center.
B4- Hospitalization when requested by the medical professional responsible for the service during the period
of validity of the “Service Order” (voucher), this period of hospitalization can be extended, at the medical
professional’s request, responsible for the assistance, for a specific period according to the plan acquired.
Such hospitalization will take place in the most appropriate hospital based on the nature of injuries or illness
of the Holder.
B5- Surgeries: When authorized by the GTA-Global Travel Assistance Operation Center, in cases of emergency that require such procedures in an immediate manner and which cannot be postponed until return of
passenger to his/her country of origin.

When the return is approved by the GTA-Global Travel Assistance Operation Center and carried out strictly
according to its determinations, the beneficiary and/or members of his/her family should accept such return
under the risk of losing all the benefits related to the present rendering of services.
B6- Transportation of Holder from the medical center in case of illness or injury: GTA-Global Travel Assistance will be responsible for transportation to the medical center indicated, according to the nature of
injuries or of illness and as recommended by the medical professional responsible for the assistance, using
any transportation mean available. All transportation should be authorized by the GTA-Global Travel
Assistance Operation Center.
B7- Physiotherapy: In case of traumatism, if authorized by the GTA-Global Travel Assistance Operation
Center, under prescription of the responsible doctor and according to the coverage indicated in the General
Conditions. Coverage included in medical expenses.
B8- Pharmaceutical Assistance: GTA-Global Travel Assistance will be responsible for the expenses incurred with medication prescribed by the doctors responsible for the assistance, only during the validity
indicated in the voucher acquired, according to the limits established in the purchased plan and per event.
1) Expenses incurred by the beneficiary will be reimbursed upon his/her return to the country of origin,
through submittal of the supporting documents original forms and descriptive notes.
2) The expenses incurred with medication should be resulting from prescription of the doctor indicated by
GTA-Global Travel Assistance Operation Center.
3) Medication within coverage should have direct relation with the reason related to the medical assistance
request and consequent diagnosis.
4) Only the expenses related to medication resulting from non-hospital assistance and based on the conditions described above will be subject to reimbursement.
5) Extension of prescriptions, as long as within the voucher validity will be authorized whenever, based on
recommendation of GTA-Global Travel Assistance Operation Center, and such medication is essential
for the health of the beneficiary.
6) Medication purchased for treatment of pre-existing illnesses or excluded from the assistance will not be
subject to reimbursement, even if the medical appointment has been authorized and covered by GTA-Global Travel Assistance.
7) Medication purchased for treatment of psychological or emotional disorder has no coverage and will
not be subject to reimbursement.
B9- MEDICAL ASSISTANCE FOR PRE-EXISTING ILLNESSES: GTA-Global Travel Assistance will be responsible for the medical assistance deriving from a pre-existing or chronic illness that resulted in a crisis
during the trip, up to the limit specified in the General Conditions, except follow-ups and controls of previous
treatments, checkup and extension of prescriptions.
1) Expenses incurred with medication are not covered even if the limit established for emergency assistance
has not been exceeded.
2) Expenses incurred with hospitalization are not covered even if the limit established has not been exceeded.
3) Expenses incurred with examinations or subsequent treatments are not covered, even if the limit foreseen
has not been exceeded.
B10- Medical Check-up: Based on the plan acquired, the Holder of the “Service Order” (Voucher) may
request a medical checkup for a non-pre-existing illness and uniquely for investigative purposes, when
authorized and scheduled by the GTA-Global Travel Assistance Operation Center, based on its sole criterion

and carried out in the medical center duly indicated.
B11 - Sports Practice Coverage: Holder of the GTA-Global Travel Assistance card will be entitled to a coverage up to the limit established in the plan, as a first emergency assistance, if he/she suffers an accident
practicing sports, as long as not practiced in a competition.
Expenses incurred with examinations or subsequent treatments are not covered, even if the limit foreseen has not been exceeded.
No repatriation coverage exists in case of accident resulting from practicing of sports.
WARNING: As related to skiing practiced in regulated lanes and as long as it is not a competition, maximum coverage for medical expenses, including repatriation expenses, will be in compliance with the
selected BRONZE NEVE or OURO NEVE plans.
B12- SPORTS:
B12.1 - PLANS ALL SPORTS E EURO SPORTS - Practice of Sports duly covered: All sports are covered in
the amateur modality, except: Bobsleigh, Olympic luge, skeleton, hunting of dangerous animals, mountaineering at more than 6.000 meters, potholing.
The limit of these plans are included in item 2.1- Medical Assistance due to accident or disease, refers to
each event that occurs during the validity of the plan and corresponds to the sum of the expenses incurred
with: professionals of the medical area per illness or accident, hospital daily rates, nursing services, complementary medical examinations, medical repatriation or due to death.
B12.2 Plan FULL SPORTS - Leisure activities and amateur sports practice characterized as competitions
up to the limit of the medical assistance for accident/illness will be covered, as long as the sport is recognized by any association, federation or even a committee and is practiced with the use of safety equipment,
license or other necessary measures. The modalities covered by these plans are mentioned here: Aerobics,
Fishing with hooks, Arch and Bowl, Tracking, Bowling, Cricket, Crocket, Curling, Kayak up to level 3, Netball,
Golf, Swimming, Tennis, Cross walking, Racquetball, Skateboard, Handball, Squash, Volleyball, Roaring,
Yachting at the coastline and Soccer.
B12.3 - Search and Rescue Expenses: GTA-Global Travel Assistance will be responsible for the search
and rescue expenses incurred with rescue to the beneficiary injured in practice of amateur sports, UP TO
THE MAXIMUM LIMIT OF THESE PLANS (ALL SPORTS and EURO SPORTS), including the expenses
incurred with helicopter or plane to organize rescue of the beneficiary.
EXCLUSION - Practice of sports by professionals in competition or training for participation in professional competitions are excluded.
Every person that lives from the practice of sports without exercising any other professional activity is
considered a professional sportsman.
B12.4 Removal in case of emergency: (PLANS ALL SPORTS and EURO SPORTS) in case of emergency,
transportation of the ill or injured beneficiary to the nearest Hospital will be made using the transportation
mean that the Medical professional responsible for the assistance considers as most appropriate according
to the seriousness of the injury or illness. Coverage included in medical expenses.
C - TRAVEL INTERRUPTION ASSURANCE (Plans All Sports and Euro Sports):

In case of accident or injury, if repatriation of the beneficiary is required, or if the Medical professional responsible for the assistance requires and is authorized by the Operation Center, the beneficiary should remain
at rest, and GTA-Global Travel Assistance will reimburse on a pro rata temporis basis, after carrying out an
audit covering the originals of the supporting documents, the SKI PASS expenses, Sports Lessons or Rental
of Sport Material up to the maximum limit of the purchased plan.
D - RECOVERY IN HOTEL (MAXIMUM 05 DAYS):
GTA-Global Travel Assistance will reimburse the expenses incurred with hotel up to the limits expressed in
the General Conditions when, due to accident or illness and according to prescription of the Medical Professional responsible for the assistance, the beneficiary should stay in forced rest, during a period of up to
5 (five) days after remaining hospitalized for a minimum period of 5 (five) days.
THE RIGHT TO THIS COVERAGE WILL NOT BE RECOGNIZED IF THE CAUSE IS INCLUDED IN THE EXCLUSION ITEM.
WARNING: For this clause as well as for any other clause that covers hotel expenses, such expenditures
are limited to simple lodging, no coverage exists for expenses with food, laundry, telephone calls, use of
taxis, internet access, etc.
E - REPATRIATION AND TRANSPORTATION:
E1- Medical Repatriation: Medical Repatriation is understood as the procedure to be carried out to return
the holder, sick or injured, from the point of his/her location to the country of origin.
a) If, and only if , recommended by the medical professional responsible for the assistance, the beneficiary
will be entitled to medical repatriation with the respective coverage of payment of fees due to change in date,
or even issue of a new air ticket (in the economic class category and subject to availability of seats). IN THIS
CASE GTA-Global Travel Assistance will be responsible for the expenses UP TO THE LIMITS ESTABLISHED
IN THE PRESENT GENERAL CONDITIONS.
b) From the date in which the beneficiary is hospitalized through the date of return trip, the present assistance includes his/her transportation in ambulance or any other mean compatible with his/her health condition,
and approved by the GTA-Global Travel Assistance Operation Center, from the hospital to the departure
airport, as well as, the support structure required to his/her comfort, including stretcher, wheelchair, etc.
c) Only GTA-Global Travel Assistance will authorize and take all the measures mentioned in this clause;
the beneficiary or any member of his/her family is prohibited to do this on their own account without the
authorization of GTA-Global Travel Assistance.
d) Repatriation will always be from the point where the ill or injured beneficiary is located to the country
informed as his/her domicile in the GTA-Global Travel Assistance voucher.
The right to medical repatriation will not be recognized if its cause is included in the EXCLUSION item.
e) Coverage will cease as from the moment in which the beneficiary is located in his/her country of origin
or if the validity of the voucher has expired.
f) The beneficiary will be entitled to medical repatriation foreseen in this clause only during the period of
validity of his/her voucher.
g) GTA-Global Travel Assistance, always aiming at the welfare of the holder, has priority on the decision
of his/her repatriation.
h) It is herein established that treatments and surgeries will be carried out only in situations characterized
as emergencies, if repatriation is not recommended by the medical professional responsible for the assistance. The non-fulfillment of this rule exempts GTA-Global Travel Assistance from coverage foreseen for
this specific situation.

i) In all cases, members of the family will be requested to return the ticket unused by the Holder, and such
ticket should be transferred to the name of GTA-Global Travel Assistance.
E2- Repatriation due to Death: In case of death of the beneficiary during the period of validity of the assistance card, as a consequence of an event not excluded from the general conditions:
a) GTA-Global Travel Assistance will comply with the administrative formalities covering repatriation of
the corpse, and will be responsible for the expenses covering conditioning (embalming if necessary) and
transportation, such transportation will be made using the most convenient mean from the death location
to the city of domicile of the holder, or to the airport of commercial line* nearest to the city of residence of
the dead beneficiary.
*airport of commercial line is the airport that allows landing and departure of planes with capacity for more
than 100 passengers.
b) All measures will be taken care of by the GTA-Global Travel Assistance Operation Center, and FAMILY
MEMBERS OR TOURISM COMPANIES ARE PROHIBITED TO ACT WITHOUT PRIOR EXPRESS AND WRITTEN AUTHORIZATION OF THE OPERATION CENTER UNDER THE RISK OF NOT BEING REIMBURSED FOR
THE EXPENSES.
c) Religious service, funeral and special coffin are EXCLUDED from this benefit.
d) GTA-Global Travel Assistance WILL HAVE NO RESPONSIBILITY if the corpse of the beneficiary suffers
transformations such as reductions, incinerations and others.
E3 - Transportation of CEO: According to the plan foreseen in the General Conditions, if the beneficiary is
travelling on an overseas business trip, and is hospitalized due to a serious medical emergency subject to
appreciation of GTA-Global Travel Assistance Operation Center, which prevents the beneficiary from performing his/her professional commitments, GTA-Global Travel Assistance will be responsible for the cost
of an air ticket in tourist class, subject to availability of seats, for another employee named by the holder’s
company to act as his/her substitute.
This benefit will only be applied if authorized and processed by the GTA Operation Center.
The reason for hospitalization should not be included in the EXCLUSIONS.
No requests for reimbursement will be accepted later, independently of any justification.
F – LIMITATIONS/EXCLUSIONS
The following assistances are expressly excluded from free of charge medical assistance:
1) chronic, pre-existing illnesses and health problems prior to issue of GTA-Global Travel Assistance voucher, or before the corresponding trip, whether known by the holder or not, as well as their aggravation and
consequences. In this case, GTA-Global Travel Assistance will provide, based on its sole criterion, the
first clinical examination that will allow the diagnosis of pre-existing illness.
WARNING: As chronic or pre-existing illnesses we give the following examples, however not limited to
them: diabetes, cardiovascular diseases (coronary insufficiency), cardiac insufficiency, valvular heart diseases, high blood pressure, kidney stone, chronic kidney insufficiency, gallstone, chronic pancreatitis,
chronic hepatopathy, diverticular disease of the colon, gastro duodenal peptic ulcer, hernia, herniated disk,
chronic obstructive pulmonary disease, bronchial asthma, malign and benign neoplasm, peripheral vascular
diseases, epilepsy, chronic anemia, chronic thrombocytopenia, sexually transmittable diseases, etc.

2) Disorders, illnesses or injuries resulting from criminal actions performed by the holder, whether directly
or indirectly.
3) Affections, injuries and complications resulting from the use or placing of piercings, earrings, other
adornments, tattoos, etc. which have been performed within the validity of the voucher or before the validity
of the voucher.
4) Diseases or injuries caused by suicide attempt or caused intentionally by the holder to himself/herself
and/or to a member of his/her family, as well as any other act that confirms the irresponsibility or imprudence on behalf of the beneficiary holder of the assistance card.
5) accidents, diseases or pathologic conditions produced by intentional ingestion of psychotropic products,
hallucinogens, alcohol and/or any other drug of similar characteristics not prescribed by a doctor indicated
by GTA-Global Travel Assistance, or directly or indirectly consequences of deviant actions or transgressions of the beneficiary.
6) diseases, disorders, injuries and their consequences, collateral effects and their complications, resulting
from treatment or care provided by individuals/and or professionals not authorized or not recognized by
the assistance medical team of GTA-Global Travel Assistance, such as practice of quackery, faith cure,
including self-medication. Homeopathic, acupuncture and kinesiotherapy treatments, thermal treatments,
chiropody, etc.
7) Disorders or injuries resulting from sun exposure.
8) Any type of hernia and its consequences.
9) Anaphylactic shock and its consequences.
10) aesthetic or rejuvenating treatments, plastic surgeries, supply, replacement or repair of prostheses,
including but not limited to, dental prostheses, orthodontic units, contact lenses, hearing aids (including
replacement of batteries) glasses (replacement, repair and update of prescriptions), etc.
11) Accidents in consequence of training, practice and/or active participation in sports competitions (professional or amateur).
12) Occurrences caused by practice of dangerous sports including, but not limited to, motorcycling, motoring, boxing, polo, water ski, diving, instrument flying, hang-gliding, flight in any device other than commercial line, parachuting, “bungee jumping”, mountaineering, skiing and/or any other winter sports.
13) Injuries caused to drivers or passengers by accidents resulting from the use of unauthorized means of
transportation for public transportation in general, vehicles without license or insurance, including airplanes,
driving motorcycles without helmet, and mopeds.
14) Accidents resulting from trips on vehicles driven by individuals without driving licenses or with expired
driving license.
15) Endemic, pandemic, epidemic or viral diseases of any origin or nature, including AIDS and its complications, as well as venereal diseases.
16) accidents and/or diseases resulting from acts of war, whether declared or not declared, chemical war,

bacteriologic wars, civil war, guerrilla, revolution, disorder, riot, uprising, sedition, upheaval, vandalism,
movement of political associative nature, strikes or lock-out events, or any other disorders of public nature.
17) events in consequence of natural disasters such as: seaquakes, earthquakes, tornados, cyclones, volcanic eruptions, and other disasters of nature as well as any other phenomenon of extraordinary character,
or event that due to its size or seriousness is considered as a national disaster or catastrophe.
18) accidents and/or diseases resulting from the use of nuclear material, including intentional or not nuclear
explosions as well as radioactive contamination or exposure to nuclear or ionizing radiation.
19) Mental diseases of any type, emotional of psychological disorders of any nature.
20) Diagnosis, control, follow-up and treatment of pregnancy, childbirth, abortion and its consequences,
unless resulting from accident.
21) Control of blood pressure, high blood pressure and its consequences.
22) Analyses and/or hospitalization for examinations, effort tests and all types of preventive check-up.
23) Cancer and all its treatments.
24) Transplants.
25) Injuries caused due to participation in bets or fights.
26) Injuries resulting, depending, arranged or facilitated by repetitive efforts or cumulative micro-traumas,
or having relation of cause and effect with such traumas, as well as injuries classified as: repetitive strain
injuries - RSIs, work-related musculoskeletal disorder - WMSDs, cumulative trauma injuries/disorders CTIs, or similar that can be accepted by the medical-scientific organizations, as well as their consequences,
treatments and post-treatments, including any time surgeries.
27) Accidents and/or diseases resulting from hijack or hijack attempt.
28) Accidents and/or diseases resulting from act of terrorism, and the beneficiary of the GTA-Global Travel
Assistance card should evidence the event, by means of supporting documents and a detailed report that
characterizes the nature of the outrage, independently of its purpose, and as long as such outrage is duly
recognized as an attempt to civil commotion by the competent authorities.
29) Intentional and malicious acts of the holder or of his/her attorneys.
30) Pain or discomfort resulting from long walks in unsatisfactory conditions (footwear problems, high
external temperature, dehydration, etc.), including but not limited to, backaches, back pains, pains in the
legs, feet, and swollen legs and feet.
31) In general all types of laboratory tests and/or treatment that have not received advance authorization
from the Operation Center of GTA-Global Travel Assistance.
32) Equivalent situations or situations acknowledged by official social security organs, to disability due to
accident where the injury causing event does not fully fits disability characterization by personal accident.

33) When it is evidenced that the reason of the overseas trip relates to treatment of a pre-existing disease
and that the current treatment has a direct or indirect link with the pre-existing disease, GTA-Global Travel
Assistance will be exempted from providing the services. For this purpose, GTA-Global Travel Assistance
reserves the right to investigate the relation between the current fact and the pre-existing disease.
34) Equivalent situations or situations acknowledged by official social security organs, to disability due to
accident where the injury causing event does not fully fits disability characterization by personal accident.
35) Illegal migratory situation and/or illegal labor situations of the holder. Professional risks; in case the
purpose of the holder’s trip is to execute tasks or jobs involving professional risks (in all cases, the services
described in these General Conditions are complementary to those provided by assistance and insurance
entities, according to industrial safety and labor liability regulations in effect in the country where the assisted infirmity or accident occurs.)
E15- Dental Assistance: GTA-Global Travel Assistance will be responsible for the expenditures related to
urgent dental assistance, limited to treatment of acute pain up to the amounts indicated in the coverage tables, according to the plan acquired. WARNING: THIS COVERAGE IS NOT PER EVENT BUT CUMULATIVE.
E16 - Escort of family member (Lodging) Maximum of 05 days: If foreseen in the General Conditions of
the product acquired, the beneficiary will be entitled to coverage of hotel expenses for his/her family escort
up to the amounts and for the period detailed in the General Conditions.
a) The right to this assistance will not be recognized if the reason of hospitalization is included in the EXCLUSION item.
b) The holder whose card validity has already expired will not be entitled to this service.
c) This benefit can be granted only if authorized and fully processed by GTA-Global Travel Assistance
Operation Center.
d) No requests for reimbursement will be accepted later, independently of any justification.
WARNING: For this clause as well as for any other clause that covers hotel expenses, such expenditures
are limited to simple lodging, no coverage exists for expenses with food, laundry, telephone calls, use of
taxis, internet access, etc.
E17 - Escort to under-aged and elderly individuals: If the holder is the only adult escort, whether of a
passenger under 15 years old, or of an elderly individual more than 75 years old, which are also holders of
a GTA card, is hospitalized due to an accident or disease covered by the present General Conditions or may
die due to a reason also covered by the present General Conditions, GTA-Global Travel Assistance will be
responsible for the escort of the under-aged or elderly individual at time of return to the country from where
the trip was originated.
E18 - Two way Air-Ticket for a member of the Family: If hospitalization of the beneficiary travelling without escort is foreseen to be for more than 05 (five) days, GTA will be responsible for the cost a one air
ticket in economy class, subject to availability of seat, so that a direct member of the family (father, mother,
brother, spouse or son/daughter) may escort the beneficiary and provide assistance.
This ticket will be issued in economy class and will have as destination the capital of the country where
the holder is hospitalized or the nearest city, as long as no direct flight exists between the points of origin

and of destination.
The right to this assistance will not be recognized if the reason of hospitalization is included in the
EXCLUSION item.
The holder whose card validity has already expired will not be entitled to this service.
This benefit can be granted only if authorized and fully processed by GTA-Global Travel Assistance Operation Center. No requests for reimbursement will be accepted later, independently of any justification.
III - RETURN TRIP ASSURANCE
FOR ALL PLANS INDICATED IN THE GENERAL CONDITIONS.
a) If the holder needs to return to his/her country of origin due to death or serious illness of one of his/her
direct member of the family: parents, spouse, siblings or sons living in such country of origin, GTA-Global
Travel Assistance will be responsible for the difference in cost of the holder’s return air ticket, when such
air ticket is of lower tariff due to the date of return indicated therein.
The benefit will be valid only in case of serious illness as long as such illness is not included in item
Limitations-Exclusions, and if death occurs in the country of issue of GTA voucher, in the holder’s domicile.
b) In case of fire, explosion, flood, or theft with damages and violence, at the domicile of the beneficiary,
while such beneficiary is travelling, if a person to handle this event is not available, and if the return original
air ticket does not allow a change in the date, GTA-Global Travel Assistance will be responsible for the
difference that corresponds to the new reservation of the air ticket, from the point where he/she is located
with destination to the city of his/her domicile. In order to be entitled to this benefit, the original incident
report or its equivalent demonstrating the event must be forwarded to the Operation Center within 24 hours.
WARNING: When the air ticket initially purchased by the beneficiary is not used, whether in the hypothesis
foreseen in the present clause, or in any other, the value corresponding to such air ticket will be recovered
by GTA-Global Travel Assistance. In case of reimbursement payment, GTA-Global Travel Assistance will
reimburse the eventual difference between the amount paid for the air ticket and the amount spent covering
the service provided by the air line.
IV - TRANSMISSION OF URGENT MESSAGES
If during the voucher validity, the Holder of the “Service Order” (voucher) requires the transmission of an
urgent message to his/her family, such request can be made to GTA-Global Travel Assistance Operation
Center.
GTA-Global Travel Assistance will not be held responsible for error in the message or incomplete messages.
V - EXPENSES COVERING FLIGHT DELAY OR CANCELLATION
If the beneficiary’s flight has a delay of more than 06 (six) consecutive hours as from the time scheduled
for departure of the original flight, and only when no other transportation alternative is available during

these 06 (six) hours, GTA-Global Travel Assistance will reimburse the Holder of the “Service Order” (voucher) up to the value established for the purchased plan, for the expenses covering hotel, meals (without
alcoholic beverages) and communication (excluding Internet expenses) incurred during this period, provided that all the original receipts are presented along with a statement from the Air Company informing
the reason for such delay or cancellation of the beneficiary’s flight, as soon as he/she returns to his/
her domicile country.
WARNING: This benefit will be cancelled in situations when the flight cancelation is a consequence of natural occurrences as: catastrophe, earthquake, rebellions, riots, acts of guerilla or against guerilla, hostilities,
retaliation, conflicts, embargos, and acts of violence, mobilization of political nature, strikes, popular acts,
lock-out events, or any act of sabotage or terrorism.
VI - ASSISTANCE IN CASE OF LOSS OF DOCUMENTS, AIR TICKETS AND
LUGGAGE (GLOBAL AND EURO SPORTS PLAN)
GTA-Global Travel Assistance will help the beneficiary, providing information, as related to the procedures
to submit claims in case of loss, theft of documents, luggage and personal items, making available to the
beneficiary the same services of its nearest Operation Center.
VII - LEGAL ASSISTANCE
GTA-Global Travel Assistance will be responsible for the expenses generated by the civil defense of the
holder that is being accused of responsibility in a traffic accident, covering the attorney’s fees up to the limit
established in the plan acquired.
VIII - BAIL BOND ADVANCE FOR TRAFFIC ACCIDENT
If the beneficiary is arrested and is assigned the criminal responsibility for a traffic accident, it can contact GTA-Global Travel Assistance to obtain the financial intermediation service in order to pay the bond
required for his/her parole.
a) The amount to be transferred from the country of origin to the country of arrest shall not exceed the limit
indicated in the plan acquired.
b) If during the proceeding, the contracting party does not comply with any Court LEGAL NOTICE, GTAGlobal Travel Assistance may request the immediate reimbursement of the financial advance.
c) The request covering the amount limited to the plan acquired can be submitted in 02 (two) manners:
1st - by holder’s request: GTA-Global Travel Assistance Operation Center will contact the family and/or
friends that are indicated as reference in the country of origin to transmit the request made by the Holder to
send given cash for payment of the bond. After the deposit of the amount by the holder to the GTA-Global
Travel Assistance office in the country of origin, Av. Ipiranga nº 318 - Bloco B -16º andar - Centro - São
Paulo, the payment order will be place to the detention location. The cash will be delivered in North American Dollars or in local currency within a period of 36 (thirty six) hours.
2nd - through signature of a Certificate of Indebtedness with actual/personal warranty, considered by
GTA-Global Travel Assistance as suitable and satisfactory, if the family and/or friends do not have the
amount of the bond or the amount of the cash limit indicated in the plan acquired by the Holder immediately
available. The amount expressed in the Certificate of Indebtedness will be returned to GTA-Global Travel

Assistance by the Holder, to Brazilian Assist Repres. e Turismo Ltda., Av. Ipiranga nº 318 - Bloco B 16º andar - Centro - São Paulo no later than 60 (sixty days), in local currency converted by the tourism
exchange rate of the North American Dollar in force at the date of the reimbursement.
IX - INDEMNITY FOR PERMANENT LOSS OF LUGGAGE
1 - Holder of the “Service Order” (voucher) will be entitled to an indemnity for permanent loss of luggage.
Coverage has as object the luggage sent for shipment at time of check-in, on an international flight or on
regulated charters.
a) No type of indemnity will be paid related to loss of non-dispatched hand-bags or luggage violated or
damaged as to its contents.
b) No type of indemnity will be paid for loss of packages independently of their content.
2 - In case of loss of luggage the Holder of the “Service Order” (voucher) should take the following measures:
a) Fill in the form corresponding to registration of loss provided by the airline: Original of PIR (Property Irregularity Report), to allow the airline to look for and find the luggage.
b) Concurrently, before leaving the airport, communicate the fact to GTA-Global Travel Assistance Operation Center, informing the name of the airline, flight number, destination and number of the PIR (Property
Irregularity Report).
c) GTA-Global Travel Assistance is ready to guide the passenger / holder as related to the procedure of
following-up the airline search and locating the luggage.
d) The airline will indemnify the passenger upon officially recognizing loss of luggage.
e) When indemnified by the airline, the holder should submit the original of the receipt and copy of the
check or of any other document that evidences payment effected by the airline, so that the Assistance
Center can proceed with payment of indemnity within the amount corresponding to the limit of coverage
included in the plan acquired.
f) The indemnity will be paid to the beneficiary in the country of origin where his/her GTA-Global Travel
Assistance card was issued no later than 30 (thirty) days as from receipt by the Operation Center of
all the documentation.
3 - Amount of the Indemnity according to the hired plan: when the luggage is declared as definitely lost
by the airline, the Holder of the “Service Order” (voucher”) will be entitled to a supplementary indemnity
equal to that established by the airline, but always limited to the amounts indicated in the General
Conditions of the hired plan.
3.1 - For effectiveness of the supplementary indemnity corresponding to the hired plan, the Holder should
forward to the travel agent or to the tourism operator, or even directly to the office of GTA-Global Travel Assistance, to Brazilian Assist Repres. e Turismo Ltda. - Av. Ipiranga nº 318 – Bloco B - 16º andar - Centro
- São Paulo or in the location where the “Service Order” (voucher) was issued, the following documents:
• Original of PIR form, according to Warsaw convention;
• Original of GTA-Global Travel Assistance Service Order (voucher);
• Document that evidences payment effected by the airline;
• Having informed GTA-Global Travel Assistance before leaving the airport, indicating the number of the
assistance request submitted to GTA-Global Travel Assistance Operation Center. All these documents
must be presented in their original forms;

• The documentation submitted will only be valid if submitted in ORIGINAL forms;
• GTA-Global Travel Assistance does not indemnify violation of luggage, even if the airline does it;
• The indemnity to be paid by GTA-Global Travel Assistance covers only the limit established in each plan,
and does not include indemnity for pain and suffering.
4 - Exclusions of this coverage:
a) Confiscation or seizure on behalf of Customs or any governmental authority;
b) Act as operator or member of the crew of the transportation mean that gave origin to the event;
c) Not notifying the transporting company through filling in of the loss registration document: PIR (Property
Irregularity Report) before leaving the arrival location;
d) Not take the required measures to protect and/or recover the lost luggage;
e) Damage to luggage and/or objects or amounts missing, will not be indemnified by GTA-Global Travel
Assistance, even if the airline proceeds with the corresponding indemnity.
X - COMPENSATION FOR DELAY IN LUGGAGE DELIVERY
Brazilian Assist Repres. e Turismo Ltda., Av. Ipiranga nº 318 - Bloco B - 16º andar - Centro - São Paulo
will reimburse the passenger, as related to the amounts spent in the purchase of basic articles, through
submittal of original of supporting documents, IF SEARCH AND LOCATION OF DISPATCHED LUGGAGE BY
THE AIRLINE IS UNSUCCESSFUL, WITHIN THE FIRST 06 (SIX) HOURS AS FROM THE COMMUNICATION
OF LOSS TO THE OPERATION CENTER.
1) The limit of coverage for the plan acquired is specified in the General Conditions.
2) To be entitled to the reimbursement, the passenger should remain overseas during the minimum period
of 06 (six) hours.
3) If the luggage is declared as definitely lost, and if the corresponding indemnity has been paid by the
airline, THE AMOUNT PAID BY THIS BENEFIT WILL BE DEDUCTED FROM THE AMOUNT PAID AS PER
CLAUSE X - INDEMNITY FOR LOSS OF LUGGAGE, OF THE PRESENT CONTRACT.
4) THIS BENEFIT WILL NOT BE DUE if loss of luggage occurs during de return trip of the beneficiary to the
country of issue of the GTA-Global Travel Assistance voucher, and/or normal residence of the holder.
XI - TRIP CANCELLATION
1 - Cancellation of trip will occur as consequence of:
a) death or serious illness, considered as urgent, of the Holder and which requires hospitalization, or prevent his/her transportation, generating a state of prostration that precludes start of trip.
b) death or hospitalization caused by accident or illness declared as sudden and acute, of spouse, parents,
siblings or children of the holder.
The enumeration is limitative and not exemplifying.
GTA-Global Travel Assistance reserves the right to request documentation that it considers as convenient
to evidence the reason for trip cancellation.
WARNING: coverage in cases foreseen in letter “b” above will be extended to spouse, and children of the
holder, if they are travelling with the Holder and are also beneficiaries of the GTA-Global Travel Assistance.
2 - Validity of coverage: Holder should acquire the GTA-Global Travel Assistance tourist package and

“Service Order” (voucher) 15 (fifteen) days prior to the date of start of the trip.
This benefit will terminate when the Holder enters the airplane that will transport him/her to the initial destination of the trip.
This benefit will lose its validity if the Operator or the Travel Agency covers this risk.
3 - Reimbursement of part of the amount by the tourism Operator/agency: If the Operator or the Agency
reimburses the passenger for part of the amount paid, the passenger will be entitled to the difference
between the amount reimbursed and the maximum limit of the coverage established for the plan acquired.
4 - Assured compensation: Holder of GTA-Global Travel Assistance “Service Order” (Voucher) that purchased an international tourist package from an operator or travel agency, will be entitled to a compensation related to the amount of the contractual penalty charged by the operator, having as maximum value
the amount indicated in the plan acquired, AS LONG AS HE/SHE SUBMITS THE ORIGINAL OF PAYMENT
VOUCHER OF SUCH PENALTY AND THE CONTRACT SIGNED WITH THE TOURISM OPERATOR. This benefit
has no age limitation.
WARNING: Calculation of this compensation has as basis the exchange rate in force on the date of
contracting.
5 - Exclusions of the coverage: Cancellation of the trip due to the following reasons is excluded from the
present coverage:
a) The date of hiring the product by the Holder of the Voucher;
b) Chronic or pre-existing illnesses suffered by the: spouse, parents, siblings or children of the Holder of the
Voucher as well as their aggravations, consequences and sequels.
c) Accidents caused directly or indirectly by practicing dangerous sports, such as: mountaineering, ski out
of regulated lanes, water ski, snowmobiling, motoring, motorcycling, boxing, parachuting, paragliding,
hang-gliding, and any other sport or athletic competition or acrobatics or that has as object competitions
of exceptional character, during participation in trips or excursions to unexplored locations.
d) Participation in criminal acts;
e) Injuries that the insured individual causes to himself/herself;
f) Alcoholism;
g) Use of drugs, narcotics or use of medication without medical recommendation;
h) Pregnancy;
i) Any type of mental disease/disorder.
6 - Subrogation: Holder to the GTA card transfers to GTA-GLOBAL TRAVEL ASSISTANCE all his/her corresponding rights and actions against natural individuals or legal entities covering damages or loss that have
been caused up to the sum that GTA guarantees due to claim reimbursement.
7 - Episodes: in the episodes foreseen in letters “a” and “b” of item 1 of the present clause, the holder of
GTA-Global Travel Assistance voucher - or in case of death of the holder, his/her spouse, parents, siblings
or children should communicate the fact to GTA-GLOBAL TRAVEL ASSISTANCE, in an evidenced manner,
no later than 24h of the event, under the risk of not receiving the compensation assured in this clause,
so as to enable checking of the communication fact with the medical team.

In addition, the insured individual should submit together with the compensation request, the following
documents:
• two-way tickets;
• GTA-Global Travel Assistance voucher;
• bills and receipts of payment effected to the agency or operator;
• in case of accident or illness, complete medical documentation with clinical history, etc;
• in case of death, a notarized copy of the Death Certificate should be forwarded;
• evidence of family relationship indicated in letter “b” of item 1 of the present clause;
• notarized copy of the CPF – Individual Taxpayer’s Registration Number

1 - HOLDER

b) All holders of the GTA-Global Travel Assistance card with age limited to 80 years are 50% (ONE HUNDRED PERCENT) covered by the insurance, as long as such cards are issued in their name, and are valid
during the trips leaving Brazil to other countries and as long as such trip is a two way trip.
2 - COVERAGE
The indemnity will be paid only in case of accidental death of the insured individual occurred during the
period of validity of the insurance. Such indemnity will be limited to the expenses covering funeral, duly
evidenced by means of original bills, when the insured individual is under 14 (fourteen) years old.
3 - INSURED AMOUNT
The insured amount will be equivalent to the plan acquired by the Insured Individual

4.1 - Suicide or suicide attempts.
4.2 - Competitions in vehicles, including preparatory training.
4.3 - Acts of declared or not declared war.
4.4 - Mental disorders resulting from the use of alcohol, drugs, narcotics or toxic substances.
5 - CLAIM NOTIFICATION
Any claim should be communicated to GTA-Global Travel Assistance Operation Center no later than 5 (five)
business days as from the date of occurrence of the event covered by this insurance.

The indemnities will be paid after analysis of all the supporting documentation delivered to the insurance
company within a period of 15 (fifteen) business days after its receipt.
6 - BENEFICIARIES
The indemnities will be paid automatically to the beneficiaries in the following manner:
1) Married insured individual: to the spouse, and in his/her absence, to the children of the couple in equal
parts;
2) Single insured individual: to the parents in equal parts and in their absence to the siblings in equal parts;
3) Other Situations: to the legal heirs in compliance with article 792 and following clauses of the Brazilian
Civil Code (Law No.10.046, dated 01/10/2002).
7 - FINAL PROVISIONS
7.1 - All the clauses of the general conditions included in the collective policy covering personal accidents,
not modified by these conditions, are applied to this insurance.
7.2 - The general conditions and terms are filed at GTA-Global Travel Assistance.

1 - Basic warranties
1.1 - Accidental Death: Assures payment of the insured capital contracted for this warranty to the
beneficiary(ies) of the Holder of the Insurance, in case of accidental death of the insured individual, resulting uniquely from covered personal accident and occurred exclusively within the period of validity of the
certificate, except if resulting from excluded risks.
1.1.1 - The warranty of accidental death of the insured individual with less than 14 (fourteen) years
old, is designed only to reimbursement of the expenses covering funeral; such expenses should be
duly evidenced through submittal of original of supporting documents; within the expenses covering
funeral those related to transportation are included. Expenses incurred with acquisition of land, graves
or charnel-houses are not covered. This reimbursement will be limited to the amount of the insured
capital contracted for this warranty.
1.2 - Total or Partial Permanent Disability due to Accident: assures to the insured individual payment of
the insured capital contracted for this warranty, taking into consideration the Disability table included in item
2 (two) of these conditions, in case of occurrence of a permanent disability of total or partial nature of one

member or organ, resulting from physical or mental injury caused by accident duly covered, when such
accidents occur within the period of insured travel, except if resulting from excluded risks.
1.2.1 - Permanent Total or Partial Disability are considered as the disability that is confirmed within the
period of up to 01 (one) year as from the date of the accident, after conclusion of treatment and final medical
discharge, as long as all the therapeutic resources available for recovery have been exhausted.
1.2.2 - The Insurance Company reserves the right to submit the Insured Individual to medical examination
in order to evidence the disability and/or evaluation of the disability level, under the risk of non-payment of
the indemnity.
1.2.3 - The permanent disability should be evidenced by submittal of a medical statement to the Insurance
Company.
1.2.4 - As related to under-aged individuals, the indemnity covering Permanent Disability will be paid in the
following manner:
a) Individuals less than 16 (sixteen) years old - the indemnity will be paid in name of the insured under-aged
individual, through a judicial authorization.
b) 16 (sixteen) to 17 (seventeen) years old Individuals - the indemnity will be paid to the insured under-aged
individual duly assisted by his/her father, mother or finally by his/her guardian or legal representative.

3 - main risks excluded from all warranties:
I. The events occurred in consequence of the following risks are excluded from insurance coverage:
a) Pre-existing diseases to the period of travel (validity of insurance) and any of their consequences,
including recovery phase and disorders under treatment and not consolidated, consequent of facts
prior to trip. This exclusion does not apply to coverage related to corpse repatriation, accidental
death and disability of permanent nature resulting from the accident;
b) Any mental disorders directly or indirectly resulting from use of alcohol, drugs, narcotics or toxic
substances;
c) Suicide whether premeditated or not and its attempt, if occurring within the first two years of initial
validity of the contract or of its re-validation after suspension;
d) Damages caused by illicit and malicious acts performed by the insured individual;
e) Use of nuclear material including intentional or not nuclear explosion as well as radioactive contamination or exposure to nuclear or ionizing radiation.
f) Hijack or hijack attempt;
g) Acts or operation of war;
h) Act of terrorism;
i) Tornados, cyclones, earthquakes, seaquakes, volcanic eruptions and other natural disasters;
j) Risks resulting from the following events, that are not included in the concept of personal accident;
k) Injuries resulting, depending, arranged or facilitated by repetitive efforts or cumulative micro-traumas, or having relation of cause and effect with such traumas, as well as injuries classified as:
repetitive strain injuries - RSIs, work-related musculoskeletal disorder - WMSDs, cumulative trauma
injuries/disorders - CTIs, or similar that can be accepted by the medical-scientific organizations, as
well as their consequences, treatments and post-treatments, including any time surgeries.
l) The situations recognized or compared by the official institutions of social security or similar entities, to “accidental disability”, in which the event that caused the injury does not integrally fit the
characterization of disability due to personal accident.
4 - Insured individual: Is the holder of the GTA-Global Travel Assistance travel card. The insurance will not

be contracted by the holder of the card, if such holder is more than 70 (seventy) years old.
5 - Insurance Premium: As from his/her adhesion to the policy, the insurance premium will be collected
automatically and in a onetime payment together with payment of the Policy Holder card.
6 - Individual Insured Capital: The Individual Capital will be defined according to the event covered, as per
Policy Holder plan table.
7 - Beneficiary Clause:
7.1 - For coverage of Accidental Death of the Insured Individual, the beneficiary of the indemnity will be
defined according to the Legislation in force, taken into consideration the limits indicated in the Policy
Holder plan table.
7.2 - Any changes will be in effect after its receipt by the Insurance Company, if submitted prior to the date
of the claim.
7.3 - In the absence of the named Beneficiary, the indemnity will be paid in compliance with the Legislation
in force.
7.4 - In case of Permanent Total or Partial Disability due to Accident, the beneficiary will be the Insured
Individual himself, taken into consideration the limits included in the Policy Holder plan table.
8 - Insurance Validity:
8.1 - Start of Validity: The insurance will start as from 0:00 (zero) hour of the day of the trip.
8.2 - Validity: The GTA-Global Travel Assistance Insurance, which validity is equivalent to the quantity of
days contracted for the trip, will be valid only after payment of amount related to the insurance.
9 - Insurance Grace Period:
9.1 - Accidental Death: Claims resulting from Personal Accident have no grace period, except in case of
suicide or its attempt, when such period will correspond to two uninterrupted years, as from the date of
adhesion of the insurance or its re-validation after suspension.
9.2 - Permanent Disability whether Total or Partial due to Accident: no grace period for this coverage.
10 - Insurance Deductible: Accidental Death and Permanent Disability whether Total or Partial due to Accident - no deductible for this coverage.
11 - Cancellation of Individual Coverage: the coverage related to individual risk is cancelled for any
Insured Individual:
a) With cancellation of the relationship between the Insured Individual and the Policy Holder.
b) Due to fraud or fraud attempt, due to misconduct or false statements on behalf of the Insured Individual or of his/her Beneficiary (ies).
c) In case of cancellation of the Policy, following the rules established in the General Conditions;
d) In compliance with the other Policy Conditions;
e) The insurance will be integrally rescinded in case of Accidental Death or Permanent Total Disability
due to Accident of the Insured Individual.
Note: The present insurance is valid for a given period of time; however the Insurance Company has the
right not to renew the policy on the date of its expiration, without any return of the insurance premiums
paid under the terms of the present policy.
12 - Claims
12.1 - In case of claim, the Operation Center (11 3150-4511) should be directly and immediately com-

municated by the Beneficiary (ies) of the insurance which will request the documents required for analysis
of the event.
12.2 - The documentation requested by the assistance center is referential, as, during the analysis and
regulation of the claim, other documents can be requested for clarification and/or evidence of the claim, and,
as from this moment, the Insurance Company has the right to demand them.
12.3 - After submittal of all the required documentation on behalf of the Insured Individual, for settlement
of the claim, the Insurance Company will effect payment of the due indemnity within a period of 30 (thirty)
days.

13 - Additional Conditions:
13.1 - The insured individual may check the registration condition of its insurance broker visiting the site
www.susep.gov.br, through the number of its registry at Susep (Superintendence of Private Insurance),
name, CNPJ (National Register of legal Entities’ number) or CPF (Individual Tax payer’s Number).
13.2 - The acceptance of the insurance is subject to risk analysis.
13.3 - Registration of this plan at Susep (Superintendence of Private Insurance) does not imply, on behalf
of the Autarchy, in incentive or recommendation to its trade.
13.4 - THIS INSURANCE MATERIAL PRESENTS A SUMMARY OF THE MAIN CHARACTERISTICS OF THE
PRODUCT. ADDITIONAL CLARIFICATIONSCAN BE FOUND IN THE GENERAL CONDITIONS THAT RULE THE
PRESENT INSURANCE AND WHICH ARE AVAILABLE IN THE SITE www.gtaassist.com.br, OR BY CALLING
THE OPERATION CENTER - (11) 3150-4511.
14 - Forum: The forum of domicile of the Insured Client is elected to settle any doubt resulting from execution of the present Summary of Insurance - GTA-Global Travel Assistance.
XIV - CONDITIONS FOR REIMBURSEMENT OF EXPENSES OF BENEFITS
INCLUDED IN THE GENERAL CONDITIONS
a) Conditions for Reimbursement: also for effect of reimbursement the cause of assistance should not be
included in the Exclusions item.
b) Communication to Operation Center: the holder should communicate the GTA-Global Travel Assistance Operation Center within the first 24 (twenty four) hours after the consequent event due to accident
or illness, informing the data related to the hired doctor and/or health center, as well as the reasons that
prevented him/her from requesting the assistance directly to the GTA-Global Travel Assistance Operation
Center, under the risk of not being entitled to reimbursement of the expenses incurred by the services.
The inexistence of information about the event at the Operation Center corresponding to the location of
the occurrence CANCELS PAYMENT OF THE REIMBURSEMENT.
c) Adequacy to the rate in force: cost of the contracted service should be in compliance with the prices
and rates in force in the country in which it has been performed.
d) Suitable documentation: the holder or the service provider, whenever the case, should submit a supporting documentation (invoices and receipts) that includes the amount of the expenses incurred, and the
GTA-Global Travel Assistance Operation Center has the right to check the accuracy of their content and/or
request additional clarifications.

1 - The documentation submitted should be in original forms, at the time of the beneficiaries return to the
country of origin and should include, when concerning a medical service: clinical history, medical diagnosis
and prescription, original voucher with date of issue, start of trip, issuing agency and age of holder attached
to a request of the holder reporting the occurrence.
2 - The documentation should be sent to the GTA-Global Travel Assistance Center (Av. Ipiranga, 318 Bloco B - 16ºandar, São Paulo - SP, CEP 01046-010) within maximum 10 (ten) days after the expiration of
the voucher. After this period of time, the holder will have no right to the reimbursement. Only expenses duly
evidenced by original documents will be reimbursed.
3 - Payments in foreign currency will follow the same exchange rate of American Dollars used at the
GTA-Global Travel Assistance Voucher issue date.
4 - Payment of reimbursement will be effected within a period of 30 (thirty) days as from the date of receipt
of the documentation by the Operation Center.
ASSISTANCE CONTRACTED DIRECTLY BY THE HOLDER: In case of direct contracting by the Holder of
professionals and/or hospitals not indicated by GTA-Global Travel Assistance Operation Center, the following conditions should be complied with:
1- Communication to GTA-Global Travel Assistance Operation Center: the event should be communicated
to GTA-Global Travel Assistance Operation Center within the maximum period of 24 hours after the event,
as well as the data related to the professional and/or hospital that has been contracted, and the reasons that
prevented submittal of a request of assistance to the Operation Center of GTA-Global Travel Assistance.
2- Authorization of GTA-Global Travel Assistance Operation Center: all medical assistance should be previously and expressly authorized by the Operation Center and fit the nature of the disease or injury suffered,
according to the circumstances of the case.
