Clin Exp Allergy 2004; 34:285-90 Anaphylaxis - drugs 40 patients • 24 NSAID (propyphenzon, diclophenac, ASS, ibuprofen, mefenamic acid) • 7 Antibiotics (amoxicillin, cefalexin, ceftriaxon, chinolon, trimethoprim, rolitetracyclin) • 4 Local anaesthetics (bupivacain, lidocain, mepivacain) • 2 RCM (ioversolum, iapomirol) • 1 Antimalaria (chinin), muscle relaxans (Chlorzoxazon), plasma expander (polygeline) Helbling A 2004 Relevant drugs causing anaphylaxis, which were missing in publication of 2004 (CH): a) perioperative anaphylaxis: muscle relaxants like rocuronium, succinylcholine… dyes: patent-blue, methylen-blue,… desinfectants: chlorhexidine b) Novel drugs/elicitors PPI like pantoprazole, omeprazole biologicals Drug induced anaphylaxis is particularly dangerous 1999-2010 (USA): • The most common cause of anaphylaxis death was drugs (58.8%, or 1446 deaths out of 2458), with no significant difference between the sexes. • RCM, muscle relaxants, antibiotic (cefuroxim….), venofer/ferrinject, specific s.c. Es werden sehr hohe Menge des Antigens rasch i.v. injiziert und damit IgE-bestückte Mastzellen überflutet: selbst IgE mit geringer Affinität für das Medi bindet genügend Antigen 2. Sensibilisierung, also die Bildung von specifischem IgE, geht einher mit einer Desensibilisierung der MC 2. «Cross-linking» Fähigkeiten eines Haptenveränderten Proteins ist höher als die eines Proteins 1. Crossreactivity and affinity: dose is important pholcodin rocuronium Pholcodin: induces high affinity IgE for Pholcodin. Reaction at minimal doses (1mg) Crossreactive with other drugs containing quarternary N structure, but with lower affinity: (100mg) rocuronium pholcodin Pholcodin and rocuronium can both bind to pholcodin specific IgE; But rocuronium is binding less affine and thus needs more molecules to engage IgE. → High dose can overcome low affinity. High drug dose may also trigger less specific IgE. High drug dose enables triggering of less affine IgE and explains anaphylaxis at first sight Sensitization (development of specific IgE) goes along with a Desensitization of mast cells. Anaphylaxis is an abnormal process elicited by the sudden encounter of IgE-loaded mast cells with high concentrations of allergen. 2. Desensitization: slow increase of antigen exposure avoids MC degranulation • Sensitization: slow increase of IgE – constant antigen concentration allergen D: difference of IgE is low: «tolerance» Specif. IgE • Desensitization: stepwise (slow) increase of antigen, at already high/constant IgE level Specif. IgE allergen Sensitization and desensitization: No symptoms in spite of presence of specific IgE IgE - crosslinking by drugs Cross-linking of two IgE molecules on distinct Fc-IgER is necessary; ? How can a small molecule crosslink two IgE ? ? Difference to protein antigens ? IgE-model: size comparison of Bet v1 and amoxicillin Bet v1 Amoxicillin IgE two distinct epitopes IgE cross-linking by protein or drug allergens • At least two distinct epitopes on one molecule required; • Crosslinking requires two distinct IgE molecules, able to react on two sites/ epitopes of the protein • Haptens form identical epitopes on different positions of the protein; The hapten specific IgE recognizes the hapten on distinct sites of the protein: this enables crosslinking of even the same IgE! Vernetzung der IgE ist nur möglich, falls die zu vernetzenden IgE mit unterschiedlichen Epitopen + im Protein reagiert. Wenn Haptene an unterschiedliche Stellen im Protein binden, können alle Haptenspezifischen IgE reagieren und die Vernetzung ist erleichtert. This may greatly facilitate cross-linking and degranulation ! and could explain the severity of drug induced anaphylaxis Anaphylaxie Symptome, wann Adrenalin? Anaphylaxis - Symptoms 100% with hypotension/loss of consciousness and/or shock (definition) • Of 118 patients: • 88% cutaneous symptoms – pruritus (88), urticaria (72), angioedema (62), erythema (44); • 72% respiratory – dyspnoe (83), larynxedema (8), bronchospasm (8), stridor (4); • 44% gastrointestinal – malaise (38), vomiting (19), diarrhoe (9), cramps (7), dysphagia (5), incontinence (3); Helbling A, Hurni T, Mueller UR, Pichler WJ. Guidelines and use of adrenalin Canadian study 2013, Spanish study 2014, Swiss experiene (Helbling): Anaphylaxis accounted for a substantial number of ED visits in adults, and the most common trigger was food. There is non-adherence to guidelines recommending epinephrine use for all cases of anaphylaxis We postulate that this may be related to concerns regarding the side effects of epinephrine in adults. Is it just non-adherence? or is it often too late for adrenaline ? Anzahl und Sensitivität der entsprechenden Test ist limitiert Wenig hilfreich: penicillins, suxamthonium, amoxicillin,…. gut: chlorhexidin Viele relevant IgE Bestimmungen auf Medikamente fehlen: Metamizole, Chinolone, PPI, Rocuronium, Patent-blue, Methylen blue,… etc…. 2-malige fast-Anaphylaxie • 42jr Frau ohne Asthma oder Polyposis nasi nahm wegen Rückenschmerzen Schmerzmittel ein (75 mg Diclofenac, 500mg Mefenaminsäure). Sie beobachtete 2x palmaren und plantaren Pruritus, Urtikaria, Gesichtsschwellung und Dyspnea, ca. 2 und 4hr danach. PGSH (NSAID) Intoleranz oder ? Bei der Anamnese interferiert der Ehemann und sagt, sie habe das Ponstan (Mefenaminsre) doch unlängst genommen und kein Problem gehabt. Sie nehme doch noch anderes … Die Patientin hat vergessen, dass sie jedesmal auch ein PPI (Pantoprazol), wie vom HA verschrieben, einnahm. Ja, unlängst als sie das Ponstan vertragen habe, hatte sie das Pantoprazol vergessen. Basophil Activation Test (BAT): Stimulation von Basophilen durch Zugabe des Medikamentes 80 CD63 (%) 60 - IL3 +IL-3 40 20 0 none anti-IgE fMLP 50 10 Pantoprazol 1 Identification of elicting drug causing (severe) drug hypersensitivity Basophil Activation Test (BAT) • Zellulärer Test mit Basophilen als Indikator Zellen für IgEvermittelte Reaktionen (basierend auf derExpression von FceRI) • Ein funktioneller Test, der nicht nur von IgE abhängig ist • Kann leicht bei verschiedenen Medi eingesetzt werden • Direkter BAT Basophils Patient + Drug = direkter BAT Identification of elicting drug causing (severe) drug hypersensitivity Direkter BAT bei Medikamenten-Allergie Routine-Tests (direct BAT): • neuromuscular blocking agents (NMBAs) • beta lactam antibiotics (amoxicillins, clavulanic acid, penicillins, cephalosporins) • fluoroquinolones • pantoprazol • carboxymethylcellulose (CMC) • metamizol Einschränkungen: • Schwieriger Test (Speziallabor) • Frisch - Blut (!!!) • „Basophil-releasibility“ unterscheidet sich von Patient zu Patient Identification of elicting drug causing (severe) drug hypersensitivity Indirect BAT Donor basophils + patient serum mit Auslöser Provokationstest ? + Positiver Test beweist eine Reaktion mit Alternative Negativer Test: Kofaktoren fehlten? belastend, aufwendig, Kreuzreaktion? Mechanismus? Provokationstest mit Auslöser: nur wenn unumgänglich Provokationstest mit Alternative: wichtig, «reassuring» bei NSAID Intoleranz obligat! Therapie, Prävention Desensibilisierung ist eine Option, aber nur transient wirksam, und muss vor jeder Gabe wiederholt werden Adrenalin Injektor (?) falls Wahrscheinlichkeit gross für zufällige Reexposition Information Klare Aussage Gute Alternative Genau und wie schwer? Kreuzreaktionen? Toleranz anderer Medikamente? Kenne die wichtigsten Auslöser! • Achtung vor rascher Allergen Beladung! • Angst!! Palmarer Juckreiz Adrenalin i.m.: es ist nie ein Fehler! • BAT als neue Möglichkeit • Besser mit Alternative! • Information und lassen Sie den Patienten mit Nadel stechen…. Before you prescribe an AdrenalinInjector for self application (Epipen, Jext,..) Let the patient practice • With attrappe • Most patients are afraid to stick a needle deep in their skin ! • Take him the fear to use a needle by letting him/her apply a sterile 19G needle on his/her thigh… • If he/she realizes, it does not hurt, he/she will be thankful and is ready to use it
