TACHYARRHYTHYMIC EVENTS WITH SYNCOPE EVENTOS TAQUIARRÍTMICOS COM SÍNCOPE Andrés Ricardo Pérez Riera MD Case of Dr. Raimundo Barbosa de Barros CLINICAL CASE - CASO CLINICO EMERGENCY ROOM MESSEJANA’S HOSPITAL EMERGÊNCIA DO HOSPITAL DE MESSEJANA 47 years old, man, farmer refers several episodes of prolonged retrosternal pain initiated 5 months ago precipitated by efforts, and that relieves with the repose. Additionally, in this period, he presented four more intense episodes followed by syncope. He also refers prolonged retrosternal pain with irradiation to the jaw that lasted about 12h, evolving to syncope episode during the travel to the hospital. He gave entrance in unconscious state to emergency room. Concomitantly, the monitor screen shows sustained regular tachycardia quickly reverted with cardioversion procedure (200j). He immediately reverted to sinus rhythm recovering totally the conscience with relief of precordial pain. -----------------------------------------------------------------------------------------------------------------------------Homem, 47 anos, agricultor refere que vem há 5 meses com episódios de dor retroesternal, precipitados pelo esforço, prolongados, e que cedem com o repouso. No período teve quatro episódios mais intensos seguidos de síncope. Refere que apresentou quadro de dor retroesternal com irradiação para mandíbula que durou cerca de 12h, evoluindo após para episódio de síncope durante o trajeto para o hospital; deu entrada na emergência inconsciente e ao monitor apresentava taquicardia regular sendo prontamente cardiovertido com 200j, retornou para ritmo sinusal recuperando a consciência e referindo melhora da precordialgia. CLINICAL CASE - CASO CLINICO B UNIT - UNIDADE B Interrogatory and Physical •Patient without risk factors for CAD •He works with pesticides •He denies sudden death in his family background •Normal physical examination ----------------------------------------------------------------------------------------Interrogatório e Exame Físico •Paciente sem fatores de risco para DAC •Trabalha com agrotóxicos •Nega morte súbita na família •Exame físico normal Echocardiogram: Cardiac cavities with normal dimensions and thickness, LV: Hypocinesis on basal inferolateral and anterolateral walls. Preserved systolic function (LVEF: 58% by the Simpson's Method), Normal diastolic diameters, Normal flows across the cavities Holter: Sinus rhythm, isolated premature ventricular contraction, absence of pathological pauses, unaffected repolarization related to the basal ECG, no report symptoms Serology for Chagas’ disease: Negative Coronariography: Normal Functional class: I group Drugs in use: amiodarone associated to beta-blockers (asymptomatic) ECOCARDIOGRAMA: Cavidades cardíacas com dimensões e espessuras normais, hipocinesia dos segmentos médio-basal, ínfero-lateral e ântero-lateral do ventriculo esquerdo. Função sistólica preservada (FEVE: 58% pelo método de Simpson) Diâmetro diastólicos normais Fluxos normais HOLTER: Ritmo de base sinusal, extrasístole ventricular isolada, ausência de pausas patológicas, repolarização inalterada com relação ao ECG basal Ausência de sintomas reportados. Sorologia para Chagas: negativo Coronariografia: normal Classe funcional: I Fármacos em uso: amiodarona+ betabloqueador (assintomático) Dear friend Doctor Andrés Ricardo Pérez Riera, I am sending a new challenge regarding the diagnosis and therapeutic. This case was presented in clinical session having generated many doubts. 1. First question: Is it a fascicular ventricular tachycardia? In case positive would we indicate ablation? 2. Second question: Do by the fact of have presented Sustained Monomorphic Ventricular Tachycardia (S-MVT) with hemodynamic instability should indicate immediately the ICD implant? 3. Third question: Do this EPS exclude the Fascicular Ventricular Tachycardia diagnosis? I would like to hear your and the colleagues opinion. Hugs. Este caso foi apresentado em sessão tendo gerado muitas dúvidas. Primeira pergunta: Trata-se de Taquicardia Ventricular fascicular? Caso positivo indicaríamos ablação? Segunda pergunta: Pelo fato de ter apresentado Taquicardia Ventricular Monomórfica Sustentada (TVMS) com instabilidade hemodinâmica deveríamos indicar de imediato o implante de um Cardiodesfibrilador?
