Icterícia neonatal: uma revisão bibliográfica Orientadora: Ms. Maria Luisa de Almeida Nunes RESUMO Tendo por finalidade estudar a icterícia neonatal, no plano de seus aspectos teóricos e práticos, envolvendo a conduta terapêutica e os procedimentos adotados pelo profissional de enfermagem, esse estudo considera que a icterícia neonatal é tida como sendo um evento frequentemente observado em recém-nascidos a termo e prematuros, resultante do aumento dos níveis séricos de bilirrubina total (BST). Reconhecendo que o problema do recém nascido ictérico é enfrentado diariamente pela equipe de neonatologia, esse estudo direciona sua argumentação para a seguinte questão problema: Que aspectos devem ser observados no diagnóstico e conduta terapêutica associados à icterícia neonatal? Assim sendo, o objetivo geral desse estudo volta-se para a busca de conhecimento em torno da problemática do neonato acometido por icterícia, tendo ainda como objetivos específicos os seguintes aspectos: identificar os tipos de icterícia no período neonatal; reconhecer os sinais e sintomas que possibilitam efetuar o diagnóstico da icterícia; descrever em que consiste as formas de atuação da enfermagem na terapêutica. Trata-se de um estudo de revisão bibliográfica, que descreve as intervenções de enfermagem com base na conduta terapêutica indicada, de modo a evitar que haja aumento da bilirrubina e danos irreversíveis ao sistema nervoso, prejudicando o desenvolvimento do neonato. Isto pode ocorrer se não houver possibilidade de intervenção feita por um profissional de saúde, podendo vir a tornar-se patológica e gerar graves complicações no sistema neurológico, pois o diagnóstico precoce e os cuidados direcionados ao quadro clínico reduzem as chances da ocorrência de danos irreversíveis. Os resultados encontrados possibilitaram concluir que a assistência de enfermagem aplicada ao neonato portador de icterícia é de fundamental importância tanto na fase de diagnóstico quanto na realização da conduta terapêutica, seja na aplicação da fototerapia, seja na aplicação da exsanguineotransfusão. ABSTRACT Aiming to study the neonatal jaundice, in terms of its theoretical and practical aspects involving the treatment administered and the procedures adopted by the professional nursing, this study finds that neonatal jaundice is seen as an event frequently observed in newborns term and premature infants, resulting in increased levels of of total serum bilirubin (TSB). Recognizing that the problem of newborn jaundice is faced daily by the neonatal team, this study directs its arguments to issue the following question: Which aspects should be observed in the diagnostic and therapeutic associated with neonatal jaundice? Thus, the general objective of this study turns to the pursuit of knowledge around the issue of neonates affected by jaundice, 15 and has, as specific objectives the following: to identify the types of jaundice in the neonatal period to recognize the signs and symptoms that make possible the diagnosis of jaundice; to describe the ways, which consists of nursing therapeutics. This is a review of the literature, which describes the nursing interventions based on therapeutic indicated in order to avoid increasing in bilirubin and irreversible damage to the nervous system, impairing the development of the neonate. This can occur if there is no possibility of intervention by a health professional which could become pathological and lead to serious complications in the neurological system, because early diagnosis and care directed to the clinical reduce the chances of the occurrence of irreversible. The results permit to conclude that nursing care applied to the neonate with jaundice is crucial to both the diagnosis and the realization of the therapeutic approach, either in the application of phototherapyor in the application of exchange transfusion.
