Versucht, eine vorwiegend pathogenetisch orientierte Sichtweise durch eine auf Autoregulation und Selbstheilung zielende Sichtweise zu ergänzen. • CAM ist eine Gruppe von diversen medizinischen Systemen, Praktiken und Produkten, die aktuell nicht als Teil der herkömmlichen wissenschaftlich fundierten Schulmedizin gesehen werden. Akupunktur – Ayurveda • Geist-Körper Interventionen – – – Meditation Hypnose Mal-, Musik- und Tanztherapie • Biologisch begründet – – – Kräutermedizin Diät / Zusätze Vitamine • Manipulativ / körperlich – – – – Chiropraktik Osteopathie Reflexzonentherapie Massage • Energetische Therapien – Reiki – Biomagnetik Feld Therapie Phytotherapie Probleme • Meist Extrakte, keine Reinstoffe • Patentschutz kaum möglich, wenig Interesse der Pharmaindustrie: wenige/keine richtigen Studien • Meist kleine bis mittelständische Firmen – beschränkte Ressourcen für Forschung – starke Abhängigkeit vom Produkt, damit von Studienergebnissen • Natürlich heisst nicht automatisch sicher und ohne Nebenwirkungen!!! (Internet…Steroide…etc.) Curcumin = Gelbwurz • Kurkuma: heiliges Gewürz, bewährte Heilpflanze in Indien • Kurkuma Hauptbestanteil im Currypulver • Die Gattung Curcuma umfasst ca. 50 Arten (Curcuma domestica, Curcuma longa) • schilfähnliche, mehrjährige Pflanze mit knolligen Rhizom mit fingerartigen Verzweigungen • 89 Patienten mit Colitis ulcerosa in Remission erhielten entweder Curcumin 2x1 g plus Sulfasalazine (SZ) oder Mesalamine oder Placebo plus SZ oder Mesalamine für 6 Monate • Klinische Aktivität (CAI) und endoskopische Aktivität (EI) im Verlauf gemessen • 2/43 (4.65 %) der Patienten mit Curcumin hatten einen Relaps gegenüber 8/39 (20.51 %) in der Placebo-Gruppe (P =. 040) • Curcumin verbesserte beide CAI (P =. 038) und EI (P =. 0001) • Schlussfolgerung: Curcumin scheint eine viel versprechende und sichere Therapie zur Remissionserhaltung bei UC zu sein. Weitere Studien sind nötig Hanai et al 2006 Clin Gastroenterol Hepatol Dec; 4(12): 15021502-6. • Die Echte Aloe (Aloe vera), auch Curacao-Aloe oder Wüstenlilie genannt ist eine Pflanzenart aus der Gattung der Aloen. Kultiviert wird sie in allen subtropischen und tropischen Regionen der Welt aber auch im Mittelmeergebiet Randomized, double-blind, placebo-controlled trial of oral Aloe vera gel for active ulcerative colitis • 44 Patienten mit milder bis moderater Colitis ulcerosa. 2x100ml Aloe vera Gel oder Placebo für 4 Wochen • Aloe vera: Klinische Remission 9/30 (30%), Verbesserung 11/30 (37%), Therapieantwort 14/30 (47%) • Placebo: Klinische Remission 1/14 (7%), Verbesserung 1/14 (7%), Therapieantwort 2/14 (14%) • Konklusion: Aloe vera führte häufiger zu einer klinischen Therapieantwort als Placebo und verbesserte die histologischen Veränderungen. Crohn) und haben generell wenig Nebenwirkungen • Wichtig: Genügend hoch dosieren, Dauertherapie!! • Nicht vergessen: Rectale Präparate!
