Correspondência | Correspondence [email protected] O Professor Lobo Antunes, neurocirurgião, no seu livro Sobre a Mão e Outros Ensaios, referindo-se aqueles momentos que se seguem a uma cirurgia elaborada, tem esta frase:”Aquela beatitude ou exaustão doce, um pouco triste, que inevitavelmente sucede ao acto 1 cirúrgico bem feito” . Estas palavras são tão certeiras e ao mesmo tempo tão carregadas de uma ternura que os amantes/executantes deste mesmo magister, um acto cirúrgico – seja qual for a Especialidade – não deixam imediatamente de se rever nelas, dando uma total concordância para a formulação deste sentimento que afinal, surpresa nossa, encontramos como característica, como elemento comum a esta prática que é nossa profissão. Este exemplo no desempenho da prática medica onde quer que estejamos, seja qual for a nossa Especialidade, é paradigma da espantoso coerência de atitudes e sintonias que, mesmo num mundo em acelerada mudança para alguma forma de civilização que ainda não antevemos, a Medicina mantém e que, em última análise, já identificáramos naquele texto, várias vezes milenar, que é o Juramento de Hipócrates. Há um fio, por vezes não explícito, mas sempre pressentido pelos praticantes desta, em simultâneo, ciência e arte. A esta marca de intemporalidade na nossa prática pode associar-se uma outra, a da globalidade. A anatomo- In his book, “On the Hand and Other Essays”, the Neurosurgeon, Professor Lobo Antunes refers to those moments that follow a complex surgery with these words: “That sense of peace or the sweet but slightly sad feeling of exhaustion that inevitably follows a suc1 cessful surgery” . These words are so accurate and at the same time so laden with tenderness that the lovers/ performers of this magister, the surgery - whatever the Specialty – cannot help but immediately identify with them. Though it may come as a surprise, we are bound to agree with this sentiment which is such a characteristic and common ingredient of our profession. No matter where we practice Medicine or what our specialty may be, this example illustrates the amazing consistency and harmony of attitudes that there are in the world of medicine. In this rapidly changing world, on its way to a still unknown form of civilization, this can still be seen in the age old Hippocratic oath. Though not explicit, the practitioners of medicine are always aware of the bond between science and art. The timeless nature of the practice of medicine can also be linked to its global nature. The anatophysiological makeup of human beings is the same, no matter where they may reside on the planet. We know that science is local and global. We know that no matter where we are, the careful application of a set of rules that Barlavento Médico Para uma Medicina Humana e Culta Barlavento Médico 6 fisiologia do Homem, onde quer que estejamos no planeta, é a mesma. Sabemos que a ciência é local e global. Sabemos que, onde quer que estejamos, a aplicação criteriosa de um conjunto de regras que regem a ciência, deve levar a resultados que serão reprodutíveis, já que se reportam à “natureza das coisas” que é, no fundo, aquele tudo que sistematicamente procuramos apreender. Os resultados que obtemos entre nós, se de acordo com as metodologias certas, devem ser os que se obtém na América ou na China. Outro destes elementos caracterizadores da Medicina é o da mudança. Quantos de nós se licenciaram com o ensinamento de que no estômago não existiam bactérias? Só assim não é, como essa nova descoberta já premiou um Nobel. Este é meramente um exemplo de alterações, de uma revolução silenciosa de paradigmas, a que assistimos. Não se trata só da crescente evolução nos meios que dispomos para diagnóstico e terapêutica, mas também do ensino e da maneira de praticar a medicina. Assistimos à tentação de substituir a compreensão da linguagem habitual à medicina milenar que passa pela relação médico-doente, pela história clínica, pela linguagem dos sinais e dos sintomas, por uma ultrapassagem - marca do tempo que atravessamos - expressa na mudança para uma outra linguagem feita de um mero somatório de exames complementares. Ser médico aqui ou em qualquer parte do mundo, seja num Hospital central ou periférico, tem muito em comum para além deste elemento. Estamos hoje num tempo diferente daquele onde os nossos mestres se formaram. Já lá vai o tempo onde as competências ficavam só nos grandes centros. Hoje a procura da qualidade de vida – seja o que for na nossa profissão sem horários para mais nada, mas aqui no Algarve o mar ao lado ninguém nos tira – o equipar de Hospitais periféricos, a justeza do reclamado pelas populações de não ter de caminhar 300 km para um tratamento quotidiano, o regresso após formação competente àquela que é a nossa terra ou algum outro motivo, foram elementos que nos fizeram optar pela Medicina no Algarve. Assim a existência de uma publicação com estas características surge com naturalidade. A palavra “barlavento” traduz um conceito: de onde sopra o vento. Esta publicação BARLAVENTO MÉDICO emerge em Portugal nesta região do Algarve e no Hospital que lhe adoptou o nome, o Hospital do Barlavento. Não temos a presunção de ser a energia motriz para novas caravelas do conhecimento, nesta época marcadamente identificada com a aceleração da globalização, já que a razão maior que leva ao aparecimento desta revista é igualmente intemporal na nossa natureza: dar primazia à formação continua de maneira que esta se reflicta na assistência que damos aos nossos doentes. Nesta revista assumimos esta evidência que é ver o Inglês como o idioma fundamental na Medicina à dimensão mundial e adoptamo-lo também como idioma base desta publicação. Esta dupla utilização do suporte em govern science should lead to results that can be reproduced, since they come from the “nature of things” - which in essence is all that we are systematically seeking to learn. The results that are obtained according to tried and true procedures should be the same as those obtained in the U.S.A. or in China. Another element which characterises medicine is change. How many of us graduated from medical school having learned that there are no bacteria in the stomach? Not only is this not so, but this new discovery produced a Nobel Prize. This is simply an example of change, a silent revolution of the rules that we are witnessing. It isn’t just the result of ongoing improvements in the diagnostic and therapeutic means available to us, but also in the teaching and way of practising medicine. We are confronted with the temptation to substitute the time-tested traditional language used between a doctor and his patient, the clinical history, the language of signs and symptoms, with another type of language (a true sign of the times) made up only of complementary exams. Doctors in this Hospital or any part of the world, whether in a big city or a remote place, have a lot in common in addition to this. Being a doctor with access to information on good practices, no matter where we practice medicine, is also something we share. We, doctors today, are in a different point in time than when our mentors graduated. Gone is the time when advances were made only in important medical centres. Today, the search for quality of life (be that as it may in a profession where free time is scarce, though here in the Algarve no one can take the sea away from us), the equipping of peripheral Hospitals, the right that populations have not to have to travel 300 km for a commonplace treatment, the return to the place we call home after expert training, were the factors that made us opt to practice Medicine in the Algarve. Therefore, the appearance of a publication with these characteristics comes about naturally. The word barlavento (windward) expresses a concept: from where the wind blows. This publication Barlavento Médico appears in Portugal in this region of the Algarve from which the Hospital got its name, Hospital do Barlavento. We do not presume to be the driving force behind new caravels of knowledge, in this era of ever-increasing globalization. Indeed, the main reason behind this magazine is to prioritise continuous training, so as to improve the assistance that we give our patients. In this magazine we acknowledge English as the world-wide lingua franca of Medicine and adopt it as a basic language of this publication. It will be published both as a paper journal, with a limited number of copies available, and online. We want BARLAVENTO MÉDICO to be read here, in the Algarve, but also in New York, Brasilia, Madrid or in Timbuktu. * It reminds me of what Prof. Abel Salazar (1889-1946) once said: “The doctor who only knows of Medicine not even of Medicine knows”. papel, com uma distribuição sempre limitada e de uma disponibilidade online vem da óbvia leitura da nossa contemporaneidade. Queremos igualmente honrar uma tradição – quem não aprecia as espantosas ilustrações no tratado De humani corporis fabrica, de Andreas Vesálio que adoptamos para página de rosto? - que ao longo da história, nos dotou com obras ilustrados de notável sentido estético numa preocupação que percorre esta publicação. E sendo uma revista com peer reviewing, como não associar a credibilidade que esta comporta, com o tornar os trabalhos acessíveis a todos pelo website? Somos pois uma publicação aberta na internet, estando os Editores profundamente empenhados nas questões e metodologias para a indexação da revista. Procuramos também como identidade nesta revista a inclusão da reflexão sobre a adequação do ensino/formação pós graduada que prestamos. Queremos um conteúdo amplo no contexto diversificado das matérias de cada uma das múltiplas Especialidades em Medicina, que faça a revista assentar num certo sincretismo (existirá tal figura? ou perguntando de outra maneira, será a medicina possível de outra forma? *). Da ciência em si nas suas múltiplas vertentes, ângulos e perspectivas, das ciências Básicas, às Especialidades bem como ao tomar em si campo para a reflexão de questões relativas à Pós-graduação ou meramente de uma vivência/reflexão propiciada pela prática médica, visamos – na expressão de Lobo 2 Antunes a propósito de Celestino da Costa – uma medicina Humana e Culta. Queremos usar estas qualidades que nos são comuns, façamos a prática da Medicina aqui no Algarve ou noutra parte do mundo, para dar sentido àquela motivação que nos fez entrar para esta profissão, sentir, perceber e melhorar a natureza Humana. Assim esperando que se sinta estimulado por BARLAVENTO MÉDICO convido-o a tomar parte nesta revista – na diversidade de talentos com que a natureza nos criou – seja como leitor ou como autor e submeta para apreciação manuscritos que se inscrevam nestes objectivos. We want to honour a tradition that, throughout history, has endowed us with illustrated works of an extraordinary aesthetic level. Who doesn’t appreciate the remarkable illustrations in the Andreas Vesálio’s treatise De humani corporis fabrica that we have used for our front page illustration? And since there is peer reviewing in this publication, why not enhance the credibility that this entails, by making the studies accessible to all on our website? We are therefore a publication on the Internet that is open and free. The Editorial Board is fully aware of the hurdles and problems it this faces in establishing and indexing this Journal. We are aware of this and are working towards this end. As part of this periodical’s identity, we also seek to reflect on the suitability of the post-graduate teaching/training that we provide. We would like the content to cover a vast number of fields and subjects of the numerous Specialties of medicine, thereby tying the identity of this magazine to a certain measure of syncretism (Does such a thing exist, or is medicine possible in any other form?). And in approaching science from multiple angles and perspectives, ranging from the Basic sciences to the Specialties, and on reflecting on issues related merely to the experience or reflections that are the result of practising medicine, this 2 publication aims at - in the words of Lobo Antunes - a humane and cultured medicine. We want to use the qualities that we have in common, whether we practise Medicine here, in the Algarve, or in any other part of the world, to give meaning to the reasons that made us enter this profession: to feel, understand and improve Human nature. In conclusion, no matter where we are in the world, I hope that BARLAVENTO MÉDICO stimulates and encourages you to take part in this magazine - either as readers or as authors and that you will submit your manuscripts that fit into its scope. Com vantagem relativamente a outros sistemas, o AEP baseia-se em critérios objectivos, sendo possível estabelecer comparações entre diferentes hospitais ou num mesmo Serviço para determinar o impacto de medidas destinadas a melhorar a adequação do uso das camas hospitalares. Permite ainda identificar as causas de adequação ou inadequação do internamento e estadia hospitalar, pelo que também é útil para estabelecer estratégias de optimização da eficiência da hospitalização. Neste artigo os autores revêem os resultados da aplicação prática do AEP em dois países do sul de Europa durante um período de 10 anos. ABSTRACT: Appropriateness Evaluation Protocol (AEP) is an instrument widely used anywhere in the world to determine the causes and magnitude of the inadequacies of the use of the hospitable beds. Like advantages as opposed to other systems, the AEP is based on objective criteria, reason why it is possible to establish comparisons between different hospitals or in a same Service to determine the impact of the measures destined to improve the adjustment of the use of the hospitable beds. In addition, it allows to identify the causes of adjustment and inadequacies of the income and stays, reason why also it turns out from utility to establish strategies of optimization of the efficiency of the hospitalization. In this article we reviewed the results of the practical application of the AEP in two countries of the south of Europe throughout 10 years. INTRODUÇÃO INTRODUCTION As restrições orçamentais ocorridas nos Estados Unidos como consequência das crises económicas dos anos 70, afectaram duas áreas que, anteriormente, gozaram de recursos praticamente ilimitados: a Defesa 1 e a Saúde . Nestas áreas habitualmente, procuravam-se resultados, independentemente dos custos que a sua execução pudesse implicar. Para a nova mentalidade que apareceu com a diminuição das compensações económicas, importava o termo eficiência que vinha sendo aplicada poucos anos antes com o intuito de se obterem os mesmos resultados com menos recur2 sos . Como consequência na área da saúde, fizeram-se vários estudos e intervenções a nível dos procedi3,4 mentos médicos que modificaram radicalmente a mentalidade da época. No início da década de 80, Avedis e Donabedian criaram o “Appropriateness 5 Evaluation Protocol” (AEP) que foi o instrumento mais utilizado na Europa desta avaliação da 8,9,10 sobreutilizacão das camas hospitalares . APPROPRIATENESS EVALUATION PROTOCOL O AEP é constituído por uma série de critérios objectivos e explícitos que classificam como adequado o internamento e estadia numa unidade hospitalar, quando o doente cumpre pelo menos um deles. Os critérios dividem-se em duas secções. A primeira é constituída por duas partes: estado do doente e serviços clínicos. A segunda secção é composta por três partes: cuidados médicos, de enfermagem e cuidados do doente. Todos eles têm implícito que a história clínica foi devidamente executada tanto pelo médico como pela enfermagem, uma vez que o revisor, deverá fazer sempre a sua apreciação com base em dados escritos e não “lendo nas entrelinhas”. A tendência de “decifrar/interpretar” 5 Appropriateness Evaluation Protocol (AEP) , which is the tool most widely used throughout Europe to assess 8,9,10 overuse of hospital beds . APPROPRIATENESS EVALUATION PROTOCOL The AEP consists of a series of objective and explicit criteria that classify as appropriate an admission or hospital stay if at least one of the criteria is met by the patient. The criteria are divided into two categories. The first consists of two factors: the patient’s condition and medical services. The second category consists of three factors: medical care, nursing care and patient care. It goes without saying that the patient’s medical record should be appropriately written up not only by the medical staff but also by the nursing staff since its reviewer must always make his or her assessment based on written data and not from ‘reading between the lines’. The tendency to ‘interpret’ the contents of a medical record and it’s illegibility are some of the main obstacles in correctly evaluating the appropriateness of 11,12 admission or hospital days of care . Barlavento Médico 10 o conteúdo dos processos clínicos e a sua difícil legibilidade são alguns dos principais obstáculos para uma correcta avaliação da pertinência de um inter11,12 namento ou dos dias de estadia hospitalar . O facto de ser uma indicação médica a justificar o internamento não implica que esta esteja correcta. Isto é, o AEP não determina se as indicações médicas são adequadas, mas apenas se com base na terapêutica prescrita, o doente reúne todas as condições necessárias para a admissão e internamento num Hospital de cuidados agudos. Assim a variedade da prática clínica será um dos “factores de confusão” que causam variação nos 13 níveis de adequação entre diferentes hospitais . Por norma, admite-se que a prática clínica considerada globalmente é uniforme e que este parâmetro não deverá afectar de forma significativa os resultados dos estudos de adequação de admissão e internamento hospitalar. A revisão e categorização dos internamentos em adequados e inadequados deve fazer-se, idealmente, em equipa: dois revisores independentes, nenhum deles com conhecimento prévio do doente, receberão uma formação acerca da utilização do AEP e da aplicação dos critérios em casos clínicos concretos. Posteriormente, avaliarão separadamente as historias clínicas e darão o seu parecer acerca da adequação do internamento ou permanência no hospital. Normalmente em caso de divergências de opinião, um terceiro revisor com mais experiência na área, dará o seu parecer. Na prática, o grau de concordância entre os revisores deve 14 ser igual ou superior a 85% . Um nível inferior indicará a necessidade de realizar uma nova reunião para esclarecer as diferentes aplicações e interpretações que possam ser dadas aos critérios de adequação. Os critérios que avaliam a adequação são diferentes consoante se trate de um internamente ou de um dia de 15 estadia no Hospital . Um maior número de critérios cumpridos não supõe um aumento da adequação, visto que esta é uma variável dicotómica, pressupõe de alguma forma que a probabilidade de ter sido julgado correctamente como adequado o internamento ou a estadia será maior. De igual modo, as causas da inadequação também são diferentes para julgar uma admissão ou um dia de permanência no hospital. Dependendo do tipo de doentes que aborde, o AEP tem diferentes versões: médico-cirúrgica, pediátrica e de cirurgia electiva. Com ele (AEP) pretende-se adaptar, de uma forma eficiente, a realidade assistencial e 16, 17, detectar eficazmente as causas de inadequação 18 . Assim, por exemplo, num protocolo cirúrgico acentua-se a necessidade do internamento do doente antes da cirurgia. Já no protocolo médico é valorizada de forma especial a possibilidade de realização de um estudo/tratamento a outro nível assistencial que não no Hospital de cuidados agudos. Além disso, em diversos países já foram efectuadas adaptações do AEP às características do sistema de to proceed. That is to say, the AEP does not determine whether the diagnosis is appropriate. More exactly, it suggests that, based on the prescribed therapy, the patient fulfils the appropriate requirements for admission or a stay in an acute care hospital. Differences among medical practitioners therefore, are, one of the ‘confusing factors’ that causes a discrepancy in the levels of 13 appropriateness amongst the different hospitals . As a rule, we accept that medical practice, as a whole, is consistent and this parameter should not significantly affect the results of the appropriateness studies for admittance and hospital stays. Ideally, there should be a team of two independent reviewers responsible for revising and categorizing admittances and stays as appropriate and inappropriate. Neither of them should have previous knowledge of the patient and they should be trained to use the AEP and apply the criteria to specific clinical cases. Next, they should evaluate each medical history separately and decide if admittance or hospital stay is appropriate. Normally, in the event of a difference of opinion, a more experienced third reviewer gives his or her position. In practical terms, the agreement rates amongst 14 the reviewers should be equal to or higher than 85% . A lower rate indicates the need for another meeting to clarify the different uses and interpretations that can be given to the criteria of appropriateness. The criterion to assess appropriateness differs for 15 admittances and hospital days of care . Meeting more of the criteria does not imply an increase in appropriateness as this is a dichotomic variable. It does, however, imply a greater probability of having correctly judged, as appropriate, an admittance or hospital days of care. By the same token, the causes of inappropriateness are also different when judging an admission or hospital days of care. There are different versions of the AEP: surgery, paediatrics and elective surgery - which depend on the type of patient. Thus, it can be used more effectively in health care today and successfully identify the causes 16,17,18 of inappropriateness . For example, the surgical protocol makes special note of the need of patient admittance prior to surgery and the ability of the clinician to study/treat the patient at a non-acute facility. Moreover, the AEP has been modified in various countries to reflect differences in the provision of health care and to optimise the effectiveness of this tool. We have the I-AEP, G-AEP, and the F-AEP which refer to the Italian, German and French versions respectively, 19,20,21 among others . Portugal also has its own version, the PRU Protocol of Review and Utilisation), which 22,23,24 has been used for local and nation-wide studies . As regards the temporal sequence of studies, the AEP may be used in concurrent, prospective, and retrospective studies. Most published studies are based on 25,26,27 retrospective reviews . This provides valuable information based on the review of a number of saúde para optimizar o rendimento desta ferramenta. Assim, podemos falar entre outras de I-AEP, G-AEP, ou F-AEP que correspondem às adaptações em Itália, 19, 20, 21 Alemanha e França, respectivamente, . Em Portugal também existe uma versão própria, o PRU (Protocolo de Revisão da Utilização), com o qual se têm realizado estudos tanto a nível local como nacional 22, 23, 24 . Quanto à sequência temporal do estudo, o AEP poderá realizar-se, de forma transversal, retrospectiva e prospectiva. O maior número de estudos publicados 25, 26, 27 baseia-se em revisões retrospectivas . Estes proporcionam uma informação importante baseada na revisão de um número de historias clínicas previamente calculados e que relativamente ao total de doentes estudados, proporcionará dados com significado estatístico. Como inconvenientes assinalemos o facto de haver necessidade de um importante investimento em termos de recursos humanos (dedicação de tempo por parte de um grupo de revisores) e o facto de só se dispor da informação da extensão e das causas da inadequação depois de ser concluído o tempo de estudo. Em caso de detecção de problemas pontuais, por exemplo durante uma determinada estação do ano, prefere-se a variante transversal também conhecida como concorrente. Nesta leva-se a cabo o estudo da adequação do internamento e estadia hospitalar dos doentes internados no hospital um determinado dia. Deste modo, obtemos dados “instantâneos” da adequação de uma forma rápida e eficiente. Como inconveniente, temos o facto de o número de doentes observados poder resultar em dados sem significado estatístico. Não obstante, mostrará tendências estatísticas que em caso de aumento de afluência aos hospitais, como no caso de epidemias, poderá ajudar a encontrar alternativas adequadas ao internamento em hospitais de cuidados agudos. Finalmente, a versão prospectiva, proporciona uma visão objectiva, diária da adequação da utilização das camas hospitalares. De forma ideal, deverá ser realizada individualmente por cada médico por motivos de eficiência (não é possível dedicar um revisor durante todo o ano para avaliar a adequação). Além de proporcionar dados acerca do sobreuso, esta versão terá um valor acrescido que é a consciencialização dos profissionais de saúde dos critérios de adequação e a valorização 28 diária da necessidade do internamento hospitalar . Quando os critérios ordinários não têm a precisão necessária para determinar a adequação devido a variantes da prática clínica, é possível recorrer a uma série de critérios considerados como extraordinários (override), para contextualizar a avaliação da adequação com os meios hospitalares. Estes critérios são utilizados habitualmente para considerar adequado um internamento que não reúne nenhum critério do Protocolo, mas que no entender dos revisores estaria previously calculated medical records and provides significant statistical data concerning the total number of patients studied. Its drawbacks include of the need for a large investment in terms of human resources (dedication of time by a group of reviewers) and the fact that information regarding the degree and causes of inappropriateness is only available after the study is concluded. The transversal version, also known as concurrent version, is usually preferred to detect specific problems, - for instance, problems during certain seasons. In this case, a study of the appropriateness of admission and hospital stays is carried out on admitted patients on a specific day. ‘Instant’ data is obtained in this manner to assess appropriateness in a rapid and efficient manner – the downside being that the number of patients observed may provide data of little statistical significance. Not withstanding, these studies enable the identification of trends, for statistical purposes, and should there be an increase in the number of people needing a hospital, as in the event of an epidemic, they may assist in finding adequate alternatives to admittance to acute care hospitals. Finally, the prospective version provides us with an objective view, on a day to day basis, of the appropriateness of the use of hospital beds. Ideally, it should be carried out by each doctor for efficiency purposes (a reviewer can not be made available all year long solely to assess appropriateness). In addition to providing data regarding over usage, this version has an added advantage which is the health care professional’s need to know the criteria for assessing appropriateness and the daily evaluation of the necessi28 ty for hospital admittances . When ordinary criteria are not precise enough to determine appropriateness due to variations of medical practice, there are a series of extraordinary criteria or ‘override’ options to contextualise the assessment of appropriateness in relation to the relevant hospitals. An override can occur when, by clinical opinion of the reviewers, an individual case requires hospitalisation despite the fact that no protocol criteria have been met. Various studies have been carried out regarding 15 this tool with contradictory results . At present, these subjective criteria should only be applied by advanced users with certified AEP training. Another less common possibility is the modification of criteria that according to the Protocol would normally justify admittance, but which in the opinion of the reviewers added a factor of confusion to the situation that was assessed. An example of this would be the case of an ordinary intra-venous prescription at a higher volume than necessary to maintain the venous system permeable. Under normal conditions, more than 500cc per day could be considered fluid therapy and be an appropriateness criteria, however if we accept as an override that fluid therapy can only be considered if over 1000cc, only those patients that receive a volume higher than this would meet the appropriateness criteria. 11 Barlavento Médico 18 Anton P et al. Actualmente só se recomenda a aplicação destes critérios subjectivos nos utilizadores avançados e com formação certificada na utilização do AEP. Outra possibilidade menos utilizada, consiste na modificação de critérios que habitualmente justificam um internamento hospitalar segundo o Protocolo, mas que no entender dos revisores, no meio avaliado foi um factor de confusão. Este poderia ser por exemplo, o caso da prescrição por rotina de soros em volume superior ao necessário para a manutenção da via venosa permeável: em condições normais, um volume superior a 500 cc por dia poderia ser considerado soroterapia e ser um critério de adequação. Mas se aceitamos como critério que a soroterapia só se considera a partir de 1000 cc por dia, unicamente os doentes que recebam um volume superior a este, terão este critério de adequação. Decidir qual é a percentagem conveniente de inadequação das admissões num Serviço de Medicina é um problema complexo. Os diferentes estudos levados a cabo indicam percentagens que oscilam entre 9 - 24% 29,30 , dependendo das características do Hospital, do sistema de saúde e da patologia própria dos doentes. Por isso não podemos decidir que existam valores categóricos de “boa” ou “má” adequação. Em vez disso, a principal utilidade desta percentagem será a comparação da adequação entre hospitais com características semelhantes e como marcador de adequação dentro de um mesmo hospital, podendo ser utilizado como elemento de valorização do efeito de estratégias 28, 31 para optimizar a utilização das camas hospitalares . AEP NA PENÍNSULA IBÉRICA Na Península Ibérica, a introdução deste instrumento só se verificou na década de 90, quando começaram a aparecer os primeiros estudos com AEP em doentes 16, médicos, pediátricos e com patologias específicas 32, 33, 34 . Os primeiros estudos foram levados a cabo em doentes internados em serviços de Medicina Interna, sendo revistas de forma retrospectiva as histórias clínicas de uma amostra dos doentes internados durante um ano. A taxa de adequação era de 17% 16. Desde então, têm sido levados a cabo vários estudos, com uma taxa de adequação de admissões hospitalares 31, 35 entre 9 e 15% . Em Portugal, em meados dos anos 90, um projecto governamental estudou as taxas de inadequação dos internamentos e estadias em vários hospitais públicos, quantificando as taxas de inadequação dos internamentos não obstétricos em 25% 22 . Posteriormente, foram levados a cabo estudos em doentes internados em Serviços de Medicina Interna, com uma taxa de inadequação de internamento ron23, 24 dando os 20% . Desde os primeiros dias da sua aplicação, têm sido divulgadas opiniões díspares quanto Determining the convenient percentage of inappropriateness of admissions in a Health Service is a complex problem. Different studies carried out indicate 29,30 percentages that vary between 9 and 24% percent , depending on the characteristics of the Hospital, its health care system and the pathology of the patients. Therefore, we cannot say that there are absolute ‘good’ or ‘bad’ figures regarding inappropriateness. Rather, the main purpose of the determined percentage is to compare inappropriateness amongst hospitals with the same characteristics and to use it as a marker of appropriateness inside the same hospital. It can be used to determine the effect of strategies in order to optimise 28,31 the usage of hospital beds . AEP IN THE IBERIAN PENINSULA The introduction of this tool only took place in the 1990’s in the Iberian Peninsula when the first studies on the AEP started to appear on medical, paediatrics 16,32,33,34 and specific pathologies patients . The first studies were carried out on patients admitted to Internal Medicine Departments. For a period of a year a retrospective review of the medical records of a sample of these patients was carried out. The rate of inappro16 priateness was 17% . Since then, various studies have been carried out with a rate of inappropriateness of 31,35 hospital admissions varying between 9 and 15% . Interna) Após instituição de medidas para melhorar adequação de internamento After application of measures to improve inappropriateness rate Taxa inadequação desceu para Rate of inappropriateness dropped to 9% à utilidade do AEP para diminuir as admissões e os 36, 37 internamentos inadequados , embora tenham sido poucos os estudos que valorizam a efectividade do AEP como ferramenta real de trabalho na prática clínica. AEP NA ANDALUZIA Na Andaluzia, região do sul de Espanha, o primeiro estudo relevante publicado data de finais dos anos 90 39 . Este estudo realizou-se no Hospital Juan Ramón Jimenez de Huelva, um Centro de segundo nível que tem como área de influência a capital da província, cidades costeiras e um hospital de primeiro nível que serve uma região periférica montanhosa num total der cerca de 250 000 pessoas. Foi estudada de forma retrospectiva a adequação dos internamentos de 201 pessoas escolhidas aleatoriamente entre as 518 internadas entre os meses de Janeiro e Março de 1997. A taxa de inadequação dos internamentos oscilou nos 13%. Como causa principal de internamento inadequado encontrou-se a necessidade do estudo de uma forma rápida e que pela demora que implica realiza-lo de forma ambulatória não permitiu a sua realização desde a consulta, correspondendo este facto a mais de metade dos internamentos inadequados. Um ano depois repetiu-se o estudo numa amostra aleatória de doentes internados num determinado dia no Serviço de Medicina Interna (estudo concorrente) e a inadequação tinha aumentado aproximadamente 15%, sendo o mesmo motivo a maior causa da inade35 quação . Depois destes resultados, foi tomada a decisão de abrir uma Consulta de Orientação Diagnostica (CAD) efectuada por um Internista com contacto telefónico directo que poderia ser consultado pelos Médicos de Família da zona de saúde e que (num período razoávelem todos os casos inferior a uma semana), atenderia pessoalmente estes pacientes caso se achasse necessário, de forma a que existisse uma diminuição de afluência de doentes ao hospital por este motivo. Ao mesmo tempo, foi criada a Consulta Rápida de Medicina Interna (CRMI), que atendia doentes referenciados das Urgências num período de uma semana. A maior parte dos doentes com síncopes, anemias para estudo e síndromes constituicionais, precisavam previamente de internamento hospitalar para se levar a cabo um estudo de forma eficiente. Na CRMI além de uma resposta em tempo útil pelo Internista, conseguiu-se através de um Protocolo de colaboração com outros Serviços - principalmente Radiologia e Cardiologia - que os exames complementares solicitados por esta consulta tivessem uma prioridade semelhante aos dos doentes hospitalizados. No caso de os resultados obtidos na CRMI aconselharem ou de a evolução clínica do paciente o justificar, procedia-se ao interna- In Portugal, in the mid 1990’s, a government project studied the rates of inappropriateness of admittances and hospital stays in various state hospitals and quantified the rates of inappropriateness of the admittan22 ces in obstetrics at 25% . After that, studies were carried out in patients admitted to Internal Medicine Departments with a rate of inappropriateness of 23,24 approximately 20% . As far back as the first days of its implementation, there have been differing opinions regarding the usefulness of the AEP to reduce 36,37 admittances and inappropriate stays , although there are few studies that emphasise the effectiveness of the 21,28,38 AEP as a real working tool in medical practice . EAP IN ANDALUSIA In Andalusia, a southern region of Spain, the first relevant study published dates back to the end of the 39 1990’s . This study took place at the Juan Ramón Jiménez Hospital of Huelva. It is a level two centre in an area that includes the capital of the province, coastal cities and a level one hospital serving a mountainous surrounding region, for a total of about 300,000 people. A retrospective study was carried out on the appropriateness of the admittance of 201 people who were randomly chosen from 518 admittances during the months of January and March of 1997. The rate of inappropriateness of the admittances varied at around 13%. The main reason for the inappropriateness of admittances was due to the need for a rapid study not possible on an out-patient basis since hospital consultations imply lengthy waits. This factor accounts for the inappropriateness of over half of the admittances. One year later, the study was repeated on a randomly chosen sample of patients admitted to the Internal Medicine Department (concurrent study) and the inappropriateness had increased to over 15%. 35 The cause of inappropriateness had not changed . After considering these results, a decision was made to open a Diagnostic Orientated Consultation (DOC) which would be handled by an Internist who would be readily available by a direct telephone line to the Family Doctors of the health region. These physicians would, in a reasonable period of time, (in all cases less than one week) personally see the patients, if it proved necessary. This would reduce the number of patients coming to the hospital for this reason. Concurrently, an Urgent Internal Medicine Consultation (UIMC) was created to see patients referred to them from the Emergency Department (ED) within the same period of time. Most of the patients, with syncope, anaemia not known to the study and constitutional syndromes required hospital admittance so that a study could efficiently be carried out. Um ano depois da aplicação destas medidas, a percentagem de inadequação dos interna31, 38 mentos desceu 9,1% , pelo que se conclui que as medidas empregadas tiveram um impacto positivo na diminuição da inadequação dos internamentos hospitalares. Com esta experiência espanhola chegamos à conclusão que o AEP é um instrumento útil para determinar, não só a adequação dos ingressos e dos dias de presença hospitalar, mas ao mesmo tempo tem um papel importante na valorização das medidas destinadas a diminuir a inadequação, podendo ser aplicado noutras unidades assistenciais para optimizar a utilização das camas hospitalares. AEP NO HOSPITAL DO BARLAVENTO ALGARVIO Barlavento Médico 14 O Hospital do Barlavento Algarvio, hoje Centro Hospitalar, é uma unidade assistencial de segundo nível que tem como área de influência a região Oeste do Algarve o que em termos populacionais representa cerca 200,000 pessoas. Em 2002 tinha uma capacidade de 262 camas, das quais 44 pertencias ao Serviço de Medicina Interna. Tendo em conta as semelhanças com o Hospital Juan Ramón Jiménez de Huelva, tentou-se extrapolar a aplicação do AEP a esta Unidade Hospitalar começando com o Serviço de Medicina Interna. Um primeiro estudo piloto transversal foi realizado em 23 doentes, levado a cabo em Março do 2003, indicava que todas as admissões hospitalares eram adequadas devido à prescrição de soros ou medicação parentética 28 . Estes resultados foram discutidos numa Reunião de Serviço e os médicos responsáveis pelos internamentos em Medicina Interna alertados para valorizar a necessidade de utilização desta via. Num novo estudo transversal realizado um mês depois verificou-se que 4,3% das 23 admissões avaliadas foram inadequadas, e só 56% dos internamentos adequados eram devido à 28 prescrição de medicação parentérica . Simultaneamente, durante este mesmo ano levou-se a cabo um estudo prospectivo com a revisão sistemática das histórias clínicas dos doentes internados num dos quartos do Serviço de Medicina I. Foram avaliados 175 internamentos, encontrando-se uma taxa de inadequação de internamento de 17,7%, fundamentalmente devido à necessidade de um estudo rápido que não seria compatível com o ritmo de funcionamento das Consultas existentes, ou por ser necessário internamento num nível assistencial diferente de um Hospital de cuidados 24 agudos . A diferença entre as taxas de inadequação entre os dois estudos, transversal e prospectivo atribui-se, apesar do pequeno tamanho da amostra dos estudos transversais, ao efeito da consciencialização da requested at consultations, to have the same priority rating as that of hospitalised patients. A patient would only be admitted to hospital if the conclusions reached by the UIMC Internist made it advisable or if the evolution of the person’s medical condition made it necessary. The advantages were that the patient would already have a diagnosis and would have completed all diagnostic testing which would significantly reduce the hospital stay. After one year of implementing these measures, the percentage of inappropriate admittances 38,31 dropped to 9.1% . It can be concluded that these measures had a positive impact on the reduction of inappropriate admissions to hospital. The Spanish experience allows us to conclude that the AEP is a helpful instrument to determine not only the inappropriateness of admissions and hospital stays but also the impact of measures used to reduce inappropriateness. Furthermore, it can be applied to other health services to optimise the usage of hospital beds. EAP IN ALGARVE The West Algarve Hospital is a level two health care service. It covers the western region of the Algarve, Portugal, and serves a population of approximately 200,000 people. In 2002 it had a 262 bed capacity, 44 of which belonged to the Internal Medicine Department. Given the similarities to the Juan Ramón Jiménez Hospital of Huelva, this hospital’s AEP was first used with the Internal Medicine Department. In March of 2003 the first Pilot cross-study was done on 23 patients and indicated that all hospital admissions were appropriate given the prescription of fluids or intra-venous medication. These results were pointed out in a departmental meeting and the physicians who were responsible for admissions were encouraged to use this method. A new cross-study, concluded one month later, found that 4.3% of the 23 admissions that were assessed were inappropriate, and only 56% of the appropriate admissions were considered appropriate due to the prescription of intra-venous medication. At the same time, a prospective study was done on patients admitted to two wards of the Internal Medicine Department. A systematic study was done of their medical records. The admissions of 175 patients were evaluated and the rate of inappropriate admissions was 17.7%. This was attributed to the need for a quick study which was incompatible with the pace of existing consultations or because a patient needed to be admitted to a 24 non-acute care health facility . In spite of the reduced sample of the cross-study, the difference between the rates of inappropriateness found in the cross and prospective studies can be explained by the increased awareness of physicians of our Centre as regards the over usage of the intra-venous route when administering medication. sobreutilização da via parentética para a administração de medicação nos médicos do nosso Centro. Depois de avaliar cuidadosamente o peso específico de cada uma das causas da inadequação do internamento e a disponibilidade dos meios humanos e infra-estruturas do nosso Centro, conclui-se que o melhor sistema de diminuir a inadequação das admissões hospitalares seria dispor de uma unidade de continuidade assistencial. Nesta o doente que não necessitasse especificamente de internamento em Medicina Interna mas que precisasse de um nível de cuidados superior ao que se pode prestar com apoio domiciliário, pudesse permanecer desde que avaliados por Medicina Interna, com seguimento por médicos de Medicina Geral e com apoio pontual da nossa especialidade em caso de necessidade. Em Janeiro de 2004 abriu-se um protocolo de colaboração com o Hospital de Santa Casa da Misericórdia, Portimão, existindo 15 camas destinadas a este fim. Para esta Unidade, independente, institucional e funcionalmente do nosso Hospital, poderiam ser transferidos os doentes avaliados por Medicina Interna que se encontrassem clinicamente estáveis e cuja única justificação do internamento fosse a necessidade de algum tratamento com internamento hospitalar. Os doentes eram avaliados diariamente por um clínico geral e, em caso de necessidade, um internista da nossa instituição deslocar-se-ia a esta Unidade. Em caso de agravamento da situação clínica, o doente voltaria para o Hospital através do Serviço de Urgência. O estudo levado a cabo em 2005 com os doentes internados durante o ano anterior foi repetido com uma metodologia After carefully weighing each of the causes of inappropriateness of admission and the availability of human resources and infrastructure of our Centre, we concluded that the best way to reduce inappropriate hospital admissions would be to have a Continuing Health Care Unit. Here, care could be provided to patients who do not necessarily need to be admitted to an Internal Medicine Department but need more care than home assistance can offer. Patients could stay in this alternative health care setting once evaluated by Internal Medicine and be followed up by General Practitioners. When necessary, the Internal Medicine Department could assist on a case by case basis. In January of 2004 a joint protocol was established with Hospital da Santa Casa de Misericordia (Mercy Hospital) and 15 beds were set aside for this purpose. Patients could be transferred to this institutionally and functionally independent hospital after having been found by the Department of Internal Medicine to be clinically stable and in need of hospital treatment. Patients were assessed daily by a General Practitioner and if necessary an Internist of our hospital would go to this Hospital to give further therapeutic guidance. If the patient’s condition deteriorated, he/she would return to the Hospital through the Emergency Department. A 2005 study with patients admitted to hospital the previous year was repeated with the same methodology. In this study, 20.8% of the admissions did not meet any criteria for admission to an acute-care hospital 15 Barlavento Médico Hospital Juan Ramón Jiménez, Huelva (Espanha) Barlavento Médico 16 idêntica, verificando-se que 20,8% das admissões não cumpriu critério nenhum de entrada num Hospital de cuidados agudos após a abertura da Unidade de 24 Cuidados Continuados . O aumento das taxas de inadequação foi um achado desfavorável cujas causas trataremos de destrinçar. A principal delas foi que os doentes, na maior parte dos casos, foram transferidos de uma unidade de internamento e não da Urgência. Portanto, concluímos que a aparição de um novo recurso de hospitalização fez com que alguns doentes, que em condições normais, não teriam acedido ao nosso serviço, fizeram-no com o objectivo de estabilizar o paciente previamente a sua transferência para a Unidade de Cuidados Continuados. Contudo, este novo recurso teve um efeito positivo, diminuindo a estadia média dos nossos doentes de 8,44 dias para 7,22 dias 24. Quando se analisaram as causas de internamento inadequado no Serviço de Medicina no estudo de 2004, verificou-se que a necessidade de internamentos num centro assistencial diferente de um hospital de cuidados agudos assumiu um papel secundário, sendo a necessidade de um estudo de forma rápida a principal justificante do internamento inadequado. Isto reflecte de uma forma indirecta que a medida adoptada tinha contribuído para diminuir a inadequação associada à indisponibilidade de instalações alternativas. O facto de existirem internamentos inadequados devido à necessidade de agilizar o estudo ambulatório fez pensar que o problema principal naquele momento era a demora na obtenção dos estudos complementares e a observação por um internista nas consultas. No mesmo ano, o Hospital Distrital de Lagos passou a integrar o nosso Centro Hospitalar como uma unidade de Internamento. Aproveitando os dados obtidos por aquele estudo, foi reorientado como um serviço de Medicina Interna com uma capacidade de 22 camas, que posteriormente passou a 30. Foram destinados a este Centro precisamente aqueles doentes que se encontravam numa situação de estabilidade, mas que precisavam de uma orientação diagnostica/terapêutica por parte de um especialista em Medicina Interna. A repetição do estudo prospectivo em 2005 mostrou que só 12,3% dos internamentos foram inadequados segundo os critérios da AEP, sendo neste caso a principal causa de inadequação o internamento para levar a cabo o estudo de uma forma rápida40. A nossa meta para os próximos anos será, portanto, estabelecer uma infra-estrutura ambulatória bem coordenada com os serviços de Atendimento Primários que permita realizar o estudo de um doente em Tempo útil sem necessidade de internamento no Hospital. Como no exemplo do Hospital Juan Ramón Jiménez de Huelva que nos permitirá, além de optimizar a utilização das camas hospitalares, diminuir a frequência no Serviço de Urgência e estabelecer novos canais de comunicação entre o Médico de Família e o Internista. Para um futuro mais distante fica a ideia de evitar o following the opening of the Continuing Health Care 24 Unit . The increase in the rates of inappropriateness was rather discouraging and we will try to unravel the reasons for this: The main one was that patients were being transferred from a hospital ward and not from the Emergency Ward. We concluded that some patients who wouldn’t normally be admitted to our department were being sent there because they needed to be stabilised prior to being transferred to a Continuing Health Care Unit. Indeed, this re-hospitalisation had a positive effect. It reduced the average stay of our patients from 8.44 days 24 to 7.22 days . When we analysed the causes of inappropriate admissions in our department in the 2004 study, we found that admission to a non acute health care facility took on a secondary role. The need for a prompt study became the main reason of inappropriate admission. The adopted measure had indirectly contributed to reduce the inappropriateness associated with the unavailability of alternative facilities. The fact that inappropriate admissions existed due to the need to expedite out patient studies made us think that the main problem at that time was the delay in obtaining additional testing and the Internist’s attention at the Consultations. That same year, the District Hospital of Lagos joined our Hospital Centre with one facility. Making use of the data obtained from that study, it was restructured as a 22 bed Internal Medicine Department which has since grown to 30 beds. Stabilised patients who needed diagnostic and therapeutic guidance by an Internal Medicine specialist were transferred to this centre. The repetition of the prospective study in 2005 showed that only 12.3% of the admissions were inappropriate according to the AEP criteria. In this case the main cause of inappropriate admission was the need to admit patients as quickly as possible for study and not 40 possible on an out patient basis . Our goal for the coming years will be, therefore, to create a coordinated out patient infrastructure with the Primary Health Care Services to expedite studies without having to admit patients to Hospital. We can use the Juan Ramón Jiménez Hospital of Huelva as an example to optimise the use of hospital beds and thus reduce the number of people coming to the ED and establish new channels of communication between Family doctor and Internist. In the more distant future we should think about avoiding hospital admission solely for the purpose of intra-venous fluid treatments and create an Internal Medicine Day Hospital which could medicate and reassess patients regularly. É igualmente útil para a detecção de causas de inadequação dos doentes admitidos e demoras de internamento hospitalar e para elaborar estratégias destinadas a diminuir a inadequação bem como medir o impacto das medidas adoptadas para aumentar a adequação do uso do recurso ao internamento. unit Hospitals in Sanitary Health regions in Europe, has demonstrated to be an efficient tool for detecting structural deficiencies and over use of acute care hospital beds in the health system. Apesar da eficácia demonstrada pela vacina antipneumocócica heptavalente na redução da incidência de doença grave pelos serótipos mais virulentos (serótipos 4, 6B, 9V, 14, 18C, 19F, 23F) em crianças abaixo de 2 anos de 2 idade , continuam a existir casos de doença invasiva por exemplo, meningite em crianças vacinadas. Por outro lado, a introdução da vacina antipneumocócica heptavalente em grupos populacionais seleccionados, isto é, crianças menores de 2 anos e crianças com patologia cardíaca, pulmonar ou imunodeficiência predisponente, tem demonstrado consequências epidemiológicas que abrangem não só as crianças vacinadas, como toda a 3, 4 comunidade. CASE REPORT CASO CLÍNICO Correspondência | Correspondence [email protected] Criança de 10 meses de idade, sem antecedentes pessoais A 10 month old fully vaccinated (including 3 doses of Prevenar®) child, with no relevant personal or fam- Da observação da criança, salientava-se uma prostração acentuada com gemido contínuo, fontanela anterior hipertensa, frequência respiratória de 27 ciclos/min., frequência cardíaca de 130 batimentos/min., tensão arterial de 107/70 mm Hg e temperatura axilar de 40ºC, sem aparente foco infeccioso à inspecção. Os exames sanguíneos iniciais revelaram parâmetros sugestivos de infecção bacteriana, havendo uma neutrofilia de 83,7%, associada a PCR muito elevada (576 mg/L). Pela conjugação do quadro clínico com os resultados analíticos foi colocada a hipótese de meningite, e efectuada uma punção lombar. Os resultados demonstraram uma proteinorráquia de 170mg/dl, uma glicorráquia de 0 mg/dl, e pleocitose ligeira com predomínio de polimorfonucleares, tendo Diplococos Gram + em exame directo em conjunto com a presença de antigénios capsulares da espécie Streptococcus, colocando assim o diagnóstico de meningite bacteriana a Streptococcus. Sob internamento criança iniciou-se terapêutica empírica com Ceftriaxona e Vancomicina, segundo o protocolo actual (Ceftriaxona ev - 100mg/kg/dose de 12/12h no primeiro dia, seguido de 100mg/kg/dia de 24/24h, e Vancomicina ev - 60mg/kg/dia, 8/8h). ily histories, fell ill with moderate fever (38-38,5ºC) and vomiting, two days before hospital admittance. Twenty-four hours after the symptoms began, there was an escalation of the fever, accompanied by expiratory grunting and prostration, for which she was observed in the paediatric emergency room. From the examination of the child, the following stood out: marked prostration with continuous expiratory grunting, a tense fontanel, heart rate=130 beats/min, respiratory rate= 27cycles/min, blood pressure= 107/70 mmHg, and a high (40ºC) fever with no obvious infection site. The initial blood tests demonstrated an apparent bacterial infection, with a neutrophil count of 83,7% associated with an elevated C-reactive protein (576 mg/L). A working diagnosis of bacterial meningitis was formulated, based on the clinical and laboratorial findings, and a lumbar puncture was performed. The results demonstrated a protein measurement of 170 mg/dl, glucose measurement of 0 mg/dl, slight pleocytosis with polymorphonuclear cell predominance, and Gram + Diplococci on direct examination with the presence of Streptococcus species capsular antigens, thereby establishing the diagnosis of streptococcal meningitis. An increased subdural effusion with a frank mass effect. 23 Barlavento Médico ou familiares relevantes, e com o calendário vacinal obrigatório (e suplementar, com 3 doses de Prevenar®) actualizado, que iniciou quarenta e oito horas antes do internamento um quadro de febre moderada (38 - 38,5ºC), acompanhada de vómitos alimentares. Dado o agravamento, com evolução desfavorável com persistência de febre alta em associação com gemido e prostração, recorreu ao serviço de urgência pediátrica do Hospital Barlavento Algarvio. Barlavento Médico 24 FOLLOW-UP FOLLOW-UP Na evolução do caso registámos ao 3º dia de internamento o isolamento de Streptococcus pneumoniae, serótipo 33F, sensível à Ceftriaxona, pelo que foi suspensa a Vancomicina. Ao 4º dia ocorreu um episódio convulsivo tónico-clónico generalizado, com parésia do membro superior direito. Em TAC crânio-encefálica (TAC-CE) feita nesse dia Figura I verificamos a existência de lesões hipodensas frontais esquerdas, e efusão subdural direita sem efeito de massa. Ao 8º dia ocorreu novo episódio convulsivo, com posterior hipertonia, tremores e nistagmo para a direita. Em TAC-CE observava-se o derrame subdural ligeiramente aumentado e lesão talâmica à direita. Ao 12º dia ao exame objectivo a criança tinha diminuição dos movimentos espontâneos dos membros, e desvio ocular para a direita. Em TAC-CE (Figura II) verificávamos um aumento da colecção subdural, com franco efeito de massa. Foi transferida para o Hospital Santa Maria, e efectuada a drenagem de um empiema subdural, estéril. Do 13º dia ao 21º dia, data da alta, registou-se uma melhoria clínica, laboratorial e imagiológica gradual e progressiva. On the third day of admittance Streptococcus pneumoniae serotype 33F, susceptible to Ceftriaxone, was isolated, by which Vancomycin was discontinued. On the fourth day the child suffered a generalized tonicclonic convulsion, with resulting right arm paresis. The CAT-scan performed (Figure I) showed hypodense left frontal lesions, and a right subdural effusion with no mass effect. On the eighth day there was a new convulsive episode, with posterior hypertonicity, tremors and right-sided nystagmus. The corresponding CAT-scan demonstrated a slight increase in the subdural effusion, and the appearance of right thalamic lesion. By the twelfth day there was noted a decrease in spontaneous motion with right ocular slant. The CAT-scan performed (Figure II) showed an increased subdural effusion with a frank mass effect. The child was transferred to the Hospital de Santa Maria, a central Hospital, and submitted to the drainage of a subdural empyema, later known to be sterile. Finally, from the thirteenth to the twenty-first day, there was a gradually progressive clinical, laboratorial and radiological improvement. DISCUSSÃO A vacina heptavalente, criada para incluir os serótipos predominantemente responsáveis por doença invasiva grave em crianças abaixo dos 2 anos de idade, apresenta, como demonstrado pelo estudo prospectivo Kaiser 3 Permanente em 2000 , uma eficácia de cerca de 87% na redução de doença pelos referidos serótipos. Existem, no 5 entanto, mais de 90 serótipos conhecidos , sendo difícil, senão mesmo impossível, aumentar esta eficácia através da inclusão de todos numa vacina “omnivalente”. A “erradicação” na comunidade vacinada dos serótipos incluídos na vacina, conduz, em relativamente pouco tempo, à emergência de novos serótipos predominantes responsáveis por doença invasiva grave. O caso apresentado é exemplo, onde uma criança vacinada desenvolveu meningite por um 33F 3 serótipo não incluído na vacina. Watson et al , no ano seguinte à introdução da vacina heptavalente em crianças de risco na Austrália, observaram um aumento estatisticamente significativo no número de casos de doença pneumocócica por serótipos não incluídos na vacina. Alterações semelhantes na colonização nasofaríngea de crianças vacinadas foram 4 6 observadas por Reinier et al e Frazão et al , na Holanda e em Portugal respectivamente. Pela existência de este elevado número de serótipos, torna-se pouco provável, ao contrário do caso do Haemofilus influenzae, a erradicação definitiva de doença pneumocócica invasiva grave através da vacinação. Contudo, a vacinação infantil induz imunidade de grupo, DISCUSSION The heptavalent antipneumococcal vaccine, especially created to include the serotypes predominantly responsible for serious invasive disease in children under 2 years of age, has, as demonstrated by the Kaiser Permanente 3 Trial in 2000 , an efficacy of 87% in reducing disease caused by those specific serotypes. There are, however, 5 about 90 known serotypes , thereby making it difficult, if not impossible, the inclusion of all in an “omnivalent” vaccine. The “eradication” of the vaccine serotypes in the vaccinated population leads to the emergence, in a relatively short period of time, of new predominant serotypes responsible for serious invasive disease, as demonstrated by our case of serotype 33F meningitis 3 in a vaccinated child. Os seus sintomas, manifestações clínicas e analíticas característicos, assim como a existência de testes específicos para o microrganismo, facilitam o diagnóstico desta doença. Isto permite começar o tratamento antibiótico específico com macrólidos, a terapêutica de escolha na actualidade. Outras vezes, no entanto, esta infecção pode-se manifestar como uma pneumonia adquirida na comunidade, causada por germens habituais, com uma evolução inadequada apesar do tratamento médico estandar. Neste artigo apresentamos dois casos de pneumonia por legionella com apresentação como pneumonia adquirida na comunidade em pessoas sem comorbilidade, com má evolução ao tratamento médico instituído. Salientamos a importância da identificação precoce das alterações sugestivas desta infecção, com o objectivo de começar o tratamento médico adequado após confirmar a etiologia com os testes serológicos. ABSTRACT: Pneumonia caused by Legionella is a relatively frequent cause of hospitalisation in people without associated commorbility. Its symptoms, clinical manifestations and analytical characteristics, as well as the existence of specific tests for the causative microorganism, facilitate the diagnose of this illness, being able, for in such a way to start the specific antibiotic treatment with macrolides, which is at this time the treatment of choice. Sometimes however, this infection can be revealed as community-acquired pneumonia caused by usual germens, with an inadequate evolution despite the standard medical treatment. In this article we present two cases of pneumonia caused by legionella with typical presentation as community-acquired pneumonia in people without associated commorbility with bad evolution to the instituted medical treatment. We point out the importance of the precocious identification of the suggestive alterations of this infection so that we can start adequate medical treatment after confirming the aetiology with the serologic tests. CLINICAL CASE 1: Doente do sexo masculino, de 37 anos de idade, fumador de 60 U.M.A é admitido no Serviço de Urgência por um quadro de tosse com expectoração purulenta, dispneia, febre elevada e dor torácica tipo pleurítica com 48 horas de evolução. Na história clínica pregressa há a referir mialgias generalizadas com melhoria significativa três dias antes do início da febre. Sem antecedentes pessoais ou familiares relevantes. Ao exame objectivo apresentava-se vigil, colaborante, com temperatura axilar de 38,5ºC, frequência respiratória de 24 ciclos por minuto, frequência cardíaca de 115 batimentos por minuto e Tensão Arterial de 140/60 mmHg. À auscultação pulmonar (AP) fervores crepitantes na base do hemitórax esquerdo com sibilos dispersos. Restante exame objectivo irrelevante. Analiticamente apresentava leucocitose com neutrofilia; Hiponatrémia marcada e P.C.R elevada, (Quadro 1). Objective examination: space and time orientation, axillary temperature was 38,5ºC, respiratory frequency: 24 cycles for minute, heart rate: 115 beatings per minute and blood pressure: 140/60mmHg. Lung auscultation: crackles on the base of left hemi-thorax and dispersed hissings. The remaining examination was irrelevant. Analytically: leukocytosis with neutrophilia, marked hiponatremia and raised c-reactive protein (CRP). The admission x-ray of the thorax revealed (table1) an alveolar condensation on the posterior segment of the superior pulmonary lobe and an alveolar infiltrate on the left inferior lobe. Com o diagnóstico de pneumonia grave da comunidade iniciou tratamento com Ceftriaxone sob internamento. A evolução inicial foi desfavorável com diminuição do nível de consciência e agravamento da dispneia, tendo evoluído para insuficiência respiratória aguda pelo que foi transferido para a Unidade de Cuidados Intensivos (UCI), onde houve necessidade de ventilação mecânica. Na UCI o doente apresentou agravamento da insuficiência respiratória com aparecimento radiológico de infiltrado reticulointersticial difuso, extenso e bilateral, tendo sido diagnosticado de sindrome de distress respiratório do adulto (ARDS), (Figura 2), para além de sépsis grave com falência hemodinâmica. Na urina identificou-se o antigénio para Legionella pneumophila serogrupo 1, pelo que iniciou tratamento com eritromicina, apresentando melhoria clínica e radiológica que tornou possível ao fim de 9 dias a extubação, transferência para a enfermaria de Medicina e posterior alta com seguimento em Consulta Externa de Pneumologia. The arterial gasometry (table2) showed respiratory alkalosis with hypoxemia. It was diagnosed as a severe community-acquired pneumonia. Ceftriaxone treatment was started under hospitalisation. The initial reaction to the treatment was not as good as expected and there was a decrease on the level of conscience and worsening of the dyspnea leading to an acute respiratory insufficiency and consequent transferring of the patient to the intensive care unit (ICU), where mechanical ventilation was performed. On the ICU the respiratory patterns worsened and the x-ray showed an extensive bilateral interstitial reticulum infiltration, and the patient was diagnosed as an adult respiratory distress syndrome (ARDS) besides severe sepsis with hemodynamic failure (fig.2). It was identified the anti-gene for Legionella Pneumophila serogroup 1 by urinary culture, and treatment was started with eritromycine. There was a significant improvement on the patient status as well as on the x-ray picture. The patient was extubated after 9 days and transferred to the internal medicine ward where soon afterwards was released home. Fig. 1 - Radiografia de tórax. Posteroanterior. | Lung X-ray CLINICAL CASE 2 Doente de 34 anos de idade, sexo masculino, com hábitos tabágicos de 50 U.M.A e alcoólicos de 60 g etanol/dia é admitido no Serviço de Urgência por febre alta, tosse com expectoração purulenta acompanhado de dor torácica tipo pleurítica à direita e vómitos com 2 dias de evolução. O doente referiu um quadro auto limitado de mialgias generalizadas e diarreia aquosa de 3 dias de evolução, uma semana antes do internamento, interpretado como um quadro de gastroenterite aguda. Sem antecedentes pessoais relevantes. No exame objectivo evidenciou-se, desidratação e hipertermia de 39ºC. A.P com fervores crepitantes bilaterais. Analiticamente apresentava leucocitose com neutrofilia; hiponatrémia e PC-R elevada, assim como elevação discreta das transaminases (cerca de 2 vezes o limite superior normal), gama-GT, fosfatase alcalina e LDH, (Quadro 1). A gasometria arterial foi compatível com hipoxémia moderada com um ph normal e sem retenção de CO2 , (Quadro2). Na radiografia de tórax constatou-se a existência de pneumonia à direita com um foco de condensação alveolar na base do pulmão direito apagando a silhueta cardíaca, (Figura 3). Colocou-se o diagnóstico de pneumonia adquirida na comunidade e iniciou tratamento com Amoxicilina e Ác.clavulânico. Quarenta e oito horas depois do tratamento a evolução clínica apresentava-se desfavorável com aumento da postração, febre alta mantida e hipoxémia grave. Radiologicamente com um agravamento significativo, com aparecimento de um infiltrado reticulointersticial bilateral, mais marcado à direita, (Figura 4). Perante este agravamento clínico e radiológico, apesar da terapêutica instituída, pensamos tratar-se de numa pneumonia da comunidade por agentes atípicos e foi- A 34-year-old male, smoker (50 pack year smoking units), alcoholic habits (60g ethanol/day alcoholic units). Admitted in the emergency department with high fever, productive cough, pleuritic chest pain on the right axis and vomits that started 2 days before. The patient also referred generalized muscle pain and watery diarrhea with 3 days of evolution, one week prior to the hospitalisation interpreted as an acute gastroenteritis. Objective examination: The patient was dehydrated and axillary temperature was 39ºC. Lung auscultation: bilateral crackles and wheezes. Analytically: leukocytosis with neutrophilia, marked hiponatremia and elevated CRP, as well as discrete rise of transaminases (twice the normal value), alkaline phosfatase, LDH, GGT (table1). The arterial gasometry was compatible with moderate hypoxemia with normal ph and without CO2 retention (table2). Thorax x-ray revealed pneumonia over the right axis with alveolar condensation on the base of the right lung in such way that it was impossible to see the cardiac silhouette (fig.3). The patient was diagnosed with community-acquired pneumonia and treatment was started combining Amoxicillin and clavulanic acid. 48 hours after the treatment was started the clinical evolution was quite bad with decreased consciousness level, high fever and serious hypoxemia. The x-ray showed a significant aggravation with marked bilateral interstitial reticulum infiltration picture over the right side (fig.4). Due to clinical and radiological worsening despite instituted treatment one thought about communityacquired pneumonia by atypical agents. O doente manteve inicialmente febre alta com valores tensionais baixos, tendo sido transferido para a UCI, onde teve posteriormente uma evolução favorável sem necessidade de suporte com aminas vasoactivas, nem ventilação asssitida, pelo que ao fim de 7 dias foi transferido para a enfermaria do Serviço de Pneumologia e 2 dias depois teve alta clínica com seguimento em Consulta Externa de Pneumologia. DISCUSSÃO O termo legionelose refere-se aos dois sindromes clínicos diferentes causados pelas bacterias do género Legionella: Febre de Pontiac, um processo agudo, autolimitado, com uma duração aproximada de 48 horas, caracterizado por um quadro de febre associado a mialgias, poliartrialgias, nauseas e vómitos que não 6 necessita de tratamento antibiótico . E a Pneumonia por Legionella com uma forma de apresentação inicial similar às outras pneumonias causadas por agentes atípicos com as quais é importante establecer o diagnóstico diferencial. Geralmente a pneumonia por legionella manifesta-se inicialmente com um quadro de febre elevada, anorexia, mialgias generalizadas, tosse seca, dor torácica tipo pleurítica associado a manifestações 4 gastrointestinais como diarreia e vómitos . Em 20% dos casos, a legionelose provoca manifestações extrapulmonares sendo as mais frequentes sinusite, celulite, 7, 8 pancreatite, miopericardite e pielonefrite . Estas manifestações são secundárias à bacteriémia por 9 legionella e estão associadas a um pior prognóstico . O diagnóstico de pneumonia por legionella está condicionado pela disponibilidade de provas analíticas específicas, sendo a cultura da expectoração em meios com agar tamponado com extracto de levedura de carvão e L-cisteína (BCYE), a prova de maior sensibilidade e especificidade, embora requeiram 3 a 5 dias para serem 10 positivas . A determinação do antigénio solúvel na urina de Legionella é, depois da cultura da expectoração, a prova mais sensível e específica. É um metodo rápido, barato e fácil de executar, sendo util só para o serogrupo 1 da Legionella pneumophila que é responsavél aproxima11,12 damente por 70% das infecções. . Analititicamente é frequente a hiponatrémia, hipofosfatémia, alterações da 13 função hepática e trombocitopénia . Radiologicamente não apresenta um padrão característico, na maioria dos casos existe uma condensação broncoalveolar localizada, mas pode apresentar tambem um infiltrado difuso 14 ou instersticial . Nos dois casos clínicos apresentados, constatou-se a existência de febre elevada, tosse com expectoração purulenta e dor torácica tipo pleurítica associado a um padrão radiológico de condensação localizada e paramêtros de infecção aguda bacteriana com leucocitose maintained high fever with low blood pressure values by which he was transferred to the ICU, where he improved quite rapidly. Within 7 days the patient was transferred to the pneumology ward and 2 days later was sent home. DISCUSSION The term legionellosis refers to two different clinical syndromes caused by the Legionella bacteria: Fever of Pontiac, an acute process, auto-limited, with an approximate duration of 48 hours, characterized by several symptoms and signs such as fever, mialgyas, polyartrialgias, nauseas and vomits that do not require antibiotic treatment and the Pneumonia caused by Legionella with an initial presentation similar to other pneumonias caused by atypical agents by which it is important to establish the differential diagnosis. Usually the pneumonia caused by legionella manifests itself initially with: high fever, anorexia, mialgyas, dry cough, thoracic pain and gastrointestinal manifestations such as diarrhea and vomits. In 20% of the cases, legionellosis causes extra pulmonary manifestations being sinusitis, cellulite, pancreatittis, pericarditis and pyelonefritis. These manifestations are secondary to bacteraemia from legionella and are associated to a worse prognosis. The diagnosis of pneumonia caused by legionella is conditioned by the availability of specific analytical tests, being the sputum culture on agar and L-cysteine the most sensitive and specific test even tough it requires 3 to 5 days to reveal themselves positive. The determination of the anti-gene for legionella soluble on urine is after the sputum culture one of the most specific and sensitive test. This method is fast, cheap and easy to perform, although useful only for serogroup 1 of Legionella Pneumophila responsible for approximately 70% of infections. Analytically is rather common to obtain hiponatremia, hypophosfatemia, changes in liver function and thrombocitopenia. On the x-ray we cannot observe a characteristic standard, but on the majority of cases appears a localized bronchoalveolar condensation, but it can also show a diffuse infiltrated or an interstitial one. On both clinical cases we could observe and measure the presence of high fever, cough with purulent sputum and pleuritic chest pain related to a standard radiological condensation and acute bacterial parameters with leukocytosis, neutrophilia and elevated CRP. On clinical case 1, the patient had besides the clinical history, generalized mialgyas one week before the beginning of the fever and analytically, besides the related parameters of infection, also hiponatremia without other associated pathology. No caso clínico 1, o doente apresentara para além desta clínica, mialgias generalizadas uma semana antes do início da febre e analiticamente, para além dos paramêtros de infecção referidos, a existência de uma hiponatrémia marcada, sem outra patologia associada. No caso clínico 2, o doente referiu igualmente na história pregressa mialgias generalizadas associado a um quadro diarreia 4 dias antes do internamento, hábitos tabágicos e etanólicos marcados e analiticamente documentou-se tambem uma hiponatrémia com alterações discretas das transaminasas e LDH elevada. As manifestações clínicas apresentadas nos dois casos, associadas ao facto de se tratarem de doentes jovens e sem antecedentes patológicos de interesse, foram suficientes para pensar que se tratava de uma pneumonia comunitaria causada por agentes típicos. No entanto nos dois casos houve dados que nos poderiam alertar sobre a etiologia atípica do quadro infeccioso. Nomeadamente o quadro de mialgias generalizadas e diarreia vários dias antes da consulta ao Serviço de Urgência, quadro que não foi valorizado pela resolução autolimitada do mesmo. Do ponto de vista laboratorial, é sugestivo duma etiologia atípica a presença de hiponatrémia marcada, alterações das transaminases e LDH elevada. Os dois casos, foram interpretados como pneumonias da comunidade por agentes típicos e iniciaram tratamento com ß-lactâmicos, após colheita de culturas. Perante o agravamento clínico nos dois doentes com aumento da prostração, febre elevada persistente, insuficiência respiratória aguda e agravamento radiológico com extensão pulmonar bilateral do infiltrado broncoalveolar e dada a negatividade das culturas solicitadas após 48h da admissão hospitalar, pensou-se em uma infecção respiratória da comunidade de etiologia atípica. Ao fim de 72 horas de internamento foi pesquisado o antigénio da Legionella na urina que foi positivo em ambos os casos, o que permitiu iniciar o tratamento específico obtendo-se um resultado favorável nos dois doentes. Historicamente, o tratamento de eleição para a pneumonia por legionella foi a eritromicina, sendo ainda na 15 actualidade muito utilizada e com bons resultados . Devido aos seus efeitos secundários como as alterações gastrointestinais, flebite e ototoxicidade derivados das altas doses necessárias para a sua eficácia e às recidivas 16, 17 documentadas em doentes imunodeprimidos são cada vez mais utilizados os novos macrólidos e quinolonas como a Claritromicina e a Levofloxacina, respectivamente com melhor tolerância gastrointestinal, poder de penetração tecidular e actividade intracelular 18, 19 que a eritromicina . No caso clínico 1, optou-se pela eritromicina com boa tolerância e eficácia clínica ao fim de 2 dias de tratamento. No caso clínico 2, o doente apresentava inicialmente vómitos e náuseas motivo para a opção de utilizar outro macrólido com menor analytically was registered also hiponatremia with discrete elevation of transaminases and lactate dehydrogenase. We shall notice the fact that on both cases we were dealing with young patients with irrelevant past history and associated with the clinical symptoms and signs it was enough to diagnose community-acquired pneumonia caused by typical agents. However on the same two cases some indications were given that were able to alert us to an atypical infection, such as the generalized mialgyas and diarrhea some days before admission on the emergency department. On the laboratorial point of view, the presence of hiponatremia, changes on transaminases and LDH suggested an atypical aetiology. Both cases had been interpreted as community-acquired pneumonias by typical agents and treatment was started with B-lactamics after harvesting the cultures. Due to clinical worsening status of both patients, together with persistent fever, acute respiratory insufficiency and pulmonary radiological aggravation and since the requested urinary cultures 48 hours after admitting the patients were negative we thought about an atypical community-acquired respiratory infection. 72 hours after patients admission the anti-gene of Legionella was searched on urine culture and revealed positive on both cases, which allowed us to start the treatment with good results on both patients. Before sometime the treatment of choice for pneumonia caused by legionella was eritromycine, and nowadays is still widely used with good results. (Although due to several secondary effect such as gastrointestinal changes and ototoxicity from the high doses necessary for its effectiveness each time macrolides and quinolones such as Claritromicine and Levofloxacine, with better gastrointestinal tolerance, and ability to penetrate intracellularly are chosen instead eritromycine). In clinical case 1, eritromycine was chosen with good tolerance and clinical effectiveness after 2 days of treatment. In clinical case 2, patient presented initially vomits and nauseas and so it was decided to use a macrolide instead, which had better gastrointestinal tolerance. In this case was used intravenous claritromicine where good response was obtained within 48 hours after starting the treatment. CONCLUSION Community-acquired Pneumonia by Legionella has a mortality rate of 20% without adequate treatment and of 50% when the infection is nosocomial. An early diagnosis allows the physician to initiate the therapy earlier and in these cases mortality rate is less than10%. Not always the disease presents as respiratory infection by atypical agents and not only older people or immunosupressed are affected, so in some cases it is not easy to achieve the correct diagnose and 31 Barlavento Médico 14 MuderRR,Yu VL, Parry MF.The radiologic Barlavento Médico 32 afectação gastrointestinal, claritromicina endovenosa, obtendo neste caso igualmente uma evolução clínica favorável após 48 h de tratamento. Assim e como conclusão diremos que a pneumonia comunitária por Legionella tem uma mortalidade de 20 20% sem tratamento adequado e de 50% quando 21 a infecção é nosocomial ; o seu diagnóstico precoce permite iniciar mais rapidamente o tratamento, sendo 22, 23 a mortalidade nestes casos inferior a 10% . Nem sempre se apresenta com uma clínica de infecção respiratória por agentes atípicos e nem só as pessoas idosas ou imunodeprimidas são susceptíveis, pelo que em alguns casos não é fácil establecer o diagnóstico correcto podendo inicialmente ser confundida com uma infecção respiratória da comunidade por agentes típicos e medicada com ß-lactâmicos. Os nossos doentes, apesar do agravamento clínico inicial, tiveram uma rápida melhoria clínica, radiológica e laboratorial após o início do tratamento com macrólidos. Nestes dois casos a determinação do antigénio para Legionella pneumophila foi decisiva na resolução do quadro. Devemos suspeitar do seu diagnóstico também em doentes jovens, sem comorbilidade e que não respondan favoravelmente ao tratamento com ßlactâmicos associando empíricamente um macrólido ao tratamento inicial. Na maioria dos casos uma historia clínica completa e uma interpretação correcta das provas complementares serão úteis na identificação e tratamento precoce desta patologia. on an early phase might be misdiagnosed as community-acquired pneumonia by typical agents and treated with B-lactamics. Patients, despite the initial clinical worsening, have had a fast clinical, radiological and laboratorial improvement after macrolides were prescribed. On both cases, determination of the anti-gene for Legionella Pneumophila was crucial to solve these cases. We must also suspect this diagnosis in younger patients without associated commorbility and that do not respond properly to therapy with B-lactamics by adding a macrolide to the initial treatment. In the majority of the cases a complete clinical history and a correct interpretation of the complementary tests will be useful in an early identification and treatment of this pathology. Bem conhecida nos primórdios da patologia no contexto da tuberculosa ou outra doença inflamatória peritoneal, torna-se comum depois do advento da cirurgia abdominal na segunda metade do século XIX, pela obstrução intestinal que estas aderências pós-operatórias criam. É uma patologia que ainda hoje mantém importante morbilidade. O autor revê a evolução histórica desta doença e apresenta uma imagem dos primórdios do registo gráfico desta patologia. ABSTRACT: Knowledge of the existence of intraabdominal adhesions extends back to the ancient Egyptians. Well recognized by early pathologists following tuberculosis and other peritoneal inflammatory diseases, intestinal occlusion created by postoperative adhesions became a common complication after the advent of abdominal bowel surgery in the late 19th century. It still is considered to be the cause of important morbidity today. The author reviews the historical progression of this pathosis and presents a very early graphic image found. No entanto as consequências da patologia originada pelas bridas e aderências intra-abdominais, permaneceu praticamente inalterada. Sabemos hoje que estas membranas ou autênticos cordões de tecido fibroso que ligam vários órgãos intra-abdominais que normalmente não têm pontos fixos entre si lideram as causas de obstrução intestinal. São responsáveis por mais de 40% de todos os casos de obstrução intestinal 1 e de 60 – 70% do intestino delgado . Para além da oclusão intestinal que pode ocorrer no pós-operató2 rio imediato ou 34 anos após o acto cirúrgico , esta patologia é responsável ainda por quadros de dor abdominal, infertilidade – 15 a 20% desta é causada 3 4 5 pelas aderências – gravidez ectópica , dispareunia ou 6 isquémia intestinal . O conhecimento das adesões remonta igualmente ao antigo Egipto onde há registo de severas adesões pélvi7 cas num doente, embora não se sugira um tratamento . A descrição mais antiga de adesões em relação com lesão funcional de órgão, tanto quanto se conhece, é o registo no Talmud babilónio neste caso para adesões 8 pleurais . Ao longo do tempo, em particular na Idade Média, vai persistindo um debate acerca das adesões pleurais como causa ou consequência de perfuração. A partir de Hunter (1728-1793) temos estabelecida a rela- In the second half of the twentieth century, enormous advances were made in virtually all fields of medicine. However, on entering the twenty-first century, the graveness of the consequences of abdominal fibrosis, with its adherences and fibrotic bands, has remained practically unaltered. We know today that these membranes, or authentic fibrotic laces, bind the intra-abdominal viscera that normally do not have fixed points between each other leading to intestinal occlusion. These membranes are responsible for 60-70% of all small bowel disease and more than 40% of all the cases 1 of intestinal occlusion . Intestinal occlusion can occur in the immediate postoperative period or 34 years 2 after surgery . It is still responsible for abdominal pain, 3 infertility (15 to 20% are caused by adhesions) , ectopic 4 5 6 pregnancy , dispareunia and intestinal ischemia . The knowledge of adhesions retraces equally to ancient Egypt where a case of severe pelvic adhesions in a sick person is registered on papyrus, although there is 7 no suggestion of a treatment . The oldest description of adhesions in relation with functional organ injury, as far back as is known, is a register in the Babylonian 8 Talmud of a case of pleural adhesions . Throughout evolution, with particular reference to the Medieval Age, the debate persisted concerning pleural adhesions as a cause or a consequence of perforation. Estas bridas que podem ser congénitas são normalmente secundárias a um processo intrabdominal como uma cirurgia, uma inflamação ou um trauma. Estas lesões referenciadas nos primórdios da patologia no contexto da tuberculosa ou outra doença inflamatória peritoneal, tornam-se comuns depois do advento da cirurgia abdominal na segunda metade do século XIX, pela obstrução intestinal que estas aderências pós-ope10 ratórias criam . Brant em 1897, descreve um caso de adesividade no íleo depois de exerése de quisto do ová11 rio . Payr em 1911, descreve a relação entre as adesões a fibrinólise e a importância das primeiras 48 h para a 12 formação destas aderências . Posteriormente, utilizou a expressão “problemas de adesividade peritoneal” chamando a atenção da maior relevância do problema pelo maior número de laparotomias que cirurgiões e ginecologistas praticavam. Valoriza a lesão da mucosa pelo trauma na laparotomia asséptica e a lesão do peritoneu pela falta de humidade, lesões químicas e térmicas. Ao longo dos anos vão sendo propostas uma série de soluções para evitar a formação destas aderências. Wiseman8 em 2000 dá-nos uma visão ampla do tema e 1 between the adhesions, peritonitis and infection . Danbarski, in 1889, described with basis on experimental 10 work on the dog , postoperative causes of peritoneal adhesions, emphasizing on adhesions located on the ileum, and concluded that adhesions could be induced by the gauze fibres as well as by intestinal scarifica10 tion . Brant, in 1897, described a case of adhesions 11 of the ileum after excision of an ovarian cyst . Payr, in 1911, described the relationship between adhesions, fibrinolysis and the importance of the first 48 hours 12 for the formation of these adhesions . The expression “problems of peritoneal adhesiveness” was later used by him to call attention to the great relevance of the problem through the large number of laparotomies that surgeons and gynaecologists were practising. He valued traumatic injury of the mucosa and peritoneal injury due to lack of moisture, chemical and thermal injuries in aseptic laparotomy. Congenital bands have been found to exist, but the great majority of all adhesions are secondary to some intra abdominal process such as a surgery, inflammation or intra-abdominal trauma. Adhesions following tuberculosis and other peritoneal inflammatory diseases were well recognized by early pathologists, but it was not until the advent of abdominal surgery in the 19th century that postoperative adhesions, and thus small bowel obstruction complications, became common. Barlavento Médico 36 reporta uma primeira tentativa para a resolução destas lesões motiva uma comunicação com a utilização de óleo de massagem para impedir a sua formação em 1880s. Depois, respectivamente em 1886 e 1889, tanto Muller como Malcolm, descreveram a utilização de soluções salinas para “float the intestines”. Conceito que como iremos verificar mantém toda a actualidade. Um agente chamado “fibrinolysin” (tiosinarnine e salicilato de sódio) foi introduzida em 1892, mas não teve eficácia e caiu em desuso. A goma-arábica foi recomendada como “lubricant between the viscera” em 1902, e em 1905, a Johnson and Johnson lançou no mercado uma preparação de peritoneu cecal bovino (disponível até 1993). Desde então, uma grande variedade de substâncias, algumas bem exóticas, tem sido utilizada numa tentativa de evitar a formação destas adesões pós-operatórias: intestino grosso de touro, peritoneu de tubarão, lanolina, bílis, membrana amniótica, bexiga de peixe, retalhos de epiploon e múltiplas películas. IMAGEM NOTÁVEL DE 1839 A Anatomia ciência morfológica do estudo da arquitectura do corpo baseado na dissecação, legou à História da Medicina tratados com excepcionais ilustrações. Disciplina muito tempo limitado aos dados de Galeno (129-200), iria ter um novo impulso na Idade Média a que se juntou com a dinâmica trazida pela impressa de Gutenberg, cerca de 1450. Vários autores publicam tratados de anatomia com ilustrações. De entre estes André Vesálio publica em 1543 De humani corporir fabrica, obra notável de anatomia tanto no plano conceptual com estético e chegamos ao século XIX com um corpo de excepcionais tratados de anatomia. Nessa altura com a litografia era possível publicar “sem demasiados encargos, obras iconográficas muito volumosas (…) um favor aos médicos colocar ao alcance de todos o conjunto dos trabalhos que tiveram 13 a anatomia como objecto” . Estamos nos inícios dos anos 1880s, a anatomia encontrava-se no seu apogeu e escrevia-se: “Sem a anatomia a fisiologia é apenas um tecido de fábulas mais ou menos engenhosas, a cirurgia encontra-se sem orientação e a medicina está reduzida 13 a um empirismo cego” . É patente o primado existente então da anatomia entre as disciplinas médicas e na evolução dos conceitos científicos. A anatomia patológica faz igualmente o seu percurso e entre 1828 e 1842 Léon Jean Baptiste Cruveilhier publica em Paris um memorável tratado Anatomie pathologique du corps humain. A Cruveilhier (17911874) foi conferido o título de doctor de medicina em Paris, em 1816, com uma dissertação com uma nova classificação de órgãos de acordo com suas alterações patalógicas. Em 1830 torna-se director do Hospice de la Maternité, mais tarde Salpêtrière e Charité, e em 1836 tornou-se o primeiro catedrático da cadeira de anatomia patalógica, que tinha sido estabelecida pelo 14 seu professor, Dupuytren . Dedicando sempre um A first attempt for its resolution, in 1880s, motivated a paper on the use of massage oil to hinder the formation of adhesions. In 1886, Muller spoke of the use of a saline solution and in 1889; Malcolm described the use of fluid to “float the intestines”. This concept, we go on to verify, is pertinent today. A SINGULAR IMAGE FROM 1839 Anatomy, the morphologic science of the study of the architecture of the body based on dissection, bequeathed to the History of the Medicine compendiums with exceptional illustrations. Long limited to Galen’s data (129-200), anatomy would go on to have a new impulse in the Medieval age with the Gutenberg prints cerca 1450. Various authors publish illustrated compendiums of anatomy. Among them, Andres Vesálio publishes, in 1543, De humani corporir fabrica, a highly notable work on anatomy in its conception as well as aesthetically. We arrive at XIX century with a body of exceptional textbooks on anatomy. At this point, with lithography, it was possible to publish “without too much expense, voluminous iconographic compendiums (…) as a favour to doctors to place, within reach of all, the object of a set of works of anatomy”. We are at the beginnings of the 1880s, anatomy was at its epitomy and thus was written: “Without anatomy, physiology is just a weaving of more or less ingenious tales, surgery finds itself without orientation 13 and medicine is reduced to blind empirism” . The existing primacy of anatomy then is evident amongst the medical disciplines and in the evolution of scientific concepts. Pathological anatomy equally follows suit between 1828 and 1842. Léon Jean Baptiste Cruveilhier publishes, in Paris, a memorable compendium on Anatomie pathologique du corps humain. Cruveilhier (1791-1874) was conferred with the title of Doctor of Medicine, in Paris in 1816, for his dissertation on a new classification of organs according to pathological alterations. In 1830 he became physician-in-chief and director of the Hospice de La Maternité, later Salpêtrière and the Charité, and in 1836, he became the first Professor to hold the chair of pathological anatomy, which had been established by his Professor, 14 Dupuytren . O peritoneu com uma superfície de 10,000 cm2 no adulto, quase igual à superfície da pele, diminui a fricção entre as vísceras abdominais permitindo o seu movimento livre. Apresenta à superfície células mesoteliais altamente diferenciadas e é a este nível que, após um factor desencadeante, por exemplo um acto cirúrgico, ocorre aquela cascata de eventos que pode confluir para a formação de bridas e aderências. Estas podem ser responsáveis por um cortejo de sintomatologia que pode ir da dor abdominal à oclusão intestinal. O seu diagnóstico é em larga medida confirmado intra-operatoriamente, já que não dispomos de exames complementares de diagnóstico para a sua identificação. Pela sua natureza não visualizamos estas lesões, só identificamos o seu rebate. No século XX a maior parte de tipos de materiais actualmente em utilização ou em desenvolvimento foram propostos para prevenção das aderências: líquidos, geles, barreiras, drogas fibrinolíticos, polímeros hidrossoluveis e polímeros orgânicos. No entanto após um período inicial de algum entusiasmo a prática não consagrou a sua utilização. On the surface are highly differentiated mesothelial cells. It is on this level that, a surgical act sets off a cascade of events that flows toward the formation of membranes and adhesions. These can be responsible for a parade of symptoms which can go from abdominal pain to intestinal occlusion. Its diagnosis is, in wide measure, confirmed on surgery since there are no diagnostic procedures available for its identification. Due to the nature of these formations we cannot see them, unless directly due to their effect. In the XX century, most of the types materials in use or under development currently considered for prevention of adhesions: liquids, gels, barriers, fibrinolytic drugs, water soluble polymers and organic polymers. However, after some initial enthusiasm, experience did not consecrate its use. We go back to assume a previous reference on the concept of “floating the intestines”. In fact if we maintain the intestines and abdominal organs in a solution that keeps them in movement, the injured peritoneal surfaces cannot adhere to form membranes and adhesions. Neste texto o autor descrevendo um caso de gestação peritoneal, apresenta uma imagem verdadeiramente notável do feto retido, durante 18 anos, na cavidade abdominal por gestação extra-uterina. Exibem-se várias aderências nesta estampa de grande rigor destas lesões. “on voit qu’il est encore recouvert de ses envelopes primitives, qui, après s’ètre appliquées etroitement sur lui, se porolongent sous forme de brides, et vont se fixer aux colons ascendant et transverse, en se continuant insensiblement avec le péritoine (…) La partie médiane et occipitale de la tête donne insertion à un grand nombre de cês brides (…) et qui vont s’insérer d’autre au cólon ascendent; elles m’ont paru organisées, car j’ai pu y constater la presénce de vaisseaux et de petits paquets graisseux de la grosseur d’une noisette (…) Tous ces prolongemens que nous retrouverons encore aux pieds AF, servaient de moyens de suspension à l’enfant dans la cavité abdominale.” Barlavento Médico 38 nes”. De facto se mantivermos os intestinos e órgãos intra-abdominais com uma solução que os mantenha com movimento não permitimos a junção de duas superfícies peritoneais lesadas que possam levar à formação de bridas. Para este fim vem sendo proposto o uso de solução utilizada na diálise que se mantém no abdómen por um período de cerca de 5 dias, antes da sua reabsorção. No texto que descrevia o caso da gestação intraperitoneal retida verificamos que há cerca de 180 anos atrás se utilizava uma nomenclatura, bridas e aderências, que se mantém actual. Por outro lado a noção de maturidade das bridas, já está igualmente presente nesse texto. Sabemos hoje que a maturidade pode condicionar a sua classificação Histológica e Macroscópica. A neovascularização que o autor tão bem assinala é factor de verificação na classificação Histológica das bridas de 17 Zuhlke . De acordo com a sua classificação Macroscópica que se refere à facilidade com que se liberta as aderências, desde uma película que se separa com facilidade por dissecção romba (grau I) ou onde a sua lise só é possível por dissecção com tesoura ou bisturi e onde a libertação do órgão previsivelmente acarreta a sua lesão (grau IV) a maturidade proporcionada pelo tempo que já levam desde a sua formação, deixa antever uma consistência que está de acordo com a sua observação:”serviam de meios de suspensão da criança na cavidade abdominal”. São lesões que tal como então ainda hoje nos confrontamos quotidianamente. Na imagem 3 verificamos a ansa do delgado atingida, depois da lise de brida unica, numa operação de Janeiro de 2007. Este é um campo que vem merecendo a atenção de muitos investigadores. Utilizando duas citações poderemos dizer, em conclusão, que nesta patologia oscilamos hoje entre uma constatação e um desejo. Por um lado “as aderências desde sempre fizeram parte da vida de um doente cirúrgico bem como do cirurgião. O cirurgião aceita a formação destas aderências como uma lesão inevitável, assim como algo que pode ser controlado tão facilmente como o tempo que fará 18 amanhã” . Por outro “as feridas convivem com a humanidade desde o seu início. A evolução na cura não pode ser deixada em paz com a moderna tecnologia. O grau de inflamação e fibrose necessária em tempos 19 pré-históricos pode ser excessiva hoje” . which remains in the cavity for a period of five days before being absorbed. In the text which described the case of the retained intra peritoneal gestation, 180 years ago, we verified the use of the nomenclature, adhesions and membranes, still used today. On the other hand, the notion of the maturity of membranes is equally present in that text. We understand today that maturity can condition its histological and macroscopic classification. In the second half of the twentieth century enormous advances were made in virtually all fields of medicine. Nevertheless, we have entered the twenty-first century and the situation concerning the gravity of the consequences of abdominal fibrosis, with its adherences and fibrotic bands, has remained practically unaltered. Neovascularization, which the author so well remarks, is a factor which is verified in the Zuhlke histological classification of membranes. In agreement with his macroscopic classification which refers to the ease with which the membranes are freed, from the membrane which is separated with ease through rhombus dissection (grade I), or its lysis only possible through dissection with scissors or scalpel blade where the freeing of the organ will provoke its lesion (grade IV). The maturity proportioned by time evolved from its formation allows the foresee all of a consistency in accordance with its observation: “the use of the means of suspension of the child in the abdominal cavity” Lesions, which just as before, still confront us today. In image 3, we verify that the small intestine segment affected, after lysis of the only membrane, in an operation in January of 2007. This is an area which has received the attention of several investigators. Citing two quotes, we could say that, in conclusion, in this pathology we swing today between acknowledgement and desire. On the one hand “adhesions have always been part of the life of the surgical patient and of the surgeon. The surgeon accepts the formation of these adhesions as inevitable, something so easily controlled as the weather 18 tomorrow” . On the other hand “wounds are familiar to humanity since the beginning. The evolution of healing has not kept up with modern technology. O que é certo é que cada vez que tenho de viajar de avião e me sento na poltrona me ocorre, será que desta vez voltarei a ouvir: “há alguma médico a bordo?” E se assim for, o que será desta vez? Voltarei a sentir aquele misto de dever e incomodidade, face ao actuar não só no ar propriamente dito, como no total desampara do que quer que seja de meios do diagnóstico à terapêutica que o hospital nos providencia? Sei que a situação não é inédita mas o facto de se ter passado em concreto comigo e de haver sempre um ensinamento e extrair, levam-me a contar este episódio onde este “há algum médico a bordo?” se fez ouvir. Janeiro 1989, saída da Faculdade de Medicina dois anos antes e ainda sem o traquejo que só a experiência nós providencia, ia gozar umas merecidas férias. Esperavamme cerca de 16 horas de voo… e, eu que sempre tive medo de voar! Do percurso faziam parte 3 paragens. A primeira em Madrid, a segunda em Frankfurt e, finalmente Banguecoque. A primeira parte decorreu dentro de uma total normalidade. Mas no trajecto Madrid – Frankfurt ouviu-se o aviso de «apertem os cintos – há ventos fortes». O avião movimenta-se com a correspondente trepidação, eu agarro-me o mais que posso à poltrona e o receio que eu sentia providenciou-me com a correspondente taquicardia. No meio do meu estado de pavor houve-se novamente a voz do piloto que pergunta: «Há algum médico a bordo? Necessitamos de ajuda». Meu Deus é comigo! Bem, levantei-me e dirigi-me para a hospedeira de bordo. Junto a uma casa de banho estava um indivíduo novo, com cerca de 40-45 anos em dificuldade respiratória e com dor torácica. Apresentava-se pálido e olhou-me com um misto de esperança e terror (ou seria o meu terror que eu via?). Pela tradução da hospedeira, percebi que era fumador e que previamente já tinha tido dois episódios de pneumotórax espontâneo. Perguntei se havia estojo de primeiros socorros a bordo ou algo que me pudessem providenciar de apoio médico. Sim havia. Mas depois mostraram-me um conjunto com umas compressas, adesivo, desinfectante e, pouco mais… E agora que fazer? Aterrar, transmitir o pânico? Não tive dificuldade sob auscultação directa com o pavilhão auricular no tórax, sentir a assimetria na propagação torácica (como foi útil a precursão com uma moeda sob a outra, recursos da velha semiologia!) do som e vibrações, para estabelecer um diagnóstico de novo pneumotorax. E de grandes dimensões. No entanto o quadro mantinha-se e agravase. Era necessário fazer algo. Tinha de arranjar algo com que perfurar o tórax. Voltei a olhar para a referida caixa e no meio do material estava uma tesoura, uma pinça e um estilete que mais parecia “Is there a Doctor onboard?” I don’t know if this happens to others or perhaps just to me. The truth is that each time I travel and sit in my seat, the thought springs to my mind, will I hear that phrase again: “Is there a doctor onboard?”. If it must be, what will it be this time? Will I feel that mixture of duty and fear of having to perform not only in the air but totally lacking any means of diagnostic or therapeutic tools which the hospital provides us with? I know that situations like this are not infrequent, but having lived through the experience and realising there is always something to learn from it, has inspired me to describe these experiences. The first situation happened in January of 1989. Having graduated from University two years before, with little experience that only time gives us, I was about to take some much deserved holidays. Awaiting me was a 16 hour flight…and I was afraid of flying! The trip entailed 3 stops. The first stop was Madrid, the second, Frankfurt and the last, Bangkok. The first leg of the trip was perfectly normal. But on the leg to Madrid-Frankfurt, heavy winds imposed a permanent fasten seat belt warning. The plane vibrated terribly as I held on tightly to my seat. My heart rate accompanied the fear that possessed me. In the middle of my state of panic, the voice of the pilot was heard asking “Is there a doctor on board? We need help”. Hoping someone else would answer, I hesitated, but no one else did. The Captain insisted. I arose from my seat and spoke to flight attendant, identifying myself as a doctor. Near the restroom was a German man, around 40-45 who was evidently in respiratory distress and complaining of thoracic pain. He was pale, looking at me with a mixture of hope and terror (or was it my terror I saw?). From the flight attendant’s translation, I understood he was a heavy smoker and had previously suffered two spontaneous pneumothoraxes. I asked for a first aid kit or something I could improvise with. The small kit I was given was made up mainly of dressings, adhesives, disinfectant and little more. Fortunately, it had a stethoscope. What to do now? Order a landing and transmit panic? Placing my ear directly to his chest, it was not difficult to hear the asymmetry of the chest sounds and vibrations (how resorting to percussion with 2 coins one top of the other, was so useful in establishing the diagnosis of a new episode of pneumothorax). It was severe and was worsening progressively. A quick decision was in order. I needed to find something to perforate the thorax with. I looked to the first aid kit and among the material was a pair of scissors, a pair of tweezers and a stiletto that looked more like a crochet hook. I hesitated, but the respiratory distress of the man no aeroporto esperando há quatro horas, quando subitamente, que as condições atmosféricas permitiam a viagem. Dirigi-me para o meu lugar no avião ainda assustada e, após a descolagem, quando o comandante anunciou que a viagem “ia ser complicada” ainda piorei. Restava-me fechar os olhos e cruzar os dedos. Passado um bocado voltamos a ouvir o comandante e a tal frase foi pronunciada:“há algum médico a bordo?” Levantei-me automaticamente. Um francês, completamente bêbado, tentava abrir uma porta de emergência. Apresentei-me, estabeleci diálogo e perguntei-lhe porque estava ele a fazer aquilo? Respondeu-me que a sua mãe morrera no dia anterior e o funeral ia decorrer em Bordéus, pelo que ele tinha de sair imediatamente. Sem grandes recursos disponíveis, ocorreu-me começar a dar-lhe água com sal, enquanto ia falando com ele. Finalmente começou a vomitar e assim continuou por cerca de um quarto de hora. Quando acabou foi altura para o sentar no seu lugar, onde se manteve, sem incomodar quem quer que fosse, até ao fim da viagem. Nessa altura verifiquei que me esquecera completamente da tempestade e das oscilações do avião. was worsening…My sense of duty impelled me to take the risk and after wiping the stiletto with alcohol, I thrust it into the thoracic cavity and felt a corresponding whistle of air leaving…and the patient began to breathe almost normally. Without something to impede the air from entering the cavity, once having relieved the pressure, I removed the stiletto and exerted pressure with a finger on the site of the puncture. I improvised with a vesicle catheter and its respective bag. We landed 15 minutes later in Abu Dhabi, the United Arab Emirates. On arrival at the airport, an ambulance was ready and waiting for us. Only after having gone “off duty”, with the patient stabilized in the ambulance, did I finally speak to my companions. I was then told that during the ordeal, which had lasted more than 30 minutes, the storm, which had started this odyssey, got even worse. I had not felt a thing and had actually quite forgotten about my fear… We continued our trip and those holidays were well spent. On our return from Bangkok, the pilot announced: “We would like to thank the Doctor who assisted a passenger of ours 11 days ago and would like her to accept a token of our appreciation”. Blushing happily, none of the passengers besides my companions realised how embarrassed I was. To my surprise, I received 6 bottles of Dom Perrignon. The Captain concluded: “the patient is well and has been discharged from the hospital”. The second situation occurred some years later. I was in Paris, preparing for my return home. When I arrived at the airport, weather conditions had deteriorated and were getting worse by the moment. We had been waiting at the airport for four hours, when suddenly there was a break in the atmospheric conditions, permitting us to take off. Frightened, I took my seat on the plane. Soon after take off, the Captain announced that the flight would be “complicated”, my fear worsened! I closed my eyes and crossed my fingers. Short moments later, I could hear the voice of the Captain again with that phrase “Is there a Doctor onboard?”. I once again signalled my presence. A Frenchman, who was completely drunk, was trying to open an emergency exit door. I introduced myself, established a dialogue and asked him why he was doing that. He responded that his mother had died the day before and that the funeral was to be in Bordeaux, so he needed to leave immediately. Without any medication, it occurred to me to start him on an oral saline solution with water and salt as I spoke to him. When he finally began to vomit, he did so for at least a quarter of an hour… When he finished, he went back to his seat where he remained for the rest of the flight without anymore disturbance. Back “off duty” and approaching our destination, I realised that I had once again missed the storm and the heavy turbulence affecting the plane! 41 Barlavento Médico uma agulha de crochet. Arrisco, não arrisco? E o doente a piorar. Arrisquei, meti o estilete na cavidade torácica, senti um silvar da correspondente saída de ar … e o doente começou a respirar. Sem dispositivo que impedisse a entrada do ar na cavidade torácica, uma vez saída a hiperpressão pleural, retirei o estilete e exerci pressão com o dedo no local da perfuração. Felizmente aterrámos cerca de 15m depois. No aeroporto já tínhamos uma ambulância e um Colega à espera do doente. Só então, após “passagem de turno” e o doente seguir estabilizado na ambulância, falando com os companheiros de viagem, soube que toda esta situação tinha durado mais de 30 minutos e ainda que a tempestade onde iniciara esta odisseia ainda tinha piorado. E eu que não dera por nada e inclusivamente, à posteriori, acho mesmo que me esqueci que ia com medo … Continuámos viagem, foram umas férias bem passadas. No regresso de Banguecoque, 11 dias depois, anúncio do piloto: “Queremos agradecer à médica que assistiu um passageiro nosso há 11 dias atrás e gostaríamos que aceitasse uma lembrança nossa”. Corei, felizmente tirando os meus companheiros ninguém deve ter dado conta do meu embaraço. Recebi seis garrafas de champanhe Dom Perrignon. O comandante concluiu: “o doente está bem e já teve alta hospitalar”. Alguns anos depois, estava em Paris e preparava-me para regressar a casa. Quando cheguei ao aeroporto as condições atmosféricas estavam como nunca as viram anteriormente e a cada segundo pioravam. Naturalmente que estava assustada. Alguns casos comportam reais melhorias quer na acuidade diagnostica quer na quantidade e rapidez da realização dos estudos. No que respeita às inovações tecnológicas nas diferentes técnicas imagiológicas, respectivos equipamentos e aportes ao exercício diagnóstico assistimos no Congresso Europeu de Radiologia de 2006, à consolidação de perspectivas e algumas promessas já avançadas em anos progressos. IMAGIOLOGIA RADIOLÓGICA PLANAR Barlavento Médico 42 (Radiologia Convencional) Os sistemas convencionais de aquisição que obtêm a imagem radiológica planar mediante a exposição de conjuntos écran/ película fotossensíveis (ou intensificadores de imagem acoplados a écrans de televisão) a fotões de alta energia gerados pelas ampolas de RX, após terem atravessado o corpo humano, estão progressivamente a ser substituídos por Sistemas Digitais cuja base é constituída por écrans de Detectores de Fósforo foto estimuláveis (BaFBr, e BaFI). Nestes últimos distinguem-se Sistemas Directos cujos detectores são directamente sensíveis aos fotões de alta energia e Sistemas Indirectos cujos detectores são sensíveis à luz emitida por finas camadas de material fluorescente acopladas aos detectores. Os Detectores produzem sinais eléctricos que são exibidos em monitores de TV e arquivados em formato digital. Os defensores da Radiologia Digital que se desenvolveu a par do Sistema PACS (Sistema de Arquivo Digital de Imagens) advogam que embora a implementação inicial seja onerosa, têm vantagens estabelecidas nomeadamente: a. Maior e mais rápido fluxo de trabalho b. Melhor relação dose de radiação / eficácia, fornecendo imagens de qualidade superior c. Separação dos processos de aquisição, processamento (ajuste de janela, reforço de contornos e redução de ruído) e exibição de imagens, permitindo a optimização separada de cada uma deles. d. Benefícios do arquivo digital das imagens com leitura, consulta e transferência facilitada de imagens (inclusive à distância - Teleradiologia). Na área da Senologia este tipo de Sistemas de Detecção Digital aperfeiçoados permitirá a transição do registo das imagens de Mamografia, em película, para a Mamografia Digital. ECOGRAFIA (Ultrassonografia) No campo da ecografia os progressos avançam em diferentes aplicações: a) A miniaturização das sondas e ecógrafos permite a construção de aparelhos com funções cada vez mais versáteis, comparáveis a um computador portátil ao qual se ligam as sondas – permitindo a portabilidade do equipamento nas instituições e entre instituições. b)Nas versões de consola os avanços baseiam-se na optimização da utilização das componentes harmónicas dos ultrassons (US) que viabilizam e potenciam a utilização dos contrastes em Ecografia. Estes são fluidos com componentes de microbolhas de gás por vezes ligados a um elemento com alvo tecidular que ao serem destruídos por um “pulso de US” revela a existência de estruturas hipervascularizadas – aumentando a sensibilidade da técnica na detecção de lesões, nomeadamente tu- morais primitivas ou metastáticas. Imagiology in digital era News from the European Congress of Radiology Medical systems particularly those dedicated to diagnostic imaging are subject to a fast process of renovation and innovation. In some situations it brings true improvements in diagnostic accuracy and workflow. With respect to technological innovatios in technical imaging the European Congress of Radiology 2006 confirmed promises and expectations forseen earlier by researchers and industry . CONVENTIONAL RADIOLOGY Emphasis should be put on digital acquisition and reading systems. Conventional acquisition systems obtain images through the exposure of a screen film (or image intensifier coupled with a TV screen) to X rays after crossing a region of interest of the human body. These systems are progressively being substituted by digital ones which are based on detectors of phosphorous (BaFBr and BaFi). There are two kinds of digital detector systems: Direct systems whose detectors are directly sensitive to high energy photons and indirect systems whose detectors are sensitive to light generated from fluorescent material coupled to the phosphorous plates. These detectors produce electrical signals, which after amplification will be displayed on TV monitors and recorded on a digital format. Defenders of Digital Radiology which developed along with PACS system claim that although initial financial investment is great the practical benefits are clearly established namely: a. Bigger and faster workflow b. Radiation dose/effectiveness ratio optimization, resulting in better quality images c. Separation of acquisition, processing (window borders and noise reduction adjustments), and displaying procedures, allowing optimization of each of them. d. Benefits of digital archiving of images and reports, such as accessibility, reviewing possibilities, and transference inclusive to and from far away places- Teleradiology. Also in the field of breast pathology, new digital detectors with better quality and resolution will allow the transition from conventional to digital mammography to become reality. ULTRASSONOGRAPHY Progress within this modality is being done with different applications: a) Miniaturization of probes and equipments has made possible the construction of new, more versatile systems, comparable to portable computers to which probes are attached through doors allowing portability inside institutions and between institutions. b) Console versions developments are being accomplished through optimization of the use of the harmonic components of ultrasound US, which in turn allows the progressive use of US, contrasts in more complex applications. US contrasts are fluids with gas bubble components, which lately were improved in their capability to attach to specific target components of tissues. RESSONÂNCIA MAGNÉTICA MAGNETIC RESONANCE IMAGING We will emphasize the following sub modalities of MRI: AngioMR (a competitor of AngioCT) and Diffusion MR, which gives information on the vascular supply and the flow direction, information that is useful for modern studies of Vascular Pathology, namely for evaluating the “penumbra areas”, and orienting the therapy of Isquemic Stroke with trombolytic pharmacologic agents. Their contribution is also important for the study of tumours blood supply. Spectroscopic MR and Chemical Shift MR which give biochemical information on cellular metabolism, through analysis of concentration and consumption of glucose in tiny compartments of tissue. Combining this information with High Resolution MR, sensitivity and specificity in the detection of tumours can be fairly increased. Functional MR uses fast radiofrequency pulses to determine the flow of oxygen and consequently inferring areas of metabolic cellular activity (e.g. neuronal) potentially pathologic. This method is complementary to Spectroscopic MR and PET. Isto é, mudou entre nós a forma como se inicia, após o curso, a prática da medicina e se faz o acesso à especialidade. Mudou aquela que vinha sendo a forma com se preparam todos aqueles que hoje são Médicos Especialistas no nosso país nas mais diversas áreas. A questão começara a colocar-se em 1985 quando Portugal aderiu à Comunidade Europeia. Informáramos então a Europa que para se ser médico com autonomia em Portugal eram necessários, após o curso numa Faculdade de Medicina, mais 2 anos – o designado Internato Geral – onde o médico recém formado se preparava para uma prática progressivamente autónoma e ao mesmo tempo fazia a sua preparação estudando para o exame de acesso à Especialidade. Era um exame escrito que não atribuía ou excluía qualquer competência mas que na prática, permitia fazer um ranking onde os alunos com melhor nota teriam as primeiras escolhas para as vagas de Especialidade que abriam nesse ano. Logo então alguma perplexidade se gerou já que, por exemplo, em Espanha este exercício autónomo para a prática da Medicina existia logo à saída das Faculdades. Progressivamente assistimos à vinda em número crescente de Colegas dos nossos países vizinhos para o exame de acesso à vaga de Especialidade logo à saída da Faculdade, enquanto os nossos médicos recém formados tinham ainda de esperar mais dois anos para poder candidatar-se a esse exame. Esta questão, bem como outras de práticas diferentes na Europa que a Comissão Europeia procurava harmonizar, criou a noção clara da necessidade de reformular o Internato Geral. No entanto tínhamos todos nós, médicos, consciência da importância deste Internato Geral na nossa formação, na qualificação do nosso desempenho. E, por outro lado, face à turbulência mundial dos tempos em que vivemos, onde o poder politico nem sempre tem consciência de onde está a mexer, mas é sempre lesto a legislar, poder-se-ia perder esta etapa verdadeiramente importante na nossa progressão profissional. Foi sem surpresa que líamos há cerca de três anos, numa das revistas de grande informação ao fim-desemana, um dossier onde se fazia um estudo acerca do que estava mal na saúde em Portugal. Nesse trabalho o factor negativo mais relevante identificado, foi a desorganização dos Serviços e - para grande espanto do economista autor do estudo - o factor mais positivo foi a qualidade do desempenho médico. Internato esse que beneficiava o candidato à Especialidade numa diversidade e quantidade de actos médicos praticados na altura certa do seu desenvolvimento clínico. Quantos de nós chegados a certos Hospitais, normalmente os mais periféricos, passados alguns dias estávamos como 1º ajudante num acto operatório e posteriormente a ler matérias que depois não tinham nada a ver com o que seria motivo de avaliação em exame de acesso à Especialidade? Mas não foi este um tempo da progressão que dotava o jovem médico com uma globalidade de visão importante na Medicina? Na prática o Internato Geral estava inscrito na matriz, na identidade, do médico em Portugal e conferia-lhe uma competência acrescida. Por tudo isto era com apreensão que aguardávamos o desenvolvimento desta questão, com medo que para se resolver um problema se criasse outro. Isto é, que se acabasse com o Internato Geral, contribuindo para um menor desempenho nos actos globais a que nos habituáramos e que não houvesse nada onde repor esses elementos. Mas este internato acabou e temos agora o Ano Comum. E vem tornar mais claro qual o caminho para onde nos dirigimos. Agora para a formação numa Especialidade o candidato passa inicialmente por um Ano Comum e entra posteriormente na Formação Especifica. Não é pelo facto de, por exemplo, se ir optar por uma especialidade como Cardiologista que este candidato não deva saber quais os procedimentos próprios de um Bloco Operatório. Procedimentos e actos que sem terem que ver directamente com uma ou outra especialidade, têm a ver com Medicina de uma maneira geral e a familiaridade aos médicos para com esses gestos, deve estar assegurada. São estas apropriações mais “básicas” que este ano, este Ano Comum, permitirá a todos os médicos desempenharem, independentemente da sua posterior opção para Formação Especifica. Pensamos que houve Sabedoria na maneira como se procedeu a esta alteração. Há que, em justiça, creditá-la à Ordem dos Médicos e ao Ministério da Saúde. Existe ainda outra vantagem que verificamos. Ao fazer o exame de acesso à especialidade coincidir com a saída da Faculdade e só sendo a nota e o número de vagas conhecidas no final do Ano Comum, na altura de escolha da opção pela Formação Especifica, este tempo decorrerá com uma tranquilidade que permite antever um período altamente frutuoso nesta passagem por Serviços com Medicina Interna, Cirurgia, Pediatria, Obstetrícia e Clínica Geral. Isto é, sem condicionamentos de uma escolha já confirmada e onde alguma valência estaria já de parte ou por um exame a preparar em simultâneo à colheita das experiências de um primeiro ano de prática clínica. Vílchez Fraga, Joel Guerreiro and Jose Parra Martín). The Prize was awarded for results presented in the area of breast tumours and establishes good practice in dynamic investigation in medicine that a tumour bank can supply. The award ceremony took place during the Experimental Techniques Symposium on the 2nd and 3rd of November of 2007 on the Gambelas University Campus in Faro This Symposium was organized by the University of the Algarve (Ualg) in partnership with the Portuguese Association of Experimental Pathology (APPE) bringing together a group of medical professionals, veterinarians, biologists, among others. Publica: Editoriais, Artigos de Revisão, Artigos de Investigação, Artigos de Opinião, Casos Clínicos, Comunicações breves, Historia da Medicina, Organização do Internato Médico, Novidades, Internet, Revisões de Livros, trabalhos originais de descrição e interpretação de investigação tanto na Clínica como na Patologia Experimental ou nas Ciências Básicas, ou de reflexão a partir da prática médica. Em caso de dúvida submeta o trabalho para apreciação. Informação aos autores | Guidelines for authors PREPARAÇÃO DO MANUSCRITO MANUSCRIPT PREPARATION As seguintes regras para formatação dos manuscritos aplicam-se a todos os originais a submeter para publicação que serão sujeitos posteriormente a tratamento editorial: Carta (cover letter/Disclosure of Interest) que acompanha o artigo e onde se pede a sua publicação deve fornecer (1) A rubrica onde o manuscrito se inscreve (ie, comunicação original, caso clínico, etc.); (2) uma declaração onde se expresse que o material não foi publicado previamente ou submetido em simultâneo para publicação noutro local e que não o será, até o autor receber a decisão do BARLAVENTO MÉDICO acerca da sua publicação; (3) informação acerca de algum conflito de interesses que os autores possam ter; e (4) fontes de financiamento do trabalho. A página de título deve incluir o nome completo e maior grau de cada autor, a instituição (ou instituições) onde o trabalho deve ser atribuído, endereço, telefone, fax e endereço de e-mail do autor responsável pela correspondência. Sumário e Palavras-chave. Um resumo estruturado de cerca de 250 palavras deve acompanhar o manuscrito e consiste em quatro parágrafos: Introdução (declarando o propósito do estudo), Métodos, Resultados, e Conclusões. Sumários estruturados não são necessário para Revisões Clínicas, Casos Clínicos onde se deve apresentar um sumário que forneça o contexto para o trabalho e realce novos aspectos do estudo ou observação, em cerca de 200 palavras. A seguir ao resumo deve fornecer-se 3 a 10 palavras-chave. As ilustrações podem ser submetidas electronicamente ou em cópia. Quando submeter as ilustrações electronicamente, use formato de TIF. Se não poder submeter as figuras electronicamente, envia 2 cópias ao Editor. As ilustrações só serão reenviadas se os autores especificamente assim o solicitarem. Uma legenda deve acompanhar qualquer figura ou ilustração. Quadros devem ser numeradas na ordem em que são mencionados no texto e apresentar um título descritivo breve. As referências devem ser citadas por ordem de aparecimento no texto, usando-se números árabes. O uso de abreviações deve estar com as do Index Medicus. Video clips. Vídeos que acompanhem um manuscrito devem ser limitado, a não mais de um minuto e para não mais de 5 MB de arquivo. Se aceites, serão publicados no site da revista. Os vídeos devem ser submetidos em QuickTime ou formato de MPEG. Os artigos aparecem em versão impressa e online da revista, devendo os autores assegurar ambos os direitos para as imagens a reproduzir. Caso Clínico inclui o estudo de até 4 casos onde se descreve uma nova situação ou se avance uma informação relevante no diagnóstico ou tratamento de uma patologia. Os manuscritos para Imagens em Medicina e Comunicações breves, devem ser limitados até quatro páginas a dois espaços (times new roman 12 pts), até cinco referências. Meeting Reports são sinopses de reuniões científicas de interesse potencial para os leitores, submetidos até 12 semanas da reunião. Noticias são comunicações breves de Congressos, Seminários, Cursos ou outras acções onde se transmite só o essencial da informação e onde se mantenha a actualidade. Correspondência. Os Editores convidam os leitores a que expressem diferentes opiniões relativamente ao editorais ou outros artigos publicados na revista. Reservam-mo-nos o direito de aceitar, rejeitar ou publicarem parcialmente essas cartas respeitando os pontos de vista expressos. Selecções da internet são textos sintéticos de artigos recentemente publicados noutras publicações com interesse potencial para os nossos leitores. Contribuições não solicitadas podem ser submetidas para apreciação. Book reviews. Rubrica dependente de pedidos de apreciação pelos Editores. BARLAVENTO MÉDICO reserva o direito para editar, rever ou condensar as colaborações para adequação à identidade da publicação. As provas de tipografia com o texto editado serão enviadas aos autores para verificação prévia à sua publicação. Todas as alterações devem ser feitas nessas provas. Separatas. The following format must be used for all submitted manuscripts, and will be subject to copy editing before publication: The cover letter/Disclosure of Interest Form should provide (1) the category of manuscript (ie, original communication, case report, etc); (2) names of sources of outside support for research; (3) information about any personal conflicts of interest of any of the authors; and (4) statement that the material has not been previously published or submitted elsewhere for publication and will not be sent to another journal until a decision is made concerning publication by BARLAVENTO MÉDICO. The title page should include the full name and highest achieved degree of each author, institution(s) to which the work should be attributed, and complete address, telephone numbers, e-mail address, and fax number of the one author who will be responsible for correspondence. Abstract and Key Words. A structured abstract of no more than 300 words must accompany the manuscript and consist of four paragraphs, each with an introductory label: Background (stating the purpose of the study), Methods, Results, and Conclusions. Structured abstracts are not necessary for Clinical Reviews, Brief Clinical Reports or Case Reports, were it should provide the context for the subject and emphasizes new aspects of the study or observation in no more than 200 words. Following the abstract should be provide 3 to 10 key words. Illustrations can be submitted electronically or in hard copy. When submitting the illustrations electronically, please use TIF format. If you are unable to submit the figures electronically, please send 2 copies to the Editor. Illustrations will only be returned if the authors specifically request that they be returned. Subtitles must be provided for all illustrations. Tables should be numbered in the order in which they are mentioned in the text, and given a brief, descriptive title. Cite references in order of appearance in text using Arabic numerals. Use of abbreviations in conformity to those used in Index Medicus. Video clips. Video clips accompanying a manuscript must be limited, at present, to no more than one minute in length and no more than 5 MB in file size. If accepted, they will be published on the Journal Web site. Videos must be submitted in either a QuickTime or MPEG format. Articles appear in both printed and online versions of the journal. Failure to get electronic permission rights may result in the images not appearing in the online version. Case Reports include case studies of 4 or fewer patients that describe a novel situation or add important insights into mechanisms or diagnosis or treatment of a disease. Manuscripts for the Brief Communications, and Images in Medicine sections should be limited to no more than four double-spaced manuscript pages (times new roman 12 pts) with no more than five references. Meeting Reports are synopses of scientific meetings of potential interest to readers submitted within 12 weeks of the meeting. Notices are brief announcements of meetings, seminars, and courses with only essential information given. Correspondence. The Editors invite comments in the form of letters or Perspectives that express differences of opinion or supporting views of previously recently published papers in the journal. The Editorial Board reserves the right to accept, reject, or excerpt letters without changing the views expressed. Selections from internet are concise overviews of articles recently published in other journals that are of potential interest to BARLAVENTO MÉDICO readers. Unsolicited contributions may be submitted to the Editor for consideration. Book reviews. Books shall be reviewed only at the discretion of the Editors. BARLAVENTO MÉDICO reserves the right to edit, revise and abridge contributions to conform to our editorial standards and style. Print proofs of the edited text, are sent back to the author for approval before publication. All corrections should be marked clearly directly on page proofs. Reprints. Once the authors, as well as all interested can download articles from our website, the Journal does not provide reprints.
