Auf der Anhöhe über Zürich in Richtung Forch steht die Klinik Hirslanden,
ein modernes Gebäude mit wunderschöner Aussicht auf die Stadt und
den See. Seit ein paar Monaten besitzt die 1932 gegründete Klinik ihre eigene,
mit hochmoderner Technik ausgerüstete Abteilung für Neurochirurgie.
The Hirslanden Clinic, a modern building with a beautiful view of the city
and the lake, stands on the hill above Zurich in the direction of Forch.
The clinic, established in 1932, has had its own department for neurosurgery
equipped with ultra-modern technology for a couple of months.

Vor eineinhalb Jahren wurde Prof. Kai-Michael Scheufler mit
dem Aufbau eines interdisziplinären Neurozentrums in Zürich
beauftragt. Dabei bestand die Möglichkeit, eine völlig neue Infrastruktur mit modernster integrierter Technik so zusammenzustellen,
dass einerseits etablierte Behandlungstechniken besonders schonend durchgeführt und andererseits auch völlig neue Verfahren
angeboten werden können. Die apparativ-technische Ausstattung
dieses neuen Kompetenzzentrums der Hirslanden Gruppe umfasst
einen vollintegrierten neurochirurgischen Operationssaal mit
hochauflösendem Computertomografen und Ultraschallgerät, die
wie viele andere Geräte direkt mit einem Navigationssystem verbunden sind. Ausserdem stehen eine vollausgerüstete Stereotaxieeinheit zur tiefen Hirnstimulation sowie modernste Mikroskope,
Endoskope und Instrumentarien zur Verfügung. Direkt gegenüber
dieser «BrainSuite» wird gegenwärtig ein weiterer Neurointerventionssaal mit modernster biplanarer Angiografieanlage für die
Neuroradiologie fertiggestellt. Somit können auch hochkomplexe
kombinierte Eingriffe durchgeführt werden.
Erst 40-jährig, hat sich Professor Kai-Michael Scheufler mit seinen
Beiträgen zur minimal invasiven Neurochirurgie bereits internationales Renommee erworben. Einige der im NeuroZentrum Hirslanden angebotenen Operationstechniken werden heute nur von

was the opportunity, in the process, to put together a completely new
infrastructure with the most modern integrated technology in such a
way that, on the one hand, established treatment techniques in particular can be carefully carried out and, on the other hand, completely new
procedures can be offered as well. The instrumental/technical equipment of this new competence centre of the Hirslanden Group entails
a fully integrated neurosurgical operating room with high-resolution
computer tomographs and ultrasound equipment, which are directly
linked to a navigation system like many other items of equipment. In
addition, a fully equipped stereotaxy unit for deep brain stimulation
as well as the most modern microscopes, endoscopes and instruments
are available. Another neurointervention room with the most modern
biplanar angiography facility for neuroradiology is currently being finished directly opposite this “brain suite”. Thus, highly complex combined operations can also be carried out.
Just 40 years old, Professor Kai-Michael Scheufler has already acquired
an international reputation with his contributions to minimally invasive
neurosurgery. Some of the operating techniques offered in the Hirslanden neurology centre are only carried out today by a few other centres

wenigen anderen Zentren weltweit durchgeführt. «Mit der Einführung der computergestützten Chirurgie hat unser Fach enorme
Fortschritte hinsichtlich der Behandlungssicherheit und Präzision
gemacht. Wir arbeiten auf einem Niveau, das noch vor einigen
Jahren in der klinischen Routineversorgung nicht vorstellbar war,
und dies kommt natürlich unseren Patienten sehr zugute», sagt
Scheufler, als er den Operationssaal verlässt, der den Laien eher an
die Kommandobrücke eines Raumschiffs erinnert.
Nach einer mehrstündigen Operation, bei der er eine degenerative Skoliose der Brust- und Lendenwirbelsäule minimal invasiv
aufgerichtet hat, findet er noch Zeit für Erläuterungen am Modell
einer Wirbelsäule. «Anstatt über die übliche Freilegung der Wirbelsäule werden die Schrauben und Stäbe nun durch kleine Hautschnitte eingebracht», erklärt der Arzt, «die Wirbelsäule selbst
bekommt der Chirurg dabei nicht mehr zu Gesicht: Er orientiert
sich lediglich anhand der Navigation sowie gelegentlicher Röntgenbilder, die während des Eingriffs gemacht werden.» Sowohl bei
der Implantation der Schrauben als auch bei den Operationen am

which is more reminiscent of the bridge of a spaceship to the layman.
After an operation lasting several hours, during which he straightened a degenerative scoliosis of the thoracic and lumbar spine with minimal invasion, he still finds time for explanations on the model of a spinal
column. “The screws and rods are now introduced through small skin
incisions instead of via the normal uncovering of the spinal column,”
the doctor explains, “and the surgeon no longer sets eyes on the spinal
column itself: he is guided only by means of the navigation system and
occasional X-ray images, which are taken during the operation.” Millimetres are involved both with the implantation of the screws and in the
operations to the brain and to the base of the skull. “The art consists of
achieving the objective of the operation safely at all times, particularly
as our apertures are becoming ever smaller. So small that we now use
the endoscope instead of the microscope on occasion in order to have

Gehirn und der Schädelbasis geht es um Millimeter. «Die Kunst besteht darin, das Operationsziel jederzeit sicher zu erreichen, zumal
unsere Zugänge immer kleiner werden. So klein, dass wir nun öfter
das Endoskop anstelle des Mikroskops benutzen, um durch minimale Öffnungen eine perfekte Sicht zu haben. Dass es dabei auch
eine Vielzahl spezieller Instrumente braucht, liegt auf der Hand»,
erläutert Scheufler.
Wie kann eine derartige Flut an teurer Hightecheinrichtung rentabel sein in einer Privatklinik mit rund 259 Betten? «Die Auslastung
der Geräte ist gut geplant, zumal es bei uns praktisch keine Wartezeiten gibt. Unsere Patienten können sich auf eine rasche Behandlung verlassen. Mit Hilfe computergestützter Verfahren besprechen
wir den geplanten Eingriff im Detail mit dem Patienten. Das übergeordnete Ziel der Operationsstrategie ist es, die Behandlung so
sicher und effektiv wie möglich zu machen.»
Ein weiterer Vorteil für die Ärzte der Hirslanden Gruppe, der
13 Privatkliniken und rund 100 Kompetenzzentren in der ganzen
Schweiz angehören, ist die Möglichkeit, mit anerkannten Kollegen
anderer Fachdisziplinen gemeinsam Eingriffe planen und durchführen zu können. «Wir arbeiten gerade im Bereich der Schädelbasischirurgie oft interdisziplinär», sagt Professor Scheufler, «dank
guter Kommunikation, flexibler Strukturen und eines Netzwerks
von Erfahrung und Wissen funktionieren wir oft besser, als es in
einem öffentlichen Spital möglich ist.» Mehrfach wurden auf diese
Weise Patienten behandelt, die zuvor als inoperabel galten und in
anderen Krankenhäusern nicht versorgt werden konnten.
Gute Leistungen allein reichen aber nicht aus. «Die Patienten
müssen auch wissen, dass es uns gibt», lacht Scheufler. Während
der Anlaufphase gehört es deshalb auch zur Aufgabe seines Teams,
das neue, in hellen, freundlichen Räumen untergebrachte NeuroZentrum bekannt zu machen. Das Ambiente, persönliche Betreuung, hochqualifiziertes Pflegepersonal und eine technische Ausrüstung auf internationalem Spitzenniveau liefern jedenfalls gute
Argumente. Um bei der rasanten Entwicklung der Spitzenmedizin
mitzuhalten, pflegt das NeuroZentrum Hirslanden in Zürich eine
enge Kooperation mit verschiedenen internationalen universitären

Scheufler.
How can such a torrent of expensive high-tech fitting-out be profitable in a private clinic with around 259 beds? «The utilisation of the
equipment is well planned, since we have practically no waiting times.
Our patients can rely on quick treatment. We discuss the planned operation with the patient in detail with the assistance of computer-aided
procedures. It is the overriding objective of the operation strategy to
make the treatment as safe and effective as possible.»
A further advantage for the doctors of the Hirslanden Group, to
which 13 private clinics and approximately 100 centres of excellence
belong in the whole of Switzerland, is the possibility to be able to plan
and carry out operations together with acknowledged colleagues of
other specialist disciplines. “We often work with an interdisciplinary
approach, particularly in the area of skull base surgery,” says Professor
Scheufler, «we often function better than is possible in a public hospital
thanks to good communication, flexible structures and a network of
experience and knowledge.» Patients, who were regarded as inoperable
previously and could not be looked after in other hospitals, have been
treated in this way repeatedly.
However, good achievements alone are not sufficient. “The patients
must also know that we are here”, Scheufler laughs. During the startup period, therefore, it is also one of his team’s tasks to publicise the
new neurology centre, accommodated in bright, friendly premises. The
ambience, personal care, highly qualified nursing staff and technical
equipment at a top-class international level provide good arguments at
any rate. In order to keep pace with the rapid development of top-class
medicine, the Hirslanden neurology centre in Zurich maintains close
collaboration with various international university centres. •
