Sie ist in fünf Größen verfügbar. 2. einer talaren Komponente (B), der Taluskappe, die in fünf Größen rechts und links vorliegt. In der Mitte der Gleitfläche befindet sich, A/P-verlaufend, eine Rippe zur Führung des Polyethylen-Gleitkerns. 3. einem Polyethylen-Gleitkern (C), dessen plane Oberfläche mit der Tibiagleitplatte und dessen konkave Unterseite mit der konvexen Taluskappe artikuliert. Die Führung des Gleitkerns A/P erfolgt durch die Längsrippe auf der Taluskappe und die korrespondierende Längsrinne auf der Unterseite des Gleitkerns. Die Gleitkerne sind in den Höhen 6, 7, 8, 9 und 10 mm verfügbar. Durch diese Konstruktion des Bewegungsmechanismus im S.T.A.R. Sprunggelenk, bei dem der Polyethylen-Gleitkern sowohl mit der tibialen als auch mit der talaren Komponente artikuliert, ist eine freie Rotation gegenüber der tibialen Komponente möglich, begrenzt nur durch die Malleoli. Damit werden Kräfte, die sich negativ auf die Verankerung der Komponenten auswirken können, neutralisiert. Die zementfrei implantierte Prothese besitzt eine Doppelbeschichtung aus Reintitan und HX® (Kalziumphosphat). Für spezielle Fälle können zementierte oder Hybridversionen (Taluskappe zementiert, Tibiagleitplatte zementfrei) indiziert sein. Das Implantations-Instrumentarium besteht aus einer Reihe von Bohr- und Sägelehren, Probeprothesen und wenigen anderen Hilfsmitteln, die eine exakt reproduzierbare Knochenresektion ermöglichen. Wegen der begrenzten Dimensionierung der S.T.A.R. Sprunggelenkprothese kann die Resektion knochensparend ausgeführt werden. 2 Description The LINK S.T.A.R.® (Scandinavian Total Ankle Replacement) Ankle Prosthesis consists of three components: A 1. A Tibial Component (A) with a highly polished flat articulation surface and two cylindrical fixation bars on the back side to anchor the implant in the subchondral bone of the tibia. Five sizes are available. C B 2. A Talar Component (B), also available in five different sizes for right and left. A ridge running anteroposteriorly in the middle of the gliding surface guides the UHMWPE Sliding Core. 3. A UHMWPE Sliding Core (C), the plane surface of which articulates with the Tibial Component while the concave underside articulates with the convex Talar Component. The anteroposterior articulation is guided by the longitudinal ridge on the Talar Component and the matching longitudinal groove in the underside of the Sliding Core. Sliding Cores are available in five heights: 6, 7, 8, 9, and 10 mm. Tibiagleitplatte/Tibial Component Polyethylen-Gleitkern/ UHMWPE Sliding Core Taluskappe/Talar Component The cementless prostheses feature a bioactive double coating of pure titanium and HX® (calcium phosphate). For special cases, cemented or hybrid versions (cemented Talar Component, cementless Tibial Component) may be indicated. The Instrument Set for implantation of the prosthesis consists of a set of Drill and Saw Guides, Trial Prostheses, and a few other auxiliary instruments that allow precise, reproducible bone resection. The small proportions of the S.T.A.R. Ankle Prosthesis mean that excessive bone loss is avoided. Indikationen und Kontraindikationen Indications and Contraindications Indikationen Indications Schmerzhafte und degenerativ erkrankte Sprunggelenke mit ausreichender Stabilität können mit der S.T.A.R. Sprunggelenkprothese versorgt werden. Painful ankles and ankles with degenerative conditions but with sufficient stability are indications for use of the S.T.A.R. Ankle Prosthesis. Relative Kontraindikationen bestehen bei: — vorangegangener tiefer Infektion im Sprunggelenksbereich oder in der Tibia — schwerer Osteoporose — sehr agressiver Arthritis, wie beispielsweise anhaltender psoriatischer Arthritis — Verlust des lateralen Seitenbandes, der nicht reparabel ist Relative Contraindications include: — previous deep infection in the ankle region or in the tibia — severe osteoporosis — very aggressive arthritis, such as persistent psoriatic arthritis — loss of lateral ligament that cannot be repaired. Absolute Kontraindikationen sind: — Talusnekrose (unter Berücksichtigung szintigraphischer Untersuchungen) — Charcot-Gelenk — neurologische Probleme, z. B. Nervenausfälle im Bereich des Fußes — Fehlen der Muskelfunktion im distalen Beinbereich — vorangegangene Sprunggelenksarthrodese unter Entfernung der Malleoli bzw. Insuffizienz des medialen und lateralen Bandapparates — tibiotalare Fehlstellung von mehr als 35° — patientenbedingte mentale Problematik, die eine komplikationslose postoperative Nachbehandlung nicht erwarten lässt Operationstechnik Zur präoperativen Planung müssen aktuelle Röntgenbilder in A/P und lateraler Projektion erstellt werden. Neue Röntgenbilder der subtalaren Gelenke sind ebenfalls nötig zur Abschätzung des Ausmaßes der subtalaren Degeneration. Eine Knochenszintigraphie wird vorgenommen zur Beurteilung der nachfolgenden Veränderungen im subchondralen Knochen des Talus und der Tibia. Sofern möglich ist auch die Erstellung einer kinematischen Gangstudie hilfreich. Empfehlenswert ist ein System zur Beurteilung von Schmerz, Funktion und Beweglichkeit. Absolute Contraindications are: — talus necrosis (as established by scintiscanning) — Charcot’s joint (neuropathic arthropathy) — neurological problems, e.g., loss of sensation in the foot region — absence of muscular function in the distal leg region — previous ankle arthrodesis with removal of the malleoli or medial and lateral ligamentous insufficiency — tibiotalar malposition of more than 35° — mental problems in the patient that make complicationfree postoperative follow-up care unlikely Surgical Technique For preoperative planning, new AP and lateral X-rays must be taken. New X-rays of the subtalar joints are likewise needed to estimate the extent of subtalar degeneration. Bone scintigraphy can be performed to assess subsequent changes in the subchondral bone of the talus and the tibia. A kinematic gait study is helpful. Der 5 mm Messblock B wird in den beweglichen Teil A eingesetzt und durch Drehen des Zahnradschlüssels bis zur tibialen Gelenkfläche vorgeschoben.* C Mittelstellung / middle position Mit der Arretierungsschraube D wird der Sägeblock horizontal ausgerichtet, um den von medial nach lateral bestehenden Höhenunterschied in der distalen Tibia auszugleichen. Der Messblock (4 mm, schräg) wird auf die Tibia Bohr- und Sägelehre geschoben. Der Fuß ist in 90°-Stellung fest an die Resektionsfläche der Tibia zu pressen. Bei der Talusresektion wird das Sägeblatt auf der Fläche des Messblocks geführt. Kann der Talus nicht gegen die Tibia gedrückt werden, bestehen zwei Vorgehensweisen: 1. Sagittale Schnitte von 1 mm von den medialen und lateralen Talusfacetten, 2. Verwendung eines Messblocks mit Distanzzunge (siehe folgende Seite) The 4 mm oblique Spacer is attached to the Tibial Drill and Saw Guide. Mit einem Markierungsstift (Abb. A), der an der vorderen Tibiakante geführt wird, wird in Achsrichtung eine Linie (Abb. B) auf dem resezierten Taluskopf gezogen. A B 9 Operationstechnik Surgical Technique Die passende Talus Bohr- und Sägelehre wird flach auf die resezierte Taluskopffläche platziert, so dass die tiefsten Punkte der beiden Ausnehmungen gerade auf der vorher angebrachten Linie liegen (siehe Abb. unten). Die Lehre wird mit zwei Drahtnägeln befestigt. Zusätzlich kann die Lehre nach Bohren eines Loches mit dem gebogenen Fixierstift durch die Bohrung in der Lehre in ihrer Position gesichert werden. Mit einer Stichsäge werden die seitlichen Resektionsschnitte durchgeführt, indem medial und lateral etwa 2 mm starke Lamellen reseziert werden. Medial sollte der Schnitt etwa 10 mm, lateral 17 mm tief reichen. N. B.: Die Sägelehre gibt eine schräge Schnittrichtung vor. The Talus Drill and Saw Guide of the correct size is positioned flat on the resected surface of the talar head, such that the apexes of its archshaped recesses align with the line previously drawn (see lower figure). The Talus Drill and Saw Guide is fixed with two Wire Pins. The Guide can be additionally secured by the curved Fixation Pin if an extra hole is drilled. The bilateral resection is then performed along the Guide using a reciprocating saw, taking off slices of approx. 2 mm laterally and medially. The cut should go down about 10 mm on the medial side and about 17 mm on the lateral side. N.B. The Saw Guide induces an oblique direction of cut. 10 Operationstechnik Surgical Technique Die Talussägelehre (entsprechend der vorher verwendeten Größe der Talus Bohr- und Sägelehre) wird zur dorsalen und frontalen Resektion mit der Haltezange auf den Talus aufgesetzt. Der Quersteg der Talus Sägelehre muss in der Mitte des resezierten Talus platziert werden. Bei korrekter Resektion der seitlichen Facetten erreicht man diese Position, indem die Lehre dorsal flach auf die Resektionsfläche aufgesetzt und bis zum Verklemmen nach ventral gezogen wird. In dieser Position wird die Talus Sägelehre mit Drahtnägeln (s. Abbildung unten) fixiert. Using the Insertion Forceps, the Talar Saw Guide corresponding in size to the Talar Drill and Saw Guide previously used is mounted on the talus to carry out the dorsal and frontal resection. Gegebenenfalls muss die frontale Resektion mit einem schlanken Meißel durchgeführt werden. The instrumentation includes a specially designed double-angled Sawblade for performing the frontal resection. In some cases, the frontal resection may have to be performed with a thin osteotome. Entsprechend seiner Größe wird die passende Fräslehre auf den Talus gesetzt und mit Drahtnägeln fixiert. Mit Hilfe dieser Fräslehre wird die Rinne für das Verankerungssegel der Taluskomponente in den Talus gefräst. Mit einem 3 mm Spiralbohrer mit Anschlag wird jeweils an den Enden des Schlitzes in der Talus Fräslehre ein Loch gebohrt und dann durch Gleitfräsen mit dem laufenden Bohrer der verbliebene Knochensteg durch den Schlitz in der Lehre abgefräst. Die gefräste Nut darf nur bis zum hinteren Rand des oberen Talusschnittes reichen, aber nicht weiter. The appropriate-sized Talar Milling Guide is placed on the talus and fixed with Wire Pins. The groove created should only reach as far as the back edge of the upper talar resection, but no further. 12 Operationstechnik Surgical Technique Vorbereitetes Implantatlager am Talus. B. mittels der Tibia-Einführungsplatte, nach dorsal bewegt. Erst dann (!) wird die Taluskomponente mit dem Einschlaginstrument eingeschlagen. Bei Zementfixierung ist dafür der Zement vorher in die Taluskappe einzubringen, um das Implantat dann in der korrekten Position zu den Resektionsflächen mit dem Einschlaginstrument für Taluskappen fest auf den Talus aufzuschlagen. Das Talussegel sollte nicht mit Zement belegt werden. The Talar Component is implanted. The fin of the Talus Component is placed in the groove. The Talar Component is then tapped backwards (dorsally) with light blows (e.g., using the Tibial Spacer Guide or similar instrument) against its front edge. Then, and only then, the Talar Component can be driven home using the Impactor. For cement fixation, the cement should first be applied to the Talar Component, which is then firmly seated on the talus with the Impactor. Cement should not be applied to the fin of the Talar Component. Die Tibia Bohr- und Sägelehre wird wieder auf die in der Tibia verbliebenen Drahtnägel gesetzt. Mit der Tiefenmesslehre wird die Ausdehnung der Tibia in der Sagittalebene definiert und auf den Spiralbohrer mit einstellbarem Anschlag übertragen (die Anschlagplatte am Spiralbohrer kann mit dem zugehörigen Inbusschlüssel verstellt werden). 5 mm werden von der gemessenen Länge abgezogen, um ein Durchbrechen der Kortikalis dorsal zu vermeiden. To avoid penetration of the posterior cortex, 5 mm is deducted from the measured distance. 13 Operationstechnik Surgical Technique Mit dem ∅ 6 mm Spiralbohrer, bei vorher eingestellten Tiefenmaß, (Anschlag/Bohrer) werden die Bohrungen für die zylindrischen Verankerungskomponenten an der Tibiagleitplatte über die Bohrlehre in die Tibia eingebracht. Zu beachten ist dabei, dass der Bohrer gerade durch die Bohrlehre läuft und die Bohrerspitze nicht in Richtung Tibiaresektionsfläche abweicht. Als Peilhilfe kann ein 6 mm Spiralbohrer wechselweise in das dem Meißel gegenüberstehende Loch eingebracht werden. The drill holes in the tibia are now opened anteriorly with a Special Gouge. Das operierte Bein soll dabei alternativ und unter Einbezug von Pausen belastet werden. Bei Anwendung von Knochenzement wird der Gips nach 14 Tagen entfernt. Bei zementfreier Arthroplastik muss der Gips vier bis sechs Wochen belassen werden. Nach Gipsentfernung sind Übungen, bei denen der Patient die Hockstellung einnimmt und die Fersen flach am Boden hält, sinnvoll. Auch Zehenstand sollte durchgeführt werden, um die Unterschenkelmuskeln zu stärken. Der operierte Sprunggelenksbereich kann für drei bis sechs Monate geschwollen sein. Während dieser Zeit ist das Auftreten von ziehendem Schmerz nach Übungen oder während des Abends üblich. Möglicherweise ist der Gebrauch elastischer Stützstrümpfe und die Hochlagerung des operierten Beins zu empfehlen. Das Ergebnis zwölf Monate postoperativ ist normalerweise von Dauer. Alle Patienten sind in fortlaufenden Serien zu registrieren, wobei die Dokumentationsbögen bei jeder Nachuntersuchung zur wissenschaftlichen Dokumentation und Erfolgskontrolle auszufüllen sind. Bis dahin nur konservative Behandlung. Nunmehr mit Schmerzen, Visual Analogue Score (VAS) 9 Punkte (max. 10 Punkte), und ein steifes Sprunggelenk. Kofoed Ankle Score 30 Punkte (max.100 Punkte). Preoperative status: A 68-year-old man with multiple distortions of his right ankle joint. So far, only conservative therapy. Patient now has pain – Visual Analogue Score (VAS): 9 points (max. 10 points) – and a stiff ankle joint. Kofoed Ankle Score: 30 points (max. 100 points). Röntgendiagnose zeigt Kippstellung des Talus, Varusstellung des Rückfußes, Adaptation des Malleolus medialis an der medialen talaren Gelenkfläche und alle Anzeichen unzureichender lateraler Ligamente. Aufgabenstellung: Wiederherstellung der Rückfußanatomie und des Talus sowie gleichzeitige Stabilisierung des Sprunggelenks. Dies stellt wegen der Rotationsfehlstellung des Talus und wegen der anatomischen Distorsion eine der schwierigsten Problemsitationen der Sprunggelenksarthroplastik dar. Vorgehen: Übliche distale Tibiaresektion. Medialisierung des Talus nach entsprechender Präparierung des Malleolus medialis. Danach wird der Rückfußkomplex in der Malleolengabel rotiert. Ist der Fuß in Neutralstellung, können die standardmäßigen Talusresektionsschnitte ausgeführt werden. Die Prothesenkomponenten werden implantiert. Danach wird die Peronäus brevis Sehne durch eine im Malleolus lateralis angebrachte Bohrung gezogen. An dem Austrittsende, anterior, wird die Sehne in zwei Teile gespalten. Ein Teil dient als Ersatz des Lig. talofibulare anterius. Der andere Teil – als Ersatz des Lig. calcaneofibulare – wird durch eine Bohrung durch die Spitze des Malleolus lateralis unter die Peronäussehnen geführt und am Calcaneus fixiert. Sprungelenk und Rückfuß sind nunmehr stabilisiert, so dass ein passender Gleitkern aus Polyethylen zwischen die Metallkomponenten der Prothese eingeführt werden kann. Die Röntgenbilder (Abb. 3 + 4) zeigen die Situation 4 Monate post-op. Nach einem Jahr liegt der Schmerz-Score VAS bei 0 Punkten, der Kofoed Ankle Score bei 98 Punkten. 18 Aim: To realign the hindfoot and ankle and simultaneously to stabilize the ankle joint by an arthroplasty. This is one of the most difficult situations to face in ankle arthroplasty, because of the malrotation of the talus and the distorted anatomy. Procedure: The normal distal tibial resection is performed. The talus is medialized by shaping the medial malleolus, then the entire hindfoot complex rotated within the ankle mortise. The foot in a neutral position, the normal talar resection cuts can be performed. The implant components are inserted. After this, the peroneus brevis tendon is taken through a drill hole in the lateral malleolus. At the outlet on the anterior side, the tendon is split into two sections. One is used as an anatomical substitute for the anterior talofibular ligament. The other is taken through a drill hole down through the tip of the lateral malleolus, under the peroneal tendons, and is fixed to the calcaneus as a substitute for the posterior calcaneofibular ligament. The ankle and hindfoot are now stable, and a polyethylene Sliding Core of appropriate size can be inserted between the metal components of the prosthesis.
