Prefácio A Doença de Hansen, na África é chamada de lepra, porém no Brasil o termo "lepra" é proibido, lá chama-se Hanseníase. Para simplificar o uso do nome desta enfermidade e também como as fotos foram tiradas em países africanos, na versão portuguesa deste livrinho será usada a forma internacional, o termo latino: Lepra. Segundo a actual nomenclatura das formas clínicas da lepra, subentende-se na forma paucibacilar (até cinco manchas típicas) e multibacilar (mais que cinco manchas típicas ou qualquer caso com baciloscopia positiva). Como paucibacilar (PB) abrange-se as formas indeterminada, tuberculóide e "borderline" (dimorfa), enquanto haja até cinco manchas, a baciloscopia deve ser negativa. Como multibacilar (MB) abrange-se as formas "borderline" (dimorfa) e lepromatosa (Virchowiana), enquanto haja mais que cinco manchas ou no caso que a baciloscopia seja positiva. Para facilitar o entendimento, decidimos adotar o Português de Portugal e não o Português do Brasil, mesmo sabendo que vivem mais doentes de Lepra no Brasil do que nos países lusófonos da África. 1. INDETERMINATE LEPROSY - PB A single, symptomless, slightly hypopigmented, ill-defined macule. The edges are clear but seem to fade into the normal skin. The surface of the lesion is even and smooth, not dry. Slight loss of temperature and pain sensation was found but light touch sensation was preserved. Peripheral nerves were not enlarged. No leprosy bacilli were found. BI = 0. 1. LEPRA INDETERMINADA - PB Mácula única, sem sintoma, levemente hipopigmentada e mal delimitada. As bordas são visíveis, mas com impressão desbotada dentro da pele normal. A superfície da lesão é plana e lisa, não é seca. Foi encontrado uma ligeira perda da sensibilidade térmica e dolorosa, mas a sensação táctil estava preservada. Os nervos periféricos não estavam enlargados. Não foram encontrados bacilos da Lepra. 1 2. TUBERCULOID LEPROSY - PB Hypopigmented macular flat lesion. The edges are well-defined in some parts of the lesion, indefinite in others. Biopsy revealed features characteristic of tuberculoid leprosy. BI = 0. 2. LEPRA TUBERCULÓIDE - PB Lesão macular plana e hipopigmentada. As bordas são bem delimitadas na maior parte das lesões, não delimitadas em outras. A biópsia revelou aspectos característicos da Lepra tuberculóide. 2 3. TUBERCULOID LEPROSY - PB A single, irregular, flat, hypopigmented lesion on the forehead. The edge of the lesion is well-defined, the surface of the lesion is drier than that of the surrounding skin. It was difficult to find loss of sensation within the lesion. Lesions on the face are less likely to be insensitive because of the rich and overlapping nerve supply to the skin of the face. Left ulnar nerve was enlarged. BI=0. 3. LEPRA TUBERCULÓIDE - PB Lesão única, irregular e hipopigmentada na testa. A borda da lesão é bem delimitada, a superfície da lesão é mais seca do que a pele ao redor da mancha. Foi difícil encontrar perda de sensação no interior da lesão. As lesões no rosto são provavelmente sensíveis, porque a pele da face é rica e sobreposta de nervos sensitivos. O nervo ulnar (cubital) esquerdo estava enlargado. A baciloscopia foi negativa. 3 4. TUBERCULOID LEPROSY - PB A single, raised, well-defined lesion lighter than the surrounding skin, pebbled and slightly erythematous at the edges, flatter and darker at the centre (starts to heal there). Quite marked loss of sensation within the lesion. Peripheral nerves were not enlarged. BI = 0. 4. LEPRA TUBERCULÓIDE - PB Lesão única, elevada e bem delimitada, mais clara do que a pele ao redor. As bordas são rugosas e levemente eritematosas, o centro é mais plano e mais escuro (aqui começa a sarar). Clara falta de sensibilidade no centro da lesão. Os nervos periféricos não estavam engrossados. A baciloscopia foi negativa. 4 5. TUBERCULOID LEPROSY - PB Two ring-like, well-defined lesions with fine papular edge and flat central area. The centre of the lower lesion regains the colour of the surrounding skin (central healing) and in a portion of the ring the papules disappear. The surface of the lesion is dry. Sensation to light touch was grossly diminished within both lesions. Right peroneal nerve was enlarged. BI = 0. 5. LEPRA TUBERCULÓIDE - PB Duas lesões semelhantes a um anel, bem delimitadas. As bordas apresentam pápulas finas e a área central é plana. No centro da lesão mais inferior, a cor da pele tornou-se pigmentada como a pele ao redor (centro sarado). A superfície da lesão é seca. A sensibilidade táctil estava claramente diminuida em ambas lesões. O nervo peroneal estava enlargado. A baciloscopia foi negativa. 5 6. TUBERCULOID LEPROSY - PB A large single lesion covering most of the back and crossing the midline. Very well-defined, deeply hypopigmented, flat with a trace of elevation at the lower edge. Sensation within the lesion was significantly diminished. Two peripheral nerves were enlarged. BI = 0. 6. LEPRA TUBERCULÓIDE - PB Uma grande lesão única cobrindo uma grande parte das costas cruzando a linha mediana. A lesão é bem delimitada e hipopigmentada, plana e um pouco elevada na borda inferior. A sensibilidade estava claramente diminuida. Dois nervos periféricos estavam enlargados. A baciloscopia foi negativa. 6 7. BORDERLINE-TUBERCULOID LEPROSY - PB A single, big, spreading well-defined, deeply hypopigmented lesion with slightly raised edges. Dry surface, almost complete loss of sensation. Two peripheral nerves were enlarged. BI = 0. 7. LEPRA BORDERLINE-TUBERCULÓIDE - PB Uma única grande lesão, extensa, bem delimitada e hipopigmentada com bordas levemente elevadas. A superfície é seca, a sensibilidade falta quase completamente. Dois nervos periféricos estavam enlargados. A baciloscopia foi negativa. 7 8. BORDERLINE-TUBERCULOIDLEPROSY - MB A small infiltrated reddish and shiny lesion with well-defined outer edges. Irregular outline with several short extensions implies that the patient's defences are not fully controlling the disease. There is no difference in the degree of infiltration between the peripheral and central parts of the lesion. This patient had 6 lesions. The remaining lesions were located at the extremities. Four peripheral nerves were enlarged. BI = 1. 8. LEPRA BORDERLINE-TUBERCULÓIDE - MB Uma pequena lesão infiltrada, avermelhada e brilhante com bordas bem delimitadas. O contorno é irregular e com pequenas extensões (isto mostra que o sistema imunológico do doente não chega a controlar a infecção). A infiltração não difere no centro e na periferia da lesão. Este doente tinha seis lesões, as outras lesões estavam localizadas nas extremidades. Quatro nervos periféricos estavam enlargados. A baciloscopia foi ligeiramente positiva. (I.B. = 1) 8 9. BORDERLINE-TUBERCULOIDLEPROSY - MB The same patient as in fig. 8. Large hypopigmented lesion located on the calf. The surface of this deeply anaesthetic lesion is dry and hairless. The edge, well-defined for the most part and delicately scaly, in some places fades less perceptibly into the surrounding skin. No leprosy bacilli were found in this lesion. BI = 0. 9. LEPRA BORDERLINE-TUBERCULÓIDE - MB O mesmo doente da foto 8. Uma grande lesão hipopigmentada localizada na panturrilha ("barriga da perna"). A lesão tem falta de sensibilidade, é seca e sem pelos. A borda é bem delimitada, em parte desbota-se na pele vizinha. Não foram encontradas bacilos nesta lesão. 9 10. BORDERLINE-TUBERCULOIDLEPROSY - MB Two lesions are located on the forearm. They are raised and only slightly hypopigmented. The lower lesion has a clearly defined, broad, infiltrated edge and a flat central area. Above there are small satellite lesions separated from the bigger lesion by an area of normal skin. Depressed centre suggests spontaneous healing while satellites indicate local spread of the disease. Sensation was only slightly diminished and no leprosy bacilli were found. This patient had 6 other more irregular but evenly infiltrated and well-defined lesions and involvement of 3 peripheral nerves. 10. LEPRA BORDERLINE-TUBERCULÓIDE - MB Duas lesões no antebraço são elevadas e só ligeiramente hipopigmentadas. A lesão mais abaixo (lado direito na foto) tem a borda claramente delimitada, ampla e infiltrada, a área central é plana. Na lesão acima (lado esquerdo na foto) há pequenas lesões satélites separadas por pele normal. A depressão central (lesão direita) sugere a cura espontânea, enquanto as lesões satélites indicam que aqui a doença espalha-se. A sensibilidade estava só levemente diminuida, não foram encontrados bacilos. Este doente tinha mais seis manchas irregulares, infiltradas e bem delimitadas. Três nervos periféricos estavam envolvidos. 10 11. BORDERLINE-TUBERCULOIDLEPROSY - MB An irregular and spreading lesion, only slightly raised, more erythematous than hypopigmented, shiny. There is no central healing and an irregular border fades imperceptibly into normal skin. Significant loss of sensation. Five more similar-looking lesions were found in this patient. Located mainly on the extremities they demonstrated a tendency to symmetry. Both ulnar nerves and left median nerve were slightly enlarged. BI = 0. 11. LEPRA BORDERLINE-TUBERCULÓIDE - MB Uma lesão irregular e se espalhando, levemente elevada, brilhante e mais eritematosa do que hipopigmentada. Não sara no centro, uma parte da borda desbota-se na pele normal. Clara perda de sensibilidade. Cinco lesões foram encontradas de aspecto similar neste doente. As lesões, na maioria, foram localizadas nas extremidades mostando uma tendência à simetria. Ambos nervos ulnares (cubitais) e o nervo mediano esquerdo estavam levemente enlargados. A baciloscopia foi negativa. 11 12. BORDERLINE-TUBERCULOIDLEPROSY - MB Large, hypopigmented flat lesions. The edges are well-defined but irregular with several satellites dropping off from the bigger lesions. The surface of the lesions is dry. There is diminution of sensation within the lesions. Left ulnar and both posterior tibial nerves are enlarged. BI = 1. 12. LEPRA BORDERLINE TUBERCULÓIDE - MB Lesões grandes, planas e hipopigmentadas. As bordas são bem delimitadas mas irregulares com alguns satélites separando-se das lesões grandes. A superfície das lesões é seca. Há diminuição da sensibilidade nas lesões. O nervo ulnar (cubital) esquerdo e ambos nervos tibiais posteriores estão enlargados. A baciloscopia foi ligeiramente positiva. (I.B. = 1) 12 13. BORDERLINE-TUBERCULOIDLEPROSY - MB Numerous deeply hypopigmented macular lesions of different size and shape, asymmetrical in distribution. All lesions are well-defined with the bigger ones showing irregular outlines and satellites. Loss of sensation. Several peripheral nerves were slightly enlarged. BI = 1. 13. LEPRA BORDERLINE-TUBERCULÓIDE - MB Numerosas lesões maculares, bem hipopigmentadas e de diferentes tamanhos e formas, assimétrico na distribuição. Todas as lesões são bem delimitadas, as maiores delas apresentam contornos iregulares e satélites. Perda da sensibilidade. Muitos nervos periféricos estavam levemente enlargados. A baciloscopia foi ligeiramente positiva (I.B. = 1). 13 14. BORDERLINELEPROSY - MB The lesions with well-defined irregular edges and satellites resemble BT leprosy. But some of them have become domeshaped (left arm) or show immune areas. In the immune area the inner edge of the infiltrated part of the lesion is more clearly defined than the outer edge in contrast to healing centres in tuberculoid leprosy where the inner and outer edges are both sharply defined. Numerous small papular lesions are also characteristic of borderline leprosy. Large lesion on the lower back crosses the midline. There was moderate loss of sensation in all lesions. Several peripheral nerves were enlarged. BI = 2. 14. LEPRA BORDERLINE - MB Estas lesões bem delimitadas com bordas irregulares e com satélites parecem com Lepra borderline-tuberculóide. Porém, algumas das lesões apresentam a forma em cúpula ("domeshaped", braço esquerdo) ou monstram "áreas imunes". Uma lesão com área imune é uma lesão anular com centro que aparece não ser atingido e com a borda interior da parte infiltrada mais claramete delimitada do que a borda exterior. Ao contrário da mancha tuberculóide, onde ambas as bordas, interior e exterior, são igualmente bem delimitadas. Numerosas pequenas lesões papulosas ("borbulhas") são também caracerísticas da Lepra borderline. A lesão grande na parte inferior das costas cruza a linha média. Havia moderada perda da sensibilidade em todas as lesões. Muitos nervos periféricos estavam enlargados. A baciloscopia foi ligeiramente positiva. (I.B. = 2) 14 15. BORDERLINE LEPROSY - MB Several skin lesions. They vary in size and shape. Some are well-defined, others poorly defined and have streaming extensions. Satellites drop off the edges. Centrally some lesions have dimplings. 15. LEPRA BORDERLINE - MB Muitas lesões da pele as quais variam de tamanho e forma. Algumas delas são bem delimitadas e outras não. Algumas monstram extensões e satélites, algumas tem covas ("achatamentos") no centro. 15 16. BORDERLINE LEPROSY - MB Elevated lesions with fairly well-defined edges and central immune areas. Delicate scaling indicates a recent, subsiding reversal reaction. 16. LEPRA BORDERLINE - MB Lesões elevadas com bastante delimitação e área imunes no centro. LEPRA BORDERLINE - MB Aspecto clínico similar a foto 16. 18. BORDERLINE-LEPROMATOUS LEPROSY - MB Numerous lesions with a tendency towards a symmetrical distribution. The lesions differ from each other in size, shape, degree of infiltration and definition. Some are flat and macular, merging imperceptibly with the surrounding skin, others are unevenly raised with papules and small nodules standing out clearly from the skin. Areas of apparently normal skin are found between the lesions. There is only slight loss of sensation within the lesions. Smears from the lesions are strongly positive. 17 18. LEPRA BORDERLINE-LEPROMATOSA - MB Numerosas lesões com uma tendência a distribuir-se simetricamente. Todas as lesões diferem em tamanho, forma, grau de infiltração e delimitatação. Algumas são planas e maculares, emergindo-se na pele normal. Outras são desigualmente elevadas com pápulas e pequenos nódulos elevando-se acima do nível da pele. Áreas aparentemente normais entre as lesões. Só há leve perda da sensibilidade nas lesões. Baciloscopias das lesões foram muito positivas. 18 19. BORDERLINE-LEPROMATOUS LEPROSY - MB A more advanced case of BL leprosy showing numerous lesions tending to be symmetrically distributed. Large number of papules and nodules as well as macular, poorly delineated lesions with areas of apparently unaffected skin between them. 19. LEPRA BORDERLINE-LEPROMATOSA - MB Caso avançado de Lepra borderline-lepromatosa mostra numerosas lesões, com a tendência à distribuição simétrica. Há grandes números de pápulas e nódulos ("borbulhas"), como também lesões maculares ("manchas"), há lesões mal delimitadas com áreas aparentemente não afectadas entre si. 19 20. BORDERLINE-LEPROMATOUS LEPROSY - MB An early case of borderline-lepromatous leprosy showing slightly infiltrated, fairly well-defined lesions. 20. There were also numerous nodules and poorly-defined macules on the trunk and extremities. BI from the facial nodule = 5. 21. The extensive lighter macular eruption on face and chest is tinea versicolor. 30. HISTOID LEPROSY (DAPSONE-RESISTANT LEPROSY) - MB Small, pearly-looking, firm, isolated nodules with central umbilication located in an area usually spared in leprosy. These lesions developed in an 40-year old LL patient irregularly treated with Dapsone for approximately 12 years. BI from these nodules 5. BI from face and ear lobes = 1, 2. 31. LEPRA HISTÓIDE (resistente à Dapsona) - MB Pequenos nódulos isolados, de modo perolado, sólidos e alguns com centro de forma umbilical, localizadas nas áreas onde normalmente não há erupções de Lepra. Acute inflammation is confined to the site of the lesions increased infiltration, erythema, scaling. Scaling indicates that the reaction subsides. 36. REAÇÃO REVERSA (tipo 1) - MB A reacção tipo 1 na Lepra borderline-tuberculóide (MB). A inflamação aguda é limitada na extensão da lesão mostrando infiltração, eritema e escamação. A escamação indica que a reação está a sarar. 36 37. REVERSAL REACTION - MB Acutely inflamed, erythematous, swollen, borderline lesion. 37. REVERSAL REACTION - MB A large facial lesion in a patient with BT leprosy is severely inflamed, swollen, desquamating. Gross swelling may lead to ulceration. 39. The flat, scaly lesion on the nape of the neck and on the upper back is of fungal origin. 40. A lesão plana e escamosa na nuca e na parte superior das costas é uma micose. 40 41. BIRTH MARK (HYPOPIGMENTED NAEVUS) Irregular, flat, deeply hypopigmented lesion. May resemble indeterminate leprosy but the history indicates that the lesion has been present for many years. There are no signs of leprosy. 41. Diagnóstico diferencial: NEVO ACRÔMICO (Sinal de Nascência) Mancha irregular, plana e bem hipopigmentada. Parece com uma lesão de Lepra indeterminada, mas o paciente relata que a mancha está presente há muitos anos ou desde do nascimento. Não há sinais da Lepra (sensibilidade, nervos …). 41 42. LEUKODERMA Numerous hypopigmented macular lesions located not only on the face, but also on the trunk and extremities. There is no loss of sensation within the lesions, no enlargement of peripheral nerves and no M. leprae bacilli in skin smears. In dark skin virtually any skin disease may result in hypopigmentation. 42. Diagnóstico diferencial: LEUCODERMIA Numerosos manchas hipopigmentadas localizadas na face, no tronco e nas extremidades, não há perda de sensibilidade nas manchas, não há enlargamento de nervos periféricos. A baciloscopia sempre é negativa. Na pele escura, quase cada doença de pele pode resultar de uma hipopigmentação. 42 43. TINEAFACIEI (RINGWORM of the FACE) Well-defined, round and slightly scaly lesion, Tinea may be confused with tuberculoid leprosy, particularly when lesions are located on the face and the results of examination of sensation are difficult to interpret. There is itching and scaling in tinea. Scaling is most pronounced in the peripheral parts of the lesions. The fungal elements can be found in scrapings from the scales. A escama é mais produzida nas partes periféricas das lesões. Os fungos (hifas) podem ser encontrados nas escamas (exame micológico). Na dúvida, tratar com pomada antimicótica. 43 44. TINEAFACIEI (RINGWORM of the FACE) A single well-defined lesion with vesicles, crusts and scales on the edge, while the centre tends to heal. Severe itch. No signs of leprosy. 44. Note sharp demarcation of the lesion and scaling most pronounced on its edge. 45. Diagnóstico diferencial: TINHA do corpo (micose) Grande lesão irregular, bem delimitada e escamosa nas nádegas. Observe a clara demarcação da lesão e a escamação principalmente localizada nas bordas. 45 46. TINEAVERSICOLOR A very common skin infection in the tropics. Large sharply defined hypopigmented patches with fine, powderish scales on the surface, distributed over the trunk, neck and extremities, seldom cause diagnostic problems. Tinea versicolor is commonly met in patients with leprosy. 46. Diagnóstico diferencial: Pitiríase versícolor Infecção cutânea a muito comum nos países tropicais. Grande mancha claramente delimitada e hipopigmentada com fina escamação escamação porosa na superfície, distribuida no tronco, no pescoço e nas extremidades; raramente causa problemas no diagnóstico. Pitiríase versícolor é encontrada também em pacientes com Lepra. 46 47. FIXED DRUG ERUPTION Multiple, flat, darker (hyperpigmented) lesions. Fixed drug eruptions occur frequently in people with dark skin and are mainly due to drug intolerance. Numerous medications may cause this eruption, including dapsone. The lesions reappear at the same sites (are fixed) after subsequent exposures to the offending agents. Hyperpigmentation is not a feature of leprosy. 47. Diagnóstico diferencial: ALERGIA MEDICAMENTOSA Manchas planas escuras (hiperpigmentadas). Ocorre frequentemente devido a uma intolerância à certas drogas na África. Um grande número de medicamentos pode causar esta reacção, até Dapsona. As lesões reaparecem nos mesmos lugares do corpo após ingerir o medicamento responsável mais uma vez. Hiperpigmentação não é uma característica da Lepra. 47 48. SEBORRHOEIC DERMATITIS Hypopigmented, scaly, itchy lesions. Note involvement of the hairy scalp and areas behind the ears. The clinical characteristics of the lesions, their location and the absence of cardinal signs of leprosy, serve to distinguish the condition from leprosy. 48. Diagnóstico diferencial: ECZEMA SEBORRÉICO Lesões hipopigmentadas, escamosas e com comichão (coceira). O couro cabeludo e as áreas atrás das orelhas são envolvidas também. As características das lesões, a localização e a ausência de sinais cardinais da Lepra permitem este diagnóstico diferencial. 48 49. PITYRIASIS ROSEA Pityriasis rosea is a relatively common condition in Africa. Numerous oval, small, only slightly elevated lesions, following the lines of cleavage of the skin. There is fine scaling on the surface. The lesions itch, sometimes severely. Pityriasis rosea disappears spontaneously, usually in a few weeks. Presence of characteristically distributed, slightly scaly lesions, preserved sensation, absence of leprosy bacilli and self-limiting character, differentiate pityriasis rosea from leprosy. 49. Diagnóstico diferencial: PITIRÍASE RÓSEA DE GIBERT A Pitiríase rósea é uma doença comum na África. Numerosas lesões pequenas e ovais escamam pouco e se orientam pelas pregas da pele. As lesões podem coçar, ás vezes fortemente. A Pitiríase rósea desaparece espontaneamente, normalmente em poucas semanas. A presença de lesões levemente escamosas, a forma e a distribuição delas, a sensibilidade preservada e a ausência de bacilos da Hansen e finalmente a cura espontânea em semanas diferecia a Pitiríase rósea da Lepra. 49 50. LICHEN PLANUS Lichen planus is a relatively common skin disease in Africa. Usually multiple, flat, greyish or bluish-brown papules, tending to coalesce. Severe itching. No signs of leprosy. 50. Diagnóstico diferencial: LÍQUEN PLANO O Líquen plano é uma doença relativamente comum na África. Muitas pápulas planas cinzas ou marron-azuladas monstram a tendência a se agrupar e confluir. Comichão (coçeira) forte. Não há sinais da Lepra. 50 51. KAPOSI'S SARCOMA This neoplastic condition is not infrequently met, especially in Central Africa. Numerous hard, bluish, vascular nodules which bleed easily. Feet and hands are the most common site of involvement. Here numerous nodules are located on the forearm. There is no nerve enlargement, no loss of sensation, no leprosy bacilli in smears. 51. Diagnóstico diferencial: SARCOMA DE KAPOSI É frequentemente encontada esta forma de neoplasia, especialmente na África Central. Numerosos nódulos duros, azulados que sangram facilmente. Pés e mãos são muitas das vezes envolvidos. Na foto vê-se muitos nódulos no antebraço. Não há nervos enlargados, nem perda da sensibilidade, nem bacilos de Hansen. 51 52. NEUROFIBROMATOSIS Multiple nodular and pendulant tumours. The tumours vary in size and are usually soft. Only small and non-pendulous tumours may be mistaken for nodules of lepromatous leprosy. The negative smear from the nodule will rule out leprosy. No other signs of leprosy. 52. Diagnóstico diferencial: NEUROFIBROMATOSE (Mal de Recklinghausen) Numerosos tumores nodulares até pendurares, variam de tamanho e são normalmente moles. Somente os pequenos tumores podem ser confundidos com a Lepra lepromatosa. A baciloscopia sempre é negativa, não há sinais de Lepra. 52
