Janeiro 2008 TiUnite Estabilidade dos implantes TiUnite® a curto e longo prazo Resumo TiUnite é o biomaterial patenteado da Nobel Biocare que comprovadamente aprimora a osseointegração e aumenta a previsibilidade do tratamento de implante, tanto a curto quanto a longo prazo. Histórico TiUnite é uma superfície porosa de óxido de titânio produzida através de um processo de oxidação anódica. Utilizando um processo eletroquímico exclusivo, a Nobel Biocare induz o crescimento da camada superficial natural de óxido do implante. As condições específicas do processo produzem um biomaterial com uma estrutura de superfície porosa única. O óxido de titânio da superfície TiUnite, altamente cristalino e enriquecido com fosfato, apresenta uma topografia de microestrutura sem características pontiagudas, e é caracterizado pela presença de poros abertos uniformemente distribuídos na extensão micrométrica inferior. A Nobel Biocare introduziu a superfície TiUnite em seus implantes Brånemark System® no ano de 2000, e nos implantes NobelReplace™ Durante seus primeiros cinco anos no mercado, foram publicados mais de 84 artigos científicos documentando investigações in-vitro, bem como estudos pré-clínicos e clínicos. Isto confirma o sucesso clínico dos implantes TiUnite para todas as qualidades ósseas em procedimentos de dois estágios, bem como nos procedimentos de carga imediata. Achados pré-clínicos e clínicos Estudos pré-clínicos anteriores mostram que a velocidade da osseointegração é aumentada com os implantes de superfície TiUnite, quando comparados com implantes de superfície maquinada. Este efeito foi medido como um aumento ósseo em contato com o implante em determinado ponto no tempo. Na verdade, as propriedades da superfície TiUnite estimulam o crescimento ósseo diretamente na sua superfície. Comparado com superfícies maquinadas, os achados clínicos demonstram que a queda na estabilidade inicial durante a fase de cicatrização foi significantemente reduzida com implantes de superfície TiUnite.1 O benefício dessa observação é que o período de risco durante os seis primeiros meses foi significantemente reduzido. A formação óssea segue os contornos das roscas dos implantes mesmo durante a fase inicial de cicatrização.2 Isto proporciona maior manutenção da estabilidade inicial do que nos implantes com superfície maquinada. Das 84 publicações, cinco estudos publicados recentemente demonstram a estabilidade do TiUnite a longo prazo.3,4,5,6,7 Nestes, mais de 550 implantes TiUnite de diferentes modelos, demonstraram uma taxa de sucesso de 94 - 100%. Os períodos de acompanhamento tiveram pelo menos dois anos de duração e foram colocados implantes tanto em função imediata quanto em protocolos de dois estágios. O estudo de Glauser et al., finalizado em 2007, é um acompanhamento de cinco anos de implantes TiUnite com carga imediata em regiões de osso poroso, confirmando a estabilidade a longo prazo dos implantes TiUnite. Nesta publicação, que demonstrou uma taxa de sucesso dos implantes de 97,1%, foi demonstrado que implantes TiUnite são clinicamente estáveis por um período maior que cinco anos. Em outras palavras, não há queda na estabilidade clínica conforme mostraram as medições de análise de freqüência de ressonância (RFA). Adicionalmente, foi feito o acompanhamento radiográfico do nível ósseo marginal por cinco anos. A inclinação da curva no intervalo de 12 a 60 meses é de menos de 0,1mm por ano. Isto mostra que os níveis ósseos ao redor dos implantes TiUnite permanecem estáveis. No acompanhamento de um ano, implantes com níveis ósseos de mais de 2mm do ponto de referência foram analisados separadamente. Apesar da exposição de mais de 2mm das roscas após um ano, esses implantes mostraram boa estabilidade e manutenção do osso marginal durante os quatro anos subsequentes. Interação com o tecido mole A superfície TiUnite interage com o tecido mole de várias maneiras: 1) A proteção proporcionada pelo selamento do tecido mole periimplantar é melhorada por um entrelaçamento biomecânico do epitélio juncional através da inserção de células epiteliais nos poros abertos do TiUnite.8 2) A estabilidade da mucosa periimplantar é fortalecida por uma rede tri-dimensional de fibras colágenas no tecido conjuntivo com o TiUnite, proporcionando resistência mecânica e permitindo suportar as forças mastigatórias.9 3) Com relação à defesa da mucosa periimplantar, os mecanismos do TiUnite correspondem exatamente aos da gengiva que envolve o dente natural.10 TiUnite até o topo nos implantes NobelDirect®, Speedy e implantes tipo Groovy Introduzido em 2004, o implante de uma peça, NobelDirect, foi o primeiro a ser beneficiado com a extensão da superfície TiUnite em contato com o tecido mole. Análises histológicas feitas em estudos pré-clínicos revelaram uma afinidade à formação óssea junto a parte interna do sulco incorporado ao parafuso do implante ao longo da extensão da porção intra-óssea. As linhas de implantes Speedy e Groovy foram lançadas em 2005. Um sulco retentivo de 100 mícrons foi introduzido nos implantes para aumentar a rapidez da osseointegração. A superfície TiUnite foi estendida até o colar do implante. Em cinco estudos em andamento patrocinados pela Nobel Biocare, implantes com superfície TiUnite até o topo, onde o colar com superfície TiUnite está em contato com o tecido mole, estão sendo revisados. A taxa de sucesso atual nestes estudos chega a 96.8 - 100%. Foram relatados níveis ósseos estáveis em um acompanhamento de um ano de 137 pacientes que receberam 256 implantes. Ininterruptamente, os níveis ósseos estáveis foram confirmados em três anos em um acompanhamento de 99 implantes com 53 pacientes. Um acompanhamento de 2 anos de um destes estudos sobre o NobelDirect foi publicado recentemente e a taxa de sucesso do implante relatada foi de 98.8%.6 Os níveis ósseos estavam estáveis entre um e dois anos e os autores concluíram que: “Dentro dos limites deste estudo, o nível ósseo marginal estável e a saúde do tecido mole sustentam a hipótese de que o implante de uma peça avaliado, tem a capacidade de preservar tanto o tecido mole como o tecido duro.” Conclusões TiUnite é um biomaterial osseocondutor de óxido de titânio produzido através de um processo de oxidação anódica. A superfície TiUnite advém dos implantes maquinados tradicionais, porém oferece resultados aperfeiçoados. Desde a sua introdução em 2000, a superfície TiUnite tem sido utilizada clinicamente e tem sido demonstrado que proporciona melhor estabilidade durante o período de cicatrização. Essa evidência proporcionou bases para o tratamento com carga imediata. Recentemente, foi publicada uma documentação clínica de cinco anos, demonstrando a estabilidade clínica a longo prazo e níveis ósseos marginais estáveis. A superfície TiUnite foi estendida até a parte coronal dos implantes Groovy, Speedy e NobelDirect.
