Dear Editor, We recently conducted a prospective study in the emergency department of a public hospital (São Paulo Hospital - SPH) in São Paulo City, Brazil. The SPH is one of the most important in São Paulo City and provides public health service for patients from different regions of Brazil. Annually, SPH has more than 1,080,000 consultations and 31,200 admissions. About 4,000 outpatients, including 1,000 in the emergency department, are served daily1. Because of these large numbers; we believe that assistance provided by SPH reflects the diversity of the Brazilian population. The aim of our study was to characterize a sample of patients who had suffered cardiac arrest and identify risk factors associated with mortality and morbidity. The sample was composed of 285 patients who were followed for 1 year after a cardiac arrest. In the sample, three patients were infected with HIV: two Caucasian males, ages 57 and 65 years, who unfortunately died during emergency care, and a 51-year old Caucasian male who survived for 1 year after the event. As individuals infected with HIV have experienced longer life expectancy with the evolution of pharmacological therapy, their rates of cardiovascular disease have increased progressively. These patients have higher rates of cardiovascular disease than uninfected controls, likely due to a combination of traditional risk factors, HIV-related inflammation, and antiretroviral therapy2. Tseng et al.3 determined the incidence of sudden cardiac death in patients with HIV. They followed 2,860 consecutive patients attended in a public HIV clinic. They found that sudden cardiac deaths comprise an unexpectedly high proportion of overall deaths in this HIV cohort and that sudden cardiac death occurred at a rate more than four times expected in the general population, with similar risk factors. As cardiac symptoms were common in victims of sudden cardiac death, the authors suggested that aggressive primary prevention of cardiovascular disease should be considered in HIV-infected individuals, especially those with traditional risk factors. Thus, education programs for cardiac arrest recognition targeted to health professionals and caregivers can improve the prognoses of individuals infected by HIV. Additionally, it is necessary 5 to disseminate basic life support knowledge to the general population , especially for caregivers and family members, because this knowledge could increase the chance of survival before the arrival of advanced life support.
