SPINCARD hCG Spincard hCG One step - Cassette Ensaio imunocromatográfico One Step para a detecção qualitativa de gravidez (hCG) no soro e/ou urina IVD Conservar a 2-30ºC USO RECOMENDADO O sistema utilizado neste teste é um imunoensaio em fase sólida para a detecção qualitativa rápida de gonadotrofina coriónica no soro e /ou urina humana. O teste destina-se unicamente para diagnóstico in vitro profissional. O teste usa-se únicamente para obter um resultado preliminar Em qualquer caso, o resultado deve ser interpretado por um profissional, especialmente ao avaliar um resultado preliminar positivo. Para confirmar os resultados analíticos, é necessário um método alternativo mais específico. SIGNIFICADO CLÍNICO A gonadotrofina coriónica humana (hCG) é uma hormona produzida pela placenta de mulheres grávidas. É detectável, tanto na urina como no soro, 7 a 10 dias após a concepção, pelo que faz dela o indicador ideal de gravidez. PRINCIPIO DO MÉTODO O SPINCARD hCG é um imunoensaio por cromatografia de fluxo lateral. O método emprega uma combinação de conjugado anticorpo monoclonal-corante e anticorpos policlonais em fase sólida para identificar selectivamente a hCG presente nas amostras, com um elevado grau de sensibilidade. Em menos de 5 minutos, podem detectar-se níveis de hCG tão baixos como 20 mIU/ml. Quando se aplica uma quantidade suficiente de amostra na placa, esta migra por capilaridade através da tira. O conjugado anticorpo-corante une-se à hCG formando um complexo anticorpo-antigénio. Este complexo une-se ao anticorpo anti-hCG da zona Teste produzindo uma banda corada rosa quando a concentração de hCG é superior a 20 mIU/ml. Na ausência de hCG, não se observa banda nenhuma na zona teste. A mistura de reacção continua a migração através da tira até à zona controlo. O conjugado livre ainda se une aos reagentes da zona controlo formando uma banda corada rosa e demostrando o correcto funcionamento do teste. REAGENTES E MATERIAL INCLUÍDOS 10 ou 25 placas embaladas com um conta-gotas MATERIAL NECESSÁRIO NÃO FORNECIDO Contentores para a recolha de amostra Cronómetro CONSERVAÇÃO E ESTABILIDADE Conservar o kit à temperatura ambiente 2-30ºC. Conservar cada placa no seu invólucro original selado. Cada placa pode ser usada até à data de validade indicada na etiqueta, se conservada sem ser aberta, na embalagem original. Não congelar nem expor a temperaturas superiores a 30°C (86°F). RECOLHA DA AMOSTRA a) Soro 1. A amostra de soro deve ser recolhida em condições de laboratório standardizadas (assépticamente de tal modo que se evite a hemólise). 2. Não se requer centrifugação nem filtração do soro. 3. Se o teste for realizado nas 48 horas após a recolha, a amostra debe ser mantida refrigerada (2-8°C). 4. Se o teste se destinar a ser feito 48 horas após a recolha, a amostra deve congelar-se a – 20 ºC. Antes de a testar, a amostra deve ser completamente descongelada, perfeitamente misturada temperada. Evitar congelar e descongelar repetidamente. 5. Em caso de turvação, elevada viscosidade ou presença de partículas na amostra de soro, deve diluir-se com o mesmo volume de tampão diluente (V/V) antes de realizar o ensaio. b) Urina 1. Para a detecção óptima de gravidez numa fase inicial, é preferível recolher a primeira urina da manhã, já que esta contém a concentração máxima de hCG. No entanto, podem usar-se amostras de urina recolhidas aleatóriamente. 2. Deve recolher-se a urina num recipiente limpo. 3. Se o teste não for realizado imediatamente, a amostra deve manter-se refrigerada (2°C – 8°C) ou num ambiente fresco (inferior a 25°C) até 24 horas. Nesse caso, temperar a amostra antes de realizar o teste. 4. Se o teste se atrasar mais de 24 horas, a amostra deve ser congelada a – 20 ºC. Antes de a testar, a amostra deve ser completamente descongelada, perfeitamente misturada e temperada. Evitar congelar e descongelar repetidamente. PRECAUÇÕES 1.As instruções incluídas no kit devem ser seguidas para a obtenção de resultados fiáveis. 2.Não abrir a embalagem contendo a placa até ao momento de realizar o teste. 3.Não usar testes caducados. Não utilizar testes com o invólucro rompido. 4. Manipular todas as amostras e material utilizado como potencialmente infeccioso. PROCEDIMENTO 1.Temperar a amostra e os outros materiais necessários para o teste, antes de realizar o ensaio. 2. Extrair a placa do envólucro e colocá-la sobre uma superfície plana e limpa. 3.Identificar cada placa com os dados da amostra. 4. Encher o conta-gotas com a amostra de soro / urina. Mantendo-o na vertical, dispensar 3 gotas no poço da amostra (100µl sem bolhas de ar). 5.Esperar até que apareçam linhas coradas. 6.Ler os resultados decorridos 3 minutos quando se trata de amostras de urina e 5 minutos quando se trata de amostras de soro. NOTA: Uma concentração baixa de hCG pode dar um resultado em forma de linha débil na zona de teste (T). LIMITAÇÕES DO PROCEDIMENTO 1. O teste destina-se apenas a diagnóstico in vitro profissional. 2. Os resultados obtidos com este kit proporcionam um resultado preliminar qualitativo, não podendo ser considerados concludentes. Um resultado positivo deve confirmar-se com técnicas alternativas mais específicas. 3. Este produto está desenhado apenas para a análise de urina / soro humanos. 4. Também se tem detectado a presença de hCG em pacientes com patologia trofoblástica gestacional e não-gestacional. Como a hCG de neoplasmas trofoblásticos é similar à encontrada durante a gravidez, estas condições, que incluem um coriocarcinoma e mola hidatiforme, devem ser excluídas antes de ser feito o diagnóstico de gravidez. 5. Uma gravidez normal pode ser distinguida de uma gravidez ectópica baseando-se únicamente nos níveis de hCG. Também, os abortos espontâneos podem criar confusão durante a interpretação dos resultados. OSIS01-P 28/02/13 6. Gravidezes muito precoces, contendo concentrações extremamente baixas de hCG podem dar resultados negativos. Nesse caso, deverá obter-se outra amostra pelo menos 48 horas mais tarde e testá-la. 7. Os níveis de hCG podem permanecer detectáveis várias semanas nassecreções normais, secreção por cesariana, abortos espontâneos ou terapêuticos. 8. Algumas amostras de soro humano com concentrações elevadas de Factor Reumatóide (RF), heterofílias ou anticorpos Forssman podem dar resultados positivos não específicos. Tais casos devem ser discriminardos antes de realizar o ensaio. 9. A presença de hidroxietil-celulose na composição de lubrificantes de catéteres pode dar lugar a falsos resultados positivos no SPINCARD hCG, em concentrações iguais ou superiores a 0.1%. CONTROLO DE QUALIDADE * Controlo de Qualidade interno O teste contém um controlo de qualidade interno, a linha C. A presença desta linha indica que se utilizou o volume correcto de amostra e que os reagentes migraram correctamente. Se a linha C não aparece, o teste deve ser considerado não válido. Neste caso, é aconselhável rever as instruções e repetir o teste com uma nova placa. * Controlo de Qualidade externo O utilizador deverá seguir a normativa de controlo de qualidade própria de cada localidade, região ou país. INTERPRETAÇÃO DE RESULTADOS Negativo: Se aparece unicamente uma banda corada na zona de controlo. Positivo: Se, junto à banda de controlo aparece outra banda claramente distinta na zona teste. Qualquer resultado positivo deve ser confirmado com um método mais específico antes de dar uma determinação positiva. NEGATIVO POSITIVO Não válido: Se não aparece bem visivel a linha de controlo. Pode dever-se a uma quantidade insuficiente de amostra ou então que se tenha aplicado a técnica incorrectamente. Recomenda-se a repetição do teste com uma nova placa e obter uma amostra fresca e testá-la 48 horas depois. CARACTERÍSTICAS DO MÉTODO 1. Exactidão: Realizou-se um estudo com um total de 159 amostras de urina positivas e negativas. Estas amostras foram testadas de aocrdo com os procedimentos, no SPINCARD hCG e outro método comercial. Especificidade Reacções cruzadas: As seguintes concentrações de hormonas homólogas à hCG não apresentaram reactividade cruzada com o SPINCARD hCG: hTSH 1,000 uIU/ml WHO 68/38 hLH 300 mIU/ml WHO 2nd IS 80/552 hFSH 1,000 mIU/ml WHO 1st IS 83/575 3. Interferências: Foram adicionadas substâncias potencialmente interferentes a amostras hCG positivas e negativas. Em cada caso, não se observaram interferências nos resultados esperados com o SPINCARD hCG. O SPINCARD hCG consegue detectar niveis de hCG de 20 mIU/ml de acordo com WHO 4 International Standard. Ocasionalmente, amostras de urina e soro com níveis inferiores a 20 mIU/ml também dão resultados positivos. 2. Amostras com níveis elevados de hCG (1,000,000 mIU/ml) resultavam sistemáticamente positivas. 3. Urinas de homens sãos e mulheres não grávidas apresentaram normalmente níveis de hCG não detectáveis com o SPINCARD hCG. Embora os níveis de hCG sejam variáveis em mulheres com gravidez precoce normal, o SPINCARD hCG normalmente é capaz de confirmar a gravidez desde o primeiro dia de atraso do período menstrual. It is for health care professional use only. This assay provides only a preliminary result. Clinical consideration and professional judgment must be applied, particularly when preliminary positive results are evaluated. A more specific alternate method must be used in order to obtain a confirmed analytical result. SUMMARY AND EXPLANATION OF THE TEST Human Chorionic Gonadotropin (hCG) is a hormone produced by the placental tissue of pregnant women. It may become detectable in both urine and serum as early as 7 to 10 days after conception, which make it an ideal indicator of pregnancy. PRINCIPLE OF THE METHOD This assay is a one-step lateral flow chromatographic immunoassay. The method employs a combination of monoclonal-dye conjugate and polyclonal-solid phase antibodies to selectively identify hCG in the test samples with a high degree of sensitivity. In less than 5 minutes, levels of hCG as low as 20 mIU/ml can be detected. As the test sample flows through the absorbent device, the labelled antibody-dye conjugate binds to the hCG forming an antibody-antigen complex. This complex binds to the anti-hCG antibody in the test zone and produces a pink-rose colour band when hCG concentration is higher than 20 mIU/ml. In the absence of hCG, there is no line in the test zone. The reaction mixture continues flowing through the absorbent device past the test zone and control zone. Unbound conjugate binds to the reagents in the control zone, producing a pink-rose colour band, demonstrating that the reagents are functioning correctly. 6. A very early pregnancy containing an extremely low concentration of hCG can give a negative result. In this case, another specimen should be obtained at least 48 hours later and tested. 7. hCG levels may remain detectable for several weeks after normal delivery, delivery by caesarean section, spontaneous abortion, or therapeutic abortion. 8. Some serum specimens with high concentration of rheumatoid factor (RF), heterophiles or Forssman antibodies may yield non specific positive results during testing. Such cases should be discriminated before testing. 9. The presence of hydroxyethyl-cellulose in the composition of catheter lubricant may give false positive results with SPINCARD hCG at a concentration equal or higher than 0.1%. QUALITY CONTROL * Internal Quality Control This test contains a built-in control feature, the C line. The presence of the C line indicates that an adequate sample volume was used and that the reagents migrated properly. If a C line does not form, the test is considered invalid. In this case, review the whole procedure and repeat the testing with a new device. * External Quality Control Users should always follow the appropriate federal, state, and local guidelines concerning the running of external quality controls. INTERPRETATION OF RESULTS Negative: One coloured band appears on the control zone. Positive: In addition to the control band a clearly distinguishable band also appears on the test zone indicating a positive result. Samples with positive results should be confirmed with a more specific method before a positive determination is made. REAGENTS AND MATERIAL SUPPLIED 10 or 25 test devices, each sealed in a pouch with a dropper pipette. MATERIAL REQUIRED BUT NOT PROVIDED Specimen collection containers Timer STORAGE AND STABILITY Store the kit at room temperature 2-30°C. Each device may be used until the expiration date printed on the label if it remains sealed in its foil pouch. Do not freeze and /or expose the kit to temperatures over 30°C (86°F). SPECIMEN COLLECTION a) Serum 1. The serum specimen should be collected under the standard laboratory conditions (aseptically in such a way as to avoid haemolysis). 2. No centrifugation or filtration of serum is required. 3. If the test is to be run within 48 hours after collection, the specimen should be stored in the refrigerator (2-8°C). 4. If testing is delayed for more than 48 hours, the specimen should be frozen( -20ºc). The frozen specimen must be completely thawed, thoroughly mixed and brought to room temperature prior to testing. Avoid repeated freezing and thawing. 5. In the case of cloudiness, high viscosity or presence of particulate matter in the serum specimen, it should be diluted with equal volume (V/V) of diluting buffer (not provided but available upon request) before testing. b) Urine 1. For optimal detection of early pregnancy, a first morning urine specimen is preferred since it contains the highest concentration of hCG. However, randomly collected urine specimens may be used. 2. Collect the urine specimen in a clean container without preservatives. 3. If testing is not immediate, the specimen should be refrigerated (2°C – 8°C) or kept cool (below 25°C) for up to 24 hours. In such case bring the specimen to room temperature prior to testing. 4. If testing is delayed more than 24 hours, the specimen should be frozen (-20ºc). The frozen specimen must be completely thawed and brought to room temperature prior to testing. Avoid repeated freezing and thawing. NEGATIVE PERFORMANCE CHARACTERISTICS 1. Accuracy A study was performed using a total of 159 positive and negative urine specimen. These specimen were assayed with SPINCARD hCG and a different comercial method according to the given procedures. Specificity Cross reactivity : The following concentration of homologous hormones are found to have no interferences with SPINCARD hCG: hTSH 1,000 uIU/ml WHO 68/38 hLH 300 mIU/ml WHO 2nd IS 80/552 hFSH 1,000 mIU/ml WHO 1st IS 83/575 3.Interferences : Potentially interfering substances were added to urine which had hCG levels of 0 and 50 mIU/ml. The instructions must be followed to obtain accurate results. 2. Do not open the sealed pouch, unless ready to conduct the assay. 3. Do not use expired devices. 4. Do not use a test from a damaged protective wrapper. 5. Dispose of all specimens and used assay materials as potentially biohazardous. ASSAY PROCEDURE 1. Refrigerated specimens and other test materials, including devices, must be equilibrated to room temperature before testing. 2. Remove the reaction device from its protective wrapper by tearing along the split. 3. Label the device with specimen identification. 4. Fill the serum/urine dropper with specimen and by holding it vertically, dispense drop-wise into sample well. Add exactly 3 drops (100µl without air bubble) of serum/urine in the sample well. 5. Read the results at 3 minutes when testing a urine specimen, or at 5 minutes when testing a serum specimen. Note: A low concentration f hCG might result on a weak line in the test result region (T) after extended period of time. LIMITATIONS 1. This test is for professional in vitro diagnostic use only. 2. Results obtained by this device provide only a preliminary qualitative result. A more specific alternate method must be used in order to obtain a confirmed result. 3. This product is designed for testing human serum/urine only. 4. In addition to pregnancy, hCG has been found in patients with both gestational and nongestational trophoblastic disease. Since the hCG of trophoblastic neoplasms is similar to that found in pregnancy, these conditions, which include choriocarcinoma and hydatidiform mole, should be ruled out before a diagnosis of pregnancy is reached. 5. A normal pregnancy can be distinguished from an ectopic pregnancy based on hCG levels alone. Also, spontaneous miscarriage may cause confusion in interpreting test results. Sensitivity th 1. SPINCARD hCG is capable of detecting hCG levels of 20 mIU/ml according to WHO 4 International Standard. Occasionally specimen containing less than 20 mIU/ml for urine and serum may also yield the positive results. 2. Specimen containing high levels of hCG (1,000,000 mIU/ml) when tested consistently gave positive results. 3. Urine from healthy men and non-pregnant women will normally show undetectable levels of hCG when tested on SPINCARD hCG. Although the hCG levels in normal early pregnant women are variable, the SPINCARD hCG usually capable of confirming pregnancy by the first day of the missed menstrual period. REFERENCES 1.
