Reagent 2 (R2) is prepared by pouring the entire contents of the R2B vial into the R2A vial. Mix gently. REAGENT STABILITY AND STORAGE Conditions: Protect from light. Close immediately after use. Storage: at 2 – 8 °C Stability: up to the expiration date Stability of R2A + R2B mixture: 30 days at 2 – 8 °C No interference up to: Ascorbic Acid 50 mg/dL Bilirubin 50 mg/dL Triclycerides 2000 mg/dL Carbamylated Hb 7.5 mmol/L Acetylated Hb 5.0 mmol/L It has been reported that results may be inconsistent in patients who have the following conditions: opiate addiction, lead-poisoning, alcoholism, ingest large doses of aspirin.6,7,8,9 It has been reported that elevated levels of HbF may lead to underestimation of HbA1c and that uremia does not interfere with HbA1c determination by immunoassay.10 It has been reported that Hemoglobin variants HbS and HbA2 are not detected by immunoassay, leading to possible inaccurate determination. Also, it has been reported that labile intermediates (Schiff base) are not detected and do not interfere with HbA1c determination by immunoassay.5 QUALITY CONTROL All control sera with % HbA1c values determined by this method can be used. We recommend the DIALAB HbA1c Direct Control Set. CALIBRATION SAMPLE STABILITY AND STORAGE Whole blood, collected with EDTA Stability: at 2 – 8 °C Hemolysate stability: at 2 – 8 °C This method utilizes the interaction of antigen and antibody to directly determine the HbA1c in whole blood. Total hemoglobin and HbA1c have the same unspecific absorption rate to latex particles. When mouse antihuman HbA1c monoclonal antibody is added (R2), latex-HbA1c-mouse anti human HbA1c antibody complex is formed. Agglutination is formed when goat anti-mouse IgG polyclonal antibody interacts with the monoclonal antibody. The amount of agglutination is proportional to the amount of HbA1c absorbed onto the surface of latex particles. The amount of agglutination is measured as absorbance. The assay requires the use of a HbA1c calibrator. We recommend the Dialab HbA1c Direct Calibrator Set. 7 days 10 days AUTOMATION Discard contaminated specimens. Special adaptations for automated analyzers can be made on request. SAMPLE PREPARATION WARNINGS AND PRECATIONS To determine HbA1c, a hemolysate must be prepared for each sample: 1. Dispense 1 ml Hemolysis Reagent into a tube. 2. Add 20 μL of well mixed whole blood and mix. 3. Allow to stand for 5 minutes or until complete lysis is evident. Es wurde auch gemeldet, dass labile Zwischenprodukte (Schiff’sche Basen) nicht erfasst werden und die HbA1c-Bestimmung durch Immunoassays nicht beeinträchtigeny.5 Alle Kontrollseren, bei denen % HbA1c-Werte mit dieser Methode gemessen wurde, können verwendet werden. A hemoglobina total e a HbA1c possuem a mesma taxa de absorção inespecífica para partículas de látex. Quando o anticorpo monoclonal de rato anti HbA1c humana é adicionado (R2), o complexo de látex-HbA1c de rato anti HbA1c humana é formado. A aglutinação é formada quando o anticorpo policlonal de cabra anti IgG de rato interage com o anticorpo monoclonal. A quantidade de aglutinação é proporcional à quantidade de HbA1c absorvida na superfície das partículas de látex.
